Granular cell myoblastoma: rare localization in the trachea. Report of a case and review of the literature.

Granular cell myoblastoma is an uncommon tumor in the respiratory tract. It usually occurs in the tongue, skin, breast, or subcutaneous tissue. When it occurs in the respiratory tract, it is usually located in the bronchus or larynx. Primary tracheal location is rare with only nine such cases reported in the literature. This report describes a 26-year-old woman with granular cell myoblastoma of the trachea. She presented with a four-year history of bronchial asthma. The tumor was surgically excised by tracheal resection and reconstruction. The patient has remained well and free of obstructive airway symptoms, without recurrence of the tumor for more than one year.

Treatment of common and plantar viral warts with human lymphoblastoid interferon-alpha--pilot studies with intralesional, intramuscular and dermojet injections.

Pilot studies with human lymphoblastoid interferon-alpha(Wellferon), utilizing the intralesional, intramuscular and dermojet routes, have been performed in 39 patients with common and plantar viral warts resistant to conventional therapy. Clinical efficacy, local and systemic toxicity, and effects on haematological and clinical chemistry parameters has been measured. Our results indicate that the intralesional route is the most favourable as it combines an acceptable therapeutic ratio with a procedure that may be performed quickly and with no prior preparation of the lesion.

Aldosterone studies in obese patients with hypertension.

Ten patients with hypertension and obesity were studied during a program of weight loss on an unrestricted sodium diet. The study showed that weight loss during the ten month period was accompanied by a significant decrease in urinary aldosterone, tetrahydroaldosterone -3-glucuronide and plasma renin activity values. It was also demonstrated that successful reduction in body weight was associated with a reduction in blood pressure. It is postulated that blood pressure reduction in obese patients during weight reduction may depend on decreases in aldosterone and plasma renin activity. The reduction in levels of tetrahydroaldosterone-3-glucuronide is due to an energy related depression of the glucuronidation process caused by carbohydrate deficiency. It is postulated that the lowered rate of metabolism of aldosterone as measured by the diminished formation of the metabolite tetrahydroaldosterone-3-glucuronide is an important determinant of blood pressure status during weight reduction.

Twenty-four hour combination chemotherapy: a feasibility study with implications for improved adjuvant treatment of breast cancer.

Forty-three patients with metastatic breast cancer were treated with a total of 385 cycles of combination chemotherapy consisting of adriamycin, cyclophosphamide, 5-fluorouracil, methotrexate and vincristine sulphate given over 24 hr and followed by a leucovorin 'rescue'. Thirty patients (70%) responded with three complete remissions. Thirteen patients did not respond, including six in whom the progression of disease was apparently arrested. Duration of response ranged from 2 to 24 months. At 20 months, 10 of 30 responding patients were alive compared with 1 of 13 non-responders. Toxicity was minimal apart from nausea and vomiting. This study confirms previous reports that intensive chemotherapy can be given safely over 24 hr without loss of therapeutic effect. This regimen is now being tested as an adjuvant to mastectomy in node-positive operable breast surgery.

Interleukin 3-dependent hematopoietic progenitor cell lines.

Several biological phenotypes of growth factor-dependent cell lines have been described in recent years, including those with T lymphocyte, neutrophil granulocyte, basophil/mast cell, B lymphocyte, and multipotential stem cell properties. The growth factors for each cell lineage are a subject of intense study. Continuous mouse bone marrow cultures infected with RNA type C viruses (retroviruses) produce nonadherent hematopoietic cells over a longer duration than control cultures. Marrow cultures derived from strains with spontaneously induced ecotropic endogenous retrovirus demonstrate a greater longevity than those from strains with no replicating virus. Cultures infected with murine leukemia virus also generate a greater number, compared with controls, of cloned permanent suspension cell lines dependent for growth on a 41,000-dalton glycoprotein (interleukin 3 [IL 3]). Some are multipotential with capacity for differentiation to erythroid, neutrophil, eosinophil, and basophil/mast cell types. Other cloned IL 3-dependent cell lines are committed to a single pathway. Studies with Friend spleen focus-forming virus indicate that the first effect in the marrow culture is mediated through a subset of adherent hematopoietic stem cells. Bone marrow culture-derived IL 3-dependent cell lines provide a model with which to study the role of viral genes in the control of differentiation and self-renewal capacity of hematopoietic stem cells.

Rationale for the surgical treatment of pulmonary metastases.

In a twenty-six year period, 90 patients underwent surgical treatment for metastatic malignant disease of the lung. The primary tumour had apparently been completely removed and there was no evidence of other metastatic spread. Thirty patients enjoyed a survival of more than five years. In 22 cases of the lung lesion was multiple and in 17, the lesions were bilateral. In patients with pulmonary metastases, but without evidence of other dissemination of the disease, resection of the lesions provided useful palliation, and 33% survived for five years of more.

Thyrotoxic myopathy presenting as dysphagia.

A case of aspiration pneumonia due to incoordination of swallowing is described. Its aetiology lies in the myopathy of thyrotoxicosis and a tentative hypothesis as to its causation is suggested.

Epileptiform seizures with maprotiline hydrochloride.

Two cases are described who developed epileptic seizures whilst taking maprotiline hydrochloride in therapeutic dosage. In both cases the electroencephalogram was normal and the fits stopped on withdrawal of the drug.

Aldosterone in myxoedema.

Ten patients with severe myxoedema were treated with regular increments of thyroxine. The urine aldosterone excretion rate and plasma-aldosterone level increased with each increasing dose of thyroxine. Two patients in whom the serum-aldosterone rose to abnormally high levels had cardiac failure which resolved when the thyroxine dose was reduced.
