The present study involved 18 cases f axillo-femoro-distal bypass performed to revascularise either a popliteal artery (8 cases) or a leg artery (10 cases). The postoperative mortality (2 cases) was not directly related to the operative procedure. This type of operation was performed in particularly grave situations of severe ischaemia or sepsis. The results obtained were interesting in two-stage operations (7 good results in 11 operations) for ischaemia. In one-stage operations, corresponding to particularly grave situations, as well as in operations for sepsis (2 cases with 2 failures), the failure rate was by contrast very high. The results obtained were compared with those found in the literature. Such very long bypasses would finally appear to be justified in very grave situations in which there is no possibility of direct surgery with the aorta as the proximal source.

A study is made in this paper of the prognostic significance of a very low distal index (index 6), revealed by irrigraphy. On the basis of examination of 55 case histories, the patients were divided into two groups A and B as a function of the distal indices at the root of the limb. The amputation rate was very high in both groups, but the prognosis was better in group A where the situation could be improved by restorative surgery at a high level. Analysis of the surgical possibilities has indicated that restorative surgery at a high level is a promising procedure, especially in group A. Surgery at the femoro-popliteal level yielded less satisfactory results because of the often considerable degradation of the arterial bed further downstream. Sympathectomy appears to be a useful supplement to restorative surgery in appropriate cases. The concept of the reduced distal index is also valuable for detecting cases of arteritis which are still well compensated but are in danger of vascular failure (dangerous claudications).
Three main types of arteriopathy in diabetic patients can be distinguished by comparing arteriographic and "irrigraphic" data. High forms, with non-specific irrigraphic findings, fall within the framework of typical arteriosclerotic types of arteritis. Distal femorotibial forms, with characteristics in irrigraphic tracings found only in diabetes, differ from the previous forms. Finally, there are hyperirrigation syndromes in which the functional element is dominant, and which can be considered as early forms of diabetic arteriopathy. These conclusions are based on a study of 180 cases, arteriographic and irrigraphic data being compared in 90 of these patients.
The semeiological nature of the clinical features described by M. Raynaud have been the subject of continuous discussion since 1862. This "phenomenon" is actually a syndrome, is histo-angeic and purely microvascular, is of acral topography, and implies a predisposition: that of a particular reaction to cold. Distal irrigation and the structure and function of glomic anastomoses show certain particular features. The "phenomenon" is a microrheological storm.

Two cases of arteriovenous fistula of the renal pedicle after nephrectomy are described, and the general features of this type of lesion reviewed in relation to the findings in these 2 cases and 52 other patients reported in the published literature. The following points are emphasized : the importance of local signs for revealing the presence of a fistula; the long period during which no signs of cardiac complications appear; the high-output cardiac failure which finally occurs is unaffected by medical treatment but responds remarkably well to surgical cure of the fistula. Hemodynamic tests were conducted in one case. The treatment of choice is resection of the aneurysmal sac with suture of the vessels, but less complex procedures, ligature of the artery and vein or the artery alone, may be sufficient.
