New trends in the application of electron ionization to liquid chromatography-mass spectrometry interfacing.

I. Introduction 88 II. Cap-EI Interface 90 A. Interface Performance 92 III. Direct-EI Interface 93 A. Interface Performance 97 IV. Conclusions 103 Acknowledgments 103 References 103 --Two recent approaches for coupling capillary scale liquid chromatography and electron ionization mass spectrometry are reviewed and discussed. The first one, Cap-EI, is the latest evolution of the micro-scale particle beam interface, in which the nebulizer has been optimized to overcome the limitations of the former approach, in terms of sensitivity and linearity. It can be easily hosted in pre-existing instruments without major modifications and can use helium and the less-expensive nitrogen to generate library-matchable electron ionization spectra. The second one is a miniaturized interface for nano- and micro-HPLC, in which the interfacing process takes place into a suitably modified ion source. Because the eluate from the column is completely transferred into the ion source for ionization, superior sensitivity, linearity, and reproducibility are obtained. No signs of chemical ionization are observed at flow rates up to 1.5 microL/min. These two interfaces demonstrate that electron ionization can be successfully used for the analysis of small-medium molecules of various polarities, and also at the trace level. The possibility to record library-matchable electron ionization spectra offers the analyst a powerful tool that can be particularly useful in real-world applications.

Herpetic whitlow as a harbinger of pediatric HIV-1 infection.

Herpetic whitlow applies to a primary or recurrent herpes simplex infection of the fingers. It is a rare condition in immunocompetent patients, and usually has a rapid evolution and prompt response to treatment. We report a 10-year-old girl referred to our department of infectious diseases with recurrent and unresponsive ulcerated and necrotic lesions of the fingers of both hands as a first sign of human immunodeficiency virus infection. Numerous instances of extensive or recurrent herpes lesions of the mouth, lips, and perianal areas have been found in the literature, but we have not found any report of periungual infection as a first manifestation of human immunodeficiency virus infection. Pediatricians and dermatologists need to be aware of this association in order to make an early diagnosis and start treatment of these patients' disease.

[Carbon dioxide laser uvulopalatopharyngoplasty (UPPP). Part I. Surgical technique].

Snoring should not be regarded merely as a socially disruptive nuisance, but also as a potential risk factor of increased morbidity and mortality. Since numerous therapeutic approaches have been developed to treat snoring, the physician is faced with the dilemma of deciding which of the many possible treatments is the best for each snorer. Snoring is a sign of partial occlusion at the oropharyngeal level. Increased nasal airway resistance (NAR) may induce pharyngeal inspiratory collapse. Surgical treatment should be directed towards hindering pharyngeal collapse by surgical resection of redundant oropharyngeal tissue and/or surgical normalization of nasal resistance. Eighty-three consecutive patients suffering from snoring as major complaint together with nasal stuffiness were included in the study. Patients with OSA were excluded from the study. Increased nasal airway resistance (NAR) was a common finding in our population. Nasal surgery, involving both the external pyramid and nasal cavity, aimed at normalizing NAR was performed in 63 patients. In 15 cases, UPPP was performed contemporaneously or as a second surgical step. In 5 patients with no nasal abnormalities, pharyngeal surgery was the only surgical procedure. Personal variation of the classic UPPP is illustrated step by step in Part I. The procedure is carried out using the carbon dioxide laser, which offers several advantages both to surgeon and patient. The modified technique, which is associated with minimal morbidity, appears easy to standardize and successful in treating snoring. Results of the surgical procedures (nasal surgery and/or carbon dioxide laser UPPP) performed to treat snoring are reported in Part II.

[Postprandial and postural dyspnea: a clinical sign of intraperitoneal pseudocyst in patients with hydrocephalus and ventriculo-peritoneal shunt].

The surgical treatment of hydrocephalus has been greatly improved by the techniques of ventriculo-peritoneal shunting. Several complications may however occur following these operative procedures. Most of the complications are related to the distal end of the shunt device and include obstruction of intraperitoneal catheter, development of inguinal hernia or hydrocele, perforation of viscera. Many observations have been dedicated to these pathological findings, due to their frequency and the relatively easy diagnosis. Intraperitoneal pseudocysts are also a known complication of ventriculo-peritoneal shunts. Owing to their relative rare incidence and the aspecificity of their clinical presentation, this last type of complication has received a minor consideration. In the present report the authors describe two cases of intraperitoneal pseudocysts clinically manifested by the occurrence of postprandial dyspnea and hiccupping, without any apparent sign of CSF shunt device dysfunction.
