Fetal enteroviral infection with cerebral ventriculomegaly and cardiomyopathy.

Fetal cerebral ventriculomegaly and cardiomyopathy were diagnosed prenatally at 35 weeks and resulted in fetal death. Although there was no post mortem pathological evidence of inflammatory lesions, fetal serum alpha interferon was abnormally high (25 IU/ml). Virologic investigations were therefore carried out on stored amniotic fluid, fetal and maternal blood samples. The presence of enterovirus was retrospectively demonstrated in amniotic fluid. The association of fetal birth defects with enteroviral infection is puzzling and suggests that more attention should be given to virologic studies in the diagnostic workup of fetal malformations otherwise considered as idiopathic.

Pharmacokinetics of interferon-alpha in pregnant women and fetoplacental passage.

Interferon (IFN) therapy is currently not approved for use during pregnancy. Two HIV-seropositive pregnant women, who were due to undergo abortion in the second trimester of pregnancy, were given a single intramuscular dose of IFN-alpha, with their informed consent. Blood samples were taken simultaneously from the mothers and fetuses, together with amniotic fluid. IFN was undetectable in the fetal blood and amniotic fluid in both cases. Pharmacokinetic parameters were similar to those in nonpregnant women. We conclude that maternal IFN-alpha during pregnancy should be safe for the fetus. The indications for IFN therapy in pregnancy could therefore be the same as those in the non-pregnant state.

[Diarlex LA and Diarlex MB: study for direct detection of rotaviruses and adenoviruses in stools].

Gastro-enteritis have an important frequency in clinician consultation especially in pediatric department. Viral responsible for these diseases are, in most cases rotaviruses and adenoviruses. From the use of electronic microscopy, the identification of these viruses has advanced : Elisa methods are now usually used in that area. More recently, easier methods using sensitized latex particles (Diarlex LA Orion Diagnostica) and immunochromatography (Diarlex MB , Orion Diagnostica) appeared and,in that study, their accuracy is compared to the electronic microscopy (for Adenoviruses) and Elisa method IDEIA Rotavirus trade mark (Dako) (for rotaviruses). Reliability, easy use, immediate results and moderate cost may assure to these new tests a leader place in all kinds of clinical laboratories.

[Neonatal jaundice occurring during infection with a Coxsackie B4 virus].

An infant, born at term, presented at the 3rd day of life an hepatitis with hepatospleomegaly, intense icterus and abnormal activities on the coagulation test. Coxsackie B 4 virus could be isolated from the urine, and later on the infant developed specific antibodies of increasing titers. The child recovered and encephalitic or myocardial involvements were not observed. The literature is reviewed and the authors underline the relative rarity but also the severity of Coxsackie B 4 infection in the neonatal period.
