Study of micturition parameters in healthy young adults using a uroflowmetric method.

Using the 21 F 01 DISA Mictiograph, the instant urine flow curves of fifty normal males and thirty-three females (aged from 18 to 35 years) were analysed. A total of 363 micturitions was obtained (146 in the female group, 217 in the male group). Ten parameters were derived from each of the urine flow curves. These parameters were: the voided volume, the micturition time, the average-flow rate, the flow at 2 s, the maximum flow rate, the percentage of difference between the maximum flow rate and the average-flow rate, the duration until the maximum flow rate, the initial slope of the curve, the slope at the time from micturition initiation of voiding to maximum flow, and the difference between the two slopes. This study demonstrated that the urine flowmeter allowed, even at very low voided volume, a complete interpretation of micturition flow curves, a finding which might be useful in paediatric patients. It was also shown that the flow parameters were significantly more favourable in females than in males, and consequently that better hydrodynamics conditions for micturition exist in women compared to men.

[Echographic aspects of biliary lithiasis and its complications].

Ultrasonic appearances of various types of gallbladder stones are described and findings on ultrasonic imaging of the main complications of biliary lithiasis analyzed. New possibilities for examination of the principal biliary pathway are being opened up by progress in improving resolution of the ultrasonoscope and by advances in knowledge of ultrasonic anatomy of the biliary tracts.

[Isoprinosine treatment of herpes zoster. Résults of a control study in 36 subjects (author's transl)].

A double-blind trial comparing isoprinosine with a placebo was conducted in 36 subjects. Improvement of skin lesions was significantly better in the treatment group (p less than or equal to 0.01). Before entering the trial, patients in the treatment group had significantly more severe pain than those in the placebo group (p less than or equal to 0.01), the difference disappeared after treatment. Healing of skin lesions was more rapid in patients treated before the 5th day of the disease (p less than or equal to 0.03). None of the subjects developed post-zoster neuralgia. The clinical effects of the drugs were accompanied by a decrease in serum rosette activity. We consider isoprinosine as an innocuous and effective treatment of herpes zoster.

Possible link between changes in plasma 24,25-dihydroxyvitamin D and healing of bone resorption in dialysis osteodystrophy.

Histomorphometric studies of bone biopsies were performed on 12 hemodialyzed patients before and after six months of treatment with 25-(OH) and 1 alpha-(OH) vitamin D3. Patients could be classified into three groups according to bone resorption: Group I with normal bone resorption; Group II with elevated initial bone resorption unresponsive to vitamin D treatment; group III with elevated initial bone resorption sensitive to vitamin D treatment. None of the patients had histological signs of osteomalacia. In Group I, plasma concentrations of 24,25-(OH)2D and the ratio of 24,25-(OH)2D to 25-(OH) D remained in the normal range throughout the study; in Group II these parameters were subnormal initially and did not increase above normal except in one case; in Group III, plasma concentrations of 24,25-(OH)2D were high before or at the beginning of vitamin D administration and normal at the time of the second biopsy and wide variations were observed in the ratio of 24,25-(OH)2D to 25-(OH)D. No difference was found between these last two groups with regard to the cumulative dose of vitamin D derivatives administered or the changes in plasma PTH, CT, calcium and phosphate. These observations suggest a specific regulation of plasma 24,25-(OH)2D concentrations in hemodialyzed patients and a possible link (independent of circulating PTH, CT, or phosphate) between this regulation and healing of bone resorption. However, no correlation was found between plasma 24,25-(OH)2D and either one of the simultaneously measured biochemical or histological parameters.

Urodynamics of neurogenic bladder in children. A comparative study with clinical and radiological conclusions.

The clinical, radiological and urodynamic investigations of 83 children are presented. Neuropathic bladders were divided into 2 main categories: those without outflow obstruction (36 patients) and those with outflow obstruction (47 patients). Although clinical and radiological findings proved sufficient for the management of these patients, the urodynamic investigations led to a precise diagnosis of the type of neuropathic bladder, an understanding of the mechanism of obstruction and, during follow-up, demonstrated the appearance of obstruction before this was shown radiologically.

