Two derivatives of lithium isodicyclopentadienide: [(1,2,3,3a,7a-eta)-4,5,6,7-tetrahydro-4,7-methanoindenido] (N,N,N1,N1-tetramethylethylenediamine)lithium and Bis (1,4,7,10-tetraoxacyclododecane)-lithium(1+) Bis[(1,2,3,3a,7a-eta)-4,5,6,7-tetrahydro-4,7-methanoindenido]lithate(1- ).

Selective crystallization of a solution of lithium isodicyclopentadienide, (isodiCp)Li, in dry thf or diethyl ether under argon has produced two lithium complexes: (isodiCp)Li(TMEDA), [Li(C10H11)(C6H16N2)], (4), and [Li(12-crown-4)2]+. [Li(isodiCp)2]-, [Li(C8H16O4)2]-[Li(C10H11)2], (5). In (4) the Li+ ion is coordinated to the two N atoms of the disordered TMEDA and is eta 5-coordinated to the Cp ring of the isodiCp ligand. The Li-(Cp ring centroid) distance is 1.906 (7) A. In (5) there are two independent half-molecules of the anion and one molecule of the cation in the asymmetric unit. In each anion, the Li+ ion occupies a crystallographic inversion center and is eta 5-coordinated to the two Cp rings of two isodiCp ligands. The Cp rings are in a staggered arrangement, as required by the inversion center. The Li-(Cp ring centroid) distances for the two anions are 1.987 (3) and 2.008 (3) A. In the cation of (5), the Li+ ion is coordinated to two 12-crown-4 ligands, one of which is disordered. Both (4) and (5) exhibit exo coordination of the Li+ ion to the isodiCp ligand, with a resultant slight endo bending of this ligand.

Hyaluronic acid in progressive systemic sclerosis.

BACKGROUND: The glycosaminoglycans metabolism is disturbed in progressive systemic sclerosis (PSS). Serum hyaluronic acid (HA) is elevated in this disease. OBJECTIVE: This study was conducted to determine the HA plasma concentrations of patients with PSS according to the different stages of the disease. METHODS: We studied 48 patients divided into three subgroups: subgroup 1 (n = 10), with skin compromise without evidence of other organ involvement; subgroup 2 (n = 21), with skin and esophagus involvement; subgroup 3 (n = 17), with skin, lung and other internal organ involvement. A radiometric assay was performed for quantification of HA. RESULTS: Our results confirm the increase in plasma HA in patients with PSS. They also suggest that lung involvement is the main feature responsible for high plasma concentrations of HA. The plasma HA levels were elevated in patients compared to normals (p <0.001). Significant differences were observed between subgroups 1 and 3 (p <0.01) and between subgroups 2 and 3 (p <0.01). A positive correlation between disease severity scores and plasma HA values was observed (p <0.01). CONCLUSION: An important elevation of HA plasma levels could be a serologic marker of disease severity, progression and degree of visceral involvement.

Cutaneous reactions after treatment with 2-chlorodeoxyadenosine.

2-chlorodeoxyadenosine (2-CdA) is a new purine analogue which has shown great efficacy in the treatment of hairy cell leukemia. Only one case of cutaneous reaction after this treatment has previously been reported. The data from 33 patients treated with 2-CdA were retrospectively reviewed. Seven of these (21%) developed a disseminated eruption during the month following 2-CdA. One had toxic epidermal necrolysis. A reaction to the associated antibiotic therapy seemed "likely" or "very likely" in 5 out of the 7 cases. The incidence of adverse drug reactions may be increased after 2-CdA, and the role of CD4+ lymphocytopenia in these reactions is discussed.

Prevention of hip fractures by correcting calcium and vitamin D insufficiencies in elderly people.

