[Sexual function and aortoiliac reconstructive surgery].

In this study of 52 patients aged between 32 and 60 years, maintenance or restoration of sexual function during aorto-iliac surgery represented a usually non-priority but certainly desirable objective. Aspects studied were: place of crossed bypass and of endarterectomy-type of aorto-prosthesis anastomosis: end-to-end or end-to-side - routine or non-routine reimplantation of a hypogastric artery. Results in this series showed a lack of advantage from the sexual point of view for end-to-side over end-to-end surgery; hypogastric reimplantation offers a supplementary guarantee of efficacy but should not be performed routinely; crossed bypass operations are certainly interesting solutions in young patients. Endarterectomy should not be rejected as a matter of principle but with respect to sexual function its indications should be reserved for cases not requiring extensive dissection of arterial axes.

[Early atheromatosis. Study of a series of 55 cases of arteritis of the lower limbs in young adults].

The study of 55 cases of atheromatous arteritis, representing 67.6 p. cent of the arteriopathies observed in young adults, permits to emphasize the following three points: 1) the classical risk factors of atheroma are frequently identified, but the importance of these multiple risk factors must be stressed; 2) aorto-iliac segmental forms represent a type of lesion which is rather typical of arteritis in young patients; an elective aorto-iliac involvement was noted in 56.3 p. cent of cases, beside more diffuse forms resembling the usual atheromatous forms; 3) the evolutive potential of these arteriopathies is difficult to evaluate and the type of lesion caused by arterial overload seems to be the most interesting factor to do so.

[Do arteriopathies in patients in their 50s present special characteristics?].

The 50s represent a hinge age-period during which atherosclerosis is more frequent but arterial disease does not appear to present features differing from those observed in neighbouring age groups. There is of course a higher frequency of atherosclerotic lesions of other arterial territories and a fairly high incidence of diabetes. With respect to the latter disease, apart from the typical association arteritis-diabetes, some cases are simply of atherosclerosis developing at the same time as the diabetes. Arteritis of lower limbs can present as two principal types in patient in their 50s: elective aorto-iliac arterial disease similar to early onset atherosclerosis in younger patients and more diffuse forms more typical of arterial disease in the elderly.

[Fibrous hyperplasia in femoro-popliteal bypasses using treated human umbilical veins].

The authors report two clinical cases of the failure due to fibrous hyperplasia of processed human umbilical vein (HUV) grafts employed as femoro-popliteal bypasses. Failure occurred after 19 months implantation at the proximal anastomosis for one graft and after 10 months at the distal anastomosis for the other. Fibrous hyperplasia was the principal cause of reoperation. The lightly vascularized, more or less compacted fibrin involved, adhering loosely to the HUV, remains thrombogenic at the blood-contacting surface. Superposed onto this phenomenon which is widespread in laboratory animals but uncommon in humans, are the usual causes of failure: delamination of the HUV wall, lipid uptake and bacterial colonization.

[Extra-abdominal desmoid tumors. Apropos of a case in the calf].

A young 15 year old girl presented with a voluminous desmoid tumor of the calf. Three attempts to excise the tumor were only partially successful because of the risk of vascular and nerve damage, and excision was incomplete on each occasion. These tactics appeared to be justified, however, since the tumor had little functional effects and appeared to become stable at the last examination. The tactical surgical problems are discussed, the benign nature of the tumor imposing conservative surgery which is often incomplete, although in certain cases the operation can be combined with a recovery arterial shunt. The various etiopathogenic elements of these tumors are described.

[The problem of Buerger-like arteriographic aspects observed in patients over 35 years of age with arterial disease].

Arteriographic appearances suggestive of Buerger's disease may be seen in patients considerably beyond the usual age of onset of the disease. Eliminating cases possibly resulting from an embolic process and diabetics, 9 patients were evaluated in the present study. Combined review of arteriographic, clinical, capillaroscopic and histological data led to the identification of 3 patient groups: Buerger's disease with ageing: 3 cases, intermediate or transitional forms: 4 cases, Finally, certain cases of arterial disease related to cold damage should be viewed in the context of the preceding forms: 2 cases.

[Surgical treatment of sequelae of phlebitis].

Surgical indications in the post-phlebitic disease are supplied by an accurate physiopathologic analysis of the mentioned syndrome. Fuller research means such as: phlebography, functional explorations enable to demonstrate great pathogenic processes that could require a surgical treatment: venous obstruction can be treated thanks to a derivation treatment Palma-type. Various techniques have been proposed as for deep venous insufficiency. Surgical procedure of superficial venous insufficiency appeared as essential in operative indications. The major problem of such a procedure was the suppression of blood escape due to perforans veins insufficiency. There was no surgical treatment of postphlebitic disease, but rather surgical techniques that could be suited to particular conditions.

[The irrigraphic profile, an orienting element in the indications for lumbar sympathectomy].

A retrospective study of 108 case-reports of patients treated by lumbar sympathectomy was conducted to determine irrigraphic parameters possessing possible predictive value for the indication of sympathectomy. The value of the Index 6 (distal irrigation index) must be certainly taken into account; the Index 1 (proximal index) appears to be of interest when analyzing data with a view to sympathectomy in diffuse forms of arteritis. Three profile-types were distinguished; statistical analysis showed no significant difference in these profiles, with respect to the sympathectomy, in relation to the clinical results obtained. Irrigraphic modifications after sympathectomy were analyzed in 72 cases; results confirmed the improved efficacy of sympathectomy at Stage 2 than at Stages 3 and 4, but also showed that at Stage 2 the profile II appeared to react more favorably and that at Stages 3 and 4 the profile I appears to react less well than the other profiles. Also studied were patients with diabetes, who react less favorably to sympathectomy, patients who fail-to respond to sympathectomy, and those with worsening of irrigraphic profiles after sympathectomy, mainly in diabetics with profile I. These findings show that it is not possible to define a precise irrigraphic profile more favorable to sympathectomy, but that they provide interesting elements to be confronted with clinical and arteriographic data when deciding on the need for sympathectomy.

[The future of the asymptomatic leg after unilateral iliac artery repair].

A retrospective study was carried out in 30 patients after unilateral iliac endarterectomy, the contralateral limb being asymptomatic, to assess the evolution of the unoperated limb. Findings indicated 4 claudications uncovered by the unilateral operation, 8 secondary thromboses (including one asymptomatic lesion) of the unoperated iliac axis--repeat operation was necessary in only one patient. Findings emphasize the importance of control of risk factors to prevent secondary deterioration of an unoperated limb. The risk of thrombosis in the primarily asymptomatic side appears to lessen with immediate insertion of an aorto-bifemoral prosthesis, but risks inherent in prosthesis application--sepsis, false aneurysm--must be allowed for.

[Digital arteritis and bleomycin. Apropos of a case].

A 47-year-old patient developed severe digital arteritis after complementary bleomycin treatment following surgery for esophageal cancer. Arteriography images showed extrinsic stenosis of the digital arteries, results of other investigations suggesting development of a sclerodermic process due to bleomycin. Ischemic lesions regressed after thoracic sympathectomy. Similar cases have been well documented but cases of associated cancer and digital ischemia lacking iatrogenic features have also been reported. The pathogenic role of bleomycin appears to be established in the present case, but the possibility of the digital ischemia developing within the framework of a paraneoplasic syndrome is discussed.
