Ultrasonic features of the kidneys of seven infants with congenital nephrotic syndrome of the Finnish type under active conservative treatment are described. In all cases the kidney were enlarged an the cortex more echogenic than the liver and spleen parenchyma. The pyramids were small with hazy borders resulting in an indistinct or lost corticomedullary border. At follow-in the echogenity of the renal cortex increased still further in four infants and the pyramids became invisible. The differential diagnosis is discussed.
The lymphographic films of 40 children with solid tumours were re-evaluated to assess the value of lymphography. In 11 children the lymphographic findings were positive. In only one child with carcinoma of the small bowel the metastatic changes in a normal-sized lymph node were probably not demonstrable on ultrasound (US) or computed tomography (CT). In the most common childhood tumours the metastatic lymph nodes were enlarged and thus demonstrable on US or CT. Therefore we suggest that US or CT should be performed first, followed by lymphography in doubtful cases only.

A secondary astrocytoma appearing 3 years after diagnosis of osteosarcoma is reported in a boy 11 years old. The boy had been treated with cytostatics according to the T-10 protocol. Treatment had been discontinued 2 years before diagnosis of the astrocytoma.
Thirteen boys and five girls with leukemia were investigated with lymphography. Eleven of these children were followed-up with postlymphography films. Children with small atrophic lymph nodes seemed to have worse prognosis than children with normal appearance of the nodes. No prognostic meaning of contrast medium persistence could be evaluated.

The lymphographic appearance of lymphoma may be simulated by a variety of other diseases, both malignant and benign, including metastases, lymphangiomatosis, sarcoidosis and lymphadenitis. In addition, non-specific reactive changes of the lymph nodes are common in lymphography of children sometimes simulating lymphoma.

Thirty-seven preterm infants with idiopathic respiratory distress syndrome were prospectively studied for the effect of the early closure of patent ductus arteriosus with indomethacin on the course of idiopathic respiratory distress syndrome. Serial retrograde aortograms were performed in all infants in order to visualize the ductus arteriosus, apart from three patients, who died early and were evaluated aortographically only once. The ductus was initially open in 27 infants and closed in 10 infants. The infants with open ductus arteriosus were randomly divided into two groups. The first group consisted of 13 infants, in whom the ductus was closed with indomethacin at a median age of 18 hours. The other 14 infants served as controls. Total time on assisted ventilation and duration of exposure to additional oxygen were significantly shorter in medicated infants than in controls. Oxygenation of infants with an initially closed ductus was better from birth and duration of their ventilatory assistance and oxygen exposure were shorter than in infants with initial ductal shunting. The data suggest that the early closure of the patent ductus arteriosus with indomethacin in distressed preterm infants has a favourable effect on the course of idiopathic respiratory distress syndrome.
Twenty-seven children aged 0.6 to 7.0 (mean 2.1) years were admitted to the hospital in 1967 and 1968 with type 7 adenoviral pneumonia. All ran a prolonged course. Type 7 adenovirus was isolated from 14 children, and in the other 13, the rise in the titer of complement-fixing antibodies to adenovirus was fourfold or greater. The outcome of the disease in these 27 children was reassessed in 1979, 9.6 to 12.1 (mean 10.7) years after the adenovirus type 7 pneumonia. Twenty-two were examined clinically and roentgenographically and all had lung function tests. Twelve had abnormal chest roentgenograms, and of these, six had bronchiectasis. Six of the ten children with normal chest x-ray films and ten of the 12 with abnormal chest roentgenograms had abnormal pulmonary function tests. Of the six patients with bronchiectasis, four showed no discernible cause of bronchiectasis other than the antecedent type 7 adenoviral infection. The other two patients had bronchial asthma, which can be a risk factor for bronchiectasis.

Aberrant locations and configurations of the thymus may occur. This is illustrated in a seven month old boy. He had a finding radiologically interpreted as a left upper lobe atelectasis, but proved at thoracotomy to be the left lobe of the thymus which was normal except for the unusual location.
The correlation between delayed venous drainage from the femoral neck and "head at risk" factors were studied in 47 patients (50 hips) with Perthes' disease. A good correlation was found between markedly disturbed venous drainage and "head at risk" factors. Lateral subluxation of the femoral head and Gage's sign correlated best with pathological venography. Thus a severely disturbed venous return from a hip with Perthes' disease seems to be a poor prognostic sign.