For a 50-year old caucasian woman today, the risk of a hip fracture over her remaining lifetime is about 17%. Tomorrow the situation will clearly be worse because the continual increase in life expectancy will cause a 3-fold rise in worldwide fracture incidence over the next 60 years, particularly in women, but also in men. In addition, a secular increase in the incidence of hip fractures in individuals of the same age has been noted in both sexes by several investigators, and the cost of hip fractures is expected to dramatically increase in the next decades. Consequently, preventive strategies are urgently required. A great deal has been learned in recent years about the risk factors for hip fracture, the pathophysiology of this fracture, and the prediction of fracture risk, particularly through bone mass measurements on the hip and biochemical evaluations of parathyroid and vitamin D status. The two main determinants of hip fractures are falls and bone loss leading to an intrinsic femoral fragility. A substantial femoral bone loss continues throughout the old age, with a continuous and exponential increase in the risk of hip fracture, and any reduction or arrest of this loss will induce an important reduction in the incidence of hip fractures. A preventive effect on the risk of hip fracture may be partly achieved by using long term estrogen replacement therapy after menopause, but also by using vitamin D and calcium supplements for a late prevention in elderly people. Vitamin D insufficiency and deficit in calcium intake are very common in elderly people living either in institutions or at home, particularly in Europe where dairy products are not fortified with vitamin D. The cumulative response to this deficit in calcium intake and low vitamin D status is a negative calcium balance which stimulates parathyroid hormone secretion. In 300 residents of nursing homes, we recently found a significant negative correlation between serum 25 OHD and log serum PTH after age-adjustment. In addition, in 446 elderly women living at home in 5 French cities and selected from the voting lists, we also found an age-adjusted relationship between serum 25 OHD and PTH concentrations. This senile secondary hyperparathyroidism is one of the determinants of femoral bone loss and can be reversed by calcium and vitamin D supplements. We have shown in a 3-year controlled prospective study that the daily use of these supplements (1.2 g of calcium and 800 IU of vitamin D3) given in a large population of 3270 elderly ambulatory women living in nursing homes reduced of 23% (intention-to-treat analysis) the number of hip fractures and other non vertebral fractures. In parallel, serum perathyroid hormone concentration was reduced of 28% and low serum 25-hydroxyvitamin D concentration returned to normal values. After 18 months of treatment the bone density of the total proximal femoral region had increased 2.7% the vitamin D3-calcium group and decreased 4.6% in the placebo group (p < 0.001). This prevention is safe and can be recommended in people living in institutions. It could be also useful in other elderly subjects particularly at risk because of a low calcium intake, an absence of solar exposure and a previous history of falls. From the data of our study we assessed the economic consequences in terms of medical cost of this prevention. In case of treatment of all women living in nursing homes in France, this would saved FF 150000000 per year, the economic balance of prevention becoming positive as soon as the age of the beginning of the prevention reaches 73.5 years. It is now possible to partly stop bone loss in elderly people and it is never too late to prevent hip fractures with calcium and vitamin D supplements.

[Diffuse normolipidemic plane xanthomatosis and monoclonal dysglobulinemia. A case with anti-lipoprotein activity of monoclonal immunoglobulin and hypocomplementemia].

INTRODUCTION: Diffuse normolipaemic plane xanthomatosis associated with monoclonal dysglobulinaemia has been recognized as a clinical entity for more than 30 years. Antilipoprotein activity has been reported in certain cases with monoclonal immunoglobulinaemia or hypocomplementaemia. CASE REPORT: A 64-year-old woman developed simultaneously diffuse normolipaemic plane xanthomatosis and monoclonal IgG kappa dysglobulinaemia. This paraprotein was initially associated with monoclonal gammapathy of undetermined signification (called "benign") before evolving into a myeloma. This aggravation was associated with an expansion of the skin lesions. Immunological exploration demonstrated the presence of IgG-lipoprotein complexes in the plasma and hypocomplementaemia suggesting incomplete activation of the classical pathway (low CH50 and fraction C4). DISCUSSION: According to the literature, the most probable hypothesis for the pathogenesis of the xanthomatosis-dysglobulinaemia association is a specific interaction between the monoclonal immunoglobulin and lipoprotein metabolism. In normolipaemic forms, the immunoglobulin-lipoprotein complexes are recognized by "waste-receptors" and accumulate in the dermal macrophages while the mechanism of lipoprotein captation in the other types of cells remains normal, explaining the absence of hyperlipidaemia.

[Urodynamic exploration in functional micturition disorders in children].

Urodynamic investigation in paediatric patients is presented. The practical aspects and usefulness of each method are discussed in the cases of functional micturition disorders based upon the most significant results from the literature and the author's experience. Urodynamic investigation in children provides a pathophysiological explanation of bladder-sphincter dysfunction and guidelines for appropriate treatment.