[Radiologic examination of the knee and ankle in stress positions (author's transl)].

The serious serious sequelae of sprains of the knee and ankle makes it imperative for the therapist to have an early and precise diagnosis. The authors propose a means of exploring these joints with the aid of a simple set up. This new technology allows the radiologist, secure from direct or indirect radiation, to carry out a standardized examination of the joint, usually unilaterally, and to make his interpretation more easily and more precisely. This method of x-raying in the stress position has its major indication in emergencies, but is equally useful in non emergencies, accomplishing a veritable over-all evaluation of the joint: varus, valgus, preternatural anterior and posterior sliding motion. The interpretation, based on direct reading of the films, differentiates the normal condition from physiological relaxation and mild and severe sprains. Naturally this method is to be used only after taking standard routine views to rule out any fractures.

Epidermal prostaglandins after topical application of a tumor promotor.

The topical application of the promoter, 12-O-tetradecanoyl-13-acetate, on the shaved backs of mice was attended by a striking increase in the PGE concentration within epidermis as early as 1 h later. This increase peaked at 24 h but the prostaglandin value remained elevated for at least 48 h. PGF, on the other hand, showed only a modest increase in epidermis with a maximum at 12 h. By 48 h, the PGF concentration returned to normal.

Motor activity of the sigmoid colon in chronic constipation: comparative study with normal subjects.

Manometric studies of the sigmoid colon were performed on 17 healthy volunteers and on 49 constipated patients, after a long period of fasting (18--20 hours). Motility was recorded using perfused catheters at basal level during 45 minutes, then 60 minutes after a 0.5 mg intravenous injection of neostigmine, and, finally, 30 minutes during and after a meal. Motor activity was assessed by a motility index (per cent of activity x mean amplitude of waves). In both normal and constipated patients, the basal motility index was very low (respectively 82 +/- 16 and 110 +/- 113). This low level of activity was due to the long fasting period imposed on all the subjects. After neostigmine the motility index increased in both controls (347 +/- 256) and constipated patients (311 +/- 325); this test, however was found to be unreliable. The meal increased the motility index to significantly higher values than after neostigmine in controls (538 +/- 215). In constipated patients the mean meal motility index was comparable with that of controls (577 +/- 549) with a large distribution of individual values. Using the mean meal motility index +/- 2 SD of the control group as a term of comparison, the patients were segregated into three groups: 'hypomotor' patients (eight cases), 'normomotor' patients (33 cases), and 'hypermotor' patients (eight cases). From the evidence of this series of clinically well-defined constipated patients, it was concluded that only the meal test is able to segregate three significant patterns of sigmoid activity and that a large number (68%) of constipated patients exhibit normal sigmoid motor activity.

[The hypocalcemia test, using human and salmon synthetic calcitonins. Paradoxical hypercalcemic responses. Responses in patients with Paget's disease treated with EHDP].

The hypercalcemia observed during the acute calcitonin test reflects the size and the activity of the osteoclastic population throughout the entire skeleton. Several calcitonins of animal origin (pork, salmon) have already been used for this test in human pathology, but the results can be flawed by the presence of anti-calcitonin antibodies. The authors demonstrate that human synthetic anti-calcitonin in man has a hypocalcemia effect identical to salmon synthetic calcitonin, with an equipotential dose for the rat. The systematic study of acute salmon calcitonin in various osteopathies makes it possible to note a certain number of paradoxical responses with prolonged hypercalcemia in the hours following injection. This is observed especially in the "hyperosteoidosis states" and the authors attempt to give it a physiopathological explanation. Finally, the acute salmon calcitonin test can be used as a mean of surveillance of the anti-osteoclastic activity of the disphosphonates in the treatment of Paget's disease.