[Enuresis and benign micturition disorders in childhood. I. Diagnosis and management].

Bedwetting is present in 5 to 7% of children aged 7 to 8 years. The history of the disorder and the examination of the child are of main importance. This is usually the first step to identify nocturnal enuresis, bladder or urethral instability and other voiding dysfunctions. Therapeutic failure is often related to an inadequate analysis of the disorder. For nocturnal enuresis, the best results are obtained with alarms and/or desmopressine; bladder instability usually requires oxybutinine chlorydrate and urethral instability can be treated with biofeedback therapy. The management of other voiding dysfunctions depends on urodynamic assessment.

Adenosine deaminase in progressive systemic sclerosis.

The aim of the present study was to confirm the increase of adenosine deaminase (ADA) activity in progressive systemic sclerosis (PSS), described by Sasaki & Nakajima, and to compare plasma ADA activity of patients in different stages of the disease. Enzyme activity was measured with a colorimetric assay. The 48 patients were subdivided into 3 groups: subgroup 1 (n = 10), disease still limited to the skin; subgroup 2 (n = 21), involvement of the skin and oesophagus; and subgroup 3 (n= 17), involvement of the skin and multiple internal organs. ADA levels were highest in subgroup 3. However, the difference with respect to subgroup 2 did not reach statistical significance. Subgroup 1 was different from controls and subgroups 2 and 3 (p<0.001). Our results confirm that ADA activity is increased in PSS, and that this finding is observed even in the early stages of the disease process. We speculate that the increase in ADA, a well-known marker of T-cell activation, might be an indicator of disease activity in PSS, in the beginning as well as during phases of exacerbation in later stages of the disease.

Small bowel enema and diagnosis of chronic nonischemic disturbance of superior mesenteric venous blood flow.

Chronic nonischemic disturbance of mesenteric venous blood flow is reported in 11 patients with a mean age of 19 years. This entity, rarely discussed in the literature, is different from acute thrombosis and chronic thrombotic forms with portal hypertension or hypercoagulopathy. In eight patients this syndrome was secondary to organic lesions of different origin: mesenteric vein squeezed by fibrous bands or an abnormal jejunal artery (four cases), lymphoma involving the distal superior mesenteric veins (three cases), hemangioma causing microthrombi (one case). In three patients no etiology or predisposing factor was found. All patients presented with rectal hemorrhage. Small bowel enema showed a constant pattern in 11 patients: small nodules, modified by compression or peristalsis, involving the mesenteric border of the jejunoileal segment, and associated with thick, straight but regular folds. Mesenteric varices were suspected and led to angiographic studies which were normal in three cases, confirmed varices in eight cases, and thrombosis in four cases. Laparotomy was normal in three cases and established the etiological diagnosis in eight cases. Varices were shown in six cases. Arteriography and laparotomy were unable to reach a complete diagnosis.

[Diffuse cholesterol embolism after fibrinolysis for myocardial infarction].

The authors report the case of a 57 year old man admitted to hospital 3 hours after the onset of an infero-latero-basal infarct who underwent thrombolytic therapy with intravenous streptokinase, in whom the immediate outcome was complicated by multiple cholesterol embolisation which cause renal failure, mesenteric ischemia, medullary ischemia and livedo reticulosis of the lower half of the body complicated by gangrene of the toes, despite the fact that cardiac catheterisation had not been performed. This case (fifth reported case) of multiple cholesterol embolisation from the abdominal aorta caused by thrombolysis is the earliest presentation of this complication in the literature.

Influence of a new vapor protective clothing layer on physical work tolerance times at 40 degrees C.

The purpose of the present study was to examine the influence of a new vapor protective clothing on physical work performance in a hot environment (40 degrees C and 25% relative humidity). Eleven unacclimatized males (28 +/- 6 years, 79 +/- 8 kg, 1.76 +/- 0.06 m) were assigned to exercise at either a light intermittent (L) (N = 6), or heavy continuous (H) (N = 5) metabolic rate. Group L alternated between 15 min of walking on a treadmill at 1.11 m.s-1 with a 0% grade and 15 min of rest. Group H walked continuously at 1.33 m.s-1 with a 3% grade. Subjects were tested wearing three clothing configurations: full nuclear, biological and chemical (NBC) protection (TOPP High) with the combat clothing worn under the NBC garment (TH + CC); full NBC protection without combat clothing (TH - CC); the new vapor protective clothing together with the NBC gloves, boots, and respirator (NPC). WTT was the time-period until rectal temperature (Tre) reached 39.3 degrees C, heart rate reached 95% maximum, dizziness or nausea precluded further exercise, or 3 h had elapsed. For group L, WTT was similar for TH + CC (113 +/- 12 min) and TH - CC (139 +/- 18 min). WTT was significantly increased for NPC where all subjects completed the 3 h in the climatic chamber. The rate of increase for Tre was significantly reduced for NPC (0.3 +/- 0.1 degree C.h-1) compared with both TH + CC (0.9 +/- 0.1 degree C.h-1) and TH - CC (0.8 +/- 0.2 degree C.h-1).(ABSTRACT TRUNCATED AT 250 WORDS)

High and intermediate imperforate anus: functional results and postoperative manometric assessment.

Between 1983 and 1989, 21 patients with high (12) and intermediate (9) imperforate anus were operated on according to the author's procedure. The anatomical results are satisfactory (20/21). The records of the clinical and paraclinical assessments of 13 of these patients with a follow-up of at least 5 years, is reported. The patient history demonstrates that 12 out of 13 are clean at night, with 1 to 3 bowel movements during the day. The intermediate forms have no accidental defecation and almost no soiling. Conversely, the high forms often soil but usually do not wear thick protections. Criteria of successful reconstruction as rectal examination, defecogram, CT scan or MRI, yield limited data but no formal conclusions. Manometric studies provide a better understanding of the bowel function after pull-through procedures, by studying the rectoanal inhibitory reflex threshold (RAIRT), the maximal anal resting closure (MARCP), the conscious rectal sensitivity threshold (CRST) and the maximal rectal compliance (MRC). Technical aspects of these recordings are detailed. Manometric recording from 13 of these 21 patients show that RAIRT is present in 3 out of 6 intermediate forms and 3 out of 7 high forms. CRST is normal in all the cases except 1. MARCP was normal in 5 out of 6 intermediate lesions and 5 out of 7 high lesions. MRC is good in 5 intermediate forms and 3 high forms.(ABSTRACT TRUNCATED AT 250 WORDS)

[The hypersensitive bowel].

The irritable bowel syndrome is above all a syndrome of intestinal pain. Although the intestinal disorders described in this syndrome have prompted several studies focused on intestinal motility, little has been learned from these studies increasing our knowledge on the pathophysiology of this syndrome. The demonstration of colonic hypermotility or various and slightly significant modification in small intestinal motility do not add much to our knowledge. For one, why are clinical signs such as abdominal pain, bowel movement disorders, and abdominal distension, and most likely other motor disorders, found in numerous normal subjects (14 to 30 percent of the normal population) who do not seed medical advice for intestinal signs which are, one must admit, not very alarming? Are patients who complain of functional digestive tract disorders, constantly seeking medical advice and heavy medication consumers, mentally ill (emotional patients, hypochondriacs, depressive, hysterics), are they just under great stress, or do they indeed have chronic pain pathology? A number of studies show clearly that the last hypothesis is most likely true: patients with the irritable bowel syndrome (i.e. hypersensitive bowel) have a chronic pain pathology because their threshold perception of pain is lower than in the normal population. The threshold tolerance to distension of the pelvic colon is lower in these patients than in asymptomatic patients. The gastric transmural potential is lower in patients complaining of functional intestinal disorders, and it is known that a fragile mucosa is highly sensitive to normally innocuous stimuli.(ABSTRACT TRUNCATED AT 250 WORDS)

Preservation of infralevator structures in imperforate anus repair.

In this report, we describe the most recent refinements to the anterior perineal approach originally described by Mollard for the repair of high and intermediate forms of imperforate anus. A skin tube is constructed from perineal skin, passed through the center of the external sphincter, and anastomosed to the fully preserved rectum with or without tapering. The anastomosis lies within the puborectalis sling and is anchored to the striated muscle complex. Sixteen patients have been operated on by this technique with excellent anatomic results in 15 patients and excellent early functional results in patients who are 2 years of age or older.
