Students from a nursing school of Delhi were surveyed anonymously using a self-administered questionnaire to explore various AIDS-related apprehensions and their possible reasons. The observations revealed that, majority of the students and their families/friends feared that these students were at risk of contracting HIV infection while providing routine patient care. A large number of students also opined that they would feel uncomfortable while talking, hugging, shaking hands, and sharing a room with an HIV positive person. The main reasons for their apprehensions were unsatisfactory anti-AIDS campaigning by the government, non-availability of sufficient protective measures in the health care settings, inadequate professional education related to prevention of HIV infection, and increase in HIV transmission following false sense of security due to excessive condom promotion. Findings of the study imply imparting factual knowledge addressing the concerns and removing misconceptions which influence attitudes and willingness of the nursing students to provide care to the HIV positives/AIDS patients, facts regarding efficacy of various preventive measures, and provision of counselling services in the event of exposure.

A case of congenital urethroanal fistula with a normal anterior urethra in a male child is reported. The fistula was demonstrated between the prostatic urethra and anorectum. This anomaly is usually associated with an atretic anterior urethra and has been variously described as a variant of a urethral duplication by some authors, and of an anorectal malformation (ARM) by others. We conclude that its rightful classification is as a variant of ARM in which the fistula is a result of persistence of the cloacal duct and corresponds to the anorecto-vestibular fistula with a normal anus (perineal canal) in a female.
This study describes a subset of patients with posterior urethral valves (PUV) who presented late in childhood. The objective was to identify factors that lead to back-pressure effects on the upper tracts, which persist in spite of adequate valve ablation in some patients, and seek factors that may preserve the upper tracts despite untreated obstruction in other patients. Six children with PUV diagnosed after infancy were evaluated. The pre-operative work-up included renal biochemistry, ultrasonography, voiding cystourethrography, and uroflowmetry. Detailed urodynamic studies, including uroflowmetry and slow-fill cystometry, were performed in all cases 6 months after surgery. Adequacy of valve fulguration was confirmed by urethroscopy. Three of the six patients had normal upper tracts; in these, there was marked improvement in peak urine flow rates after fulguration and bladder pressures were normal. The other three patients had bilateral hydroureteronephrosis, and two had chronic renal failure. This group had markedly decreased functional bladder capacity with loss of compliance at low bladder volumes and significant residual urine volumes in spite of adequate valve fulguration, suggesting myogenic detrusor failure. We conclude that in patients with PUV presenting beyond the age of 5 years, upper-tract deterioration may accompany high storage pressures in the bladder. In some boys with long-standing obstruction the upper tracts may escape damage; in our series this was associated with normal bladder dynamics and appeared unrelated to the severity or duration of outflow obstruction.

Total or partial deficiency of factor VIII and IX in the coagulation cascade leads to haemophilia. Haemophilia affecting weight-bearing joints gives a 'pseudotumour' or haemarthrosis-like condition. Surgery and cryoprecipitate infusions have been the treatment for this condition. Radiocolloids and radiation therapy have been used with some benefit. One case of ankle pseudotumour which was treated by low-dose external beam radiation is presented here.
Distant metastases from carcinoma of the palatine tonsil are very uncommon. We encountered a case of a carcinoma involving the tonsillar region that resulted in subarachnoid mestastases following local radical radiotherapy. Metastases were diagnosed following magnetic resonance imaging and spinal fluid cytology. The patient succumbed to the disease after a rapid downhill course, 1 month following diagnosis of the secondary deposit. The present paper describes the rare site of distant metastasis and reviews the relevant literature.

Two thirty three diploma students from a Nursing School, in Delhi were surveyed anonymously to seek their opinion on some AIDS-related policy issues and willingness to provide AIDS care. More than one fourth of students opinionated that health personnel should be given a choice to refuse care to the AIDS patients and an equal proportion felt that authorities should be empowered to terminate the services of such personnel. Majority were in favour of making HIV testing compulsory for various high risk groups, indoor patients and health workers. Perceiving the risk of HIV infection, 41.2% students felt that girls might not opt for nursing career, whereas few (10.7%) were considering even to leave the profession. However, majority (58-72%) were willing to nurse the high risk individuals, if adequate training and preventive measures were provided in the health care settings. The study implies framing of an AIDS policy taking in to consideration the views of health professionals and imparting comprehensive training to the nursing students in order to promote a humane approach towards AIDS care.
At Imphal (24 degrees 44' N) testes of lal munia, Estrilda amandava, began in June/July, peaked in September/ October, and thereafter declined to a minimum in December/January. Daily im treatments of 2.5-10 mg/kg/ bird/30 days of naloxone during progressive phase suppressed testicular growth, but without effects during quiescent, peak, and regression phases. Daily morphine (5 mg/kg/bird) during progressive and peak phases stimulated testicular growth, but without effects during quiescent and regression phases. Daily morphine (10 mg/kg/bird) during progressive phase stimulated the testes, an effect reversed by daily im treatments of an equivalent dose of naloxone. Seasonal changes in body weight closely correlated with testicular size. Daily im naloxone (5 and 10 mg/kg/bird/30 days) during progressive phase inhibited the increased body weight, but had no effects during quiescent, peak, and regression phases. Morphine (5 mg/kg/bird/day) during progressive and peak phases increased body weight, but had no effects during quiescent and regression phases. Morphine (10 mg/kg/bird/day) during progressive phase increased body weight, an effect which was reversed by equivalent dose of naloxone. Plumage color increased progressively between May and August/September, was maintained during October, and thereafter declined to reach dull-brown henny feathers by December. Daily im naloxone (2.5-10 mg/kg/bird/30 days) regardless of the reproductive states did not affect plumage color cycle. Morphine (5 mg/kg/ bird/day) accelerated plumage pigmentation between June and August, but had no effect during progressive or peak phases. Postnuptial decline in plumage color was inhibited by morphine (5 and 10 mg/kg/bird/day) and naloxone failed to reverse this effect. It is concluded that in the lal munia, endogenous opioid peptides are important constituents of the neuroendocrine mechanisms that influence development of the testes and body weight.
A case of congenital fistula between the prostatic urethra and the perineal skin in a ten-months-old male child is reported. The fistula was lined by squamous epithelium. The precise embryological origin of this fistulous communication remains controversial. We believe that this anomaly is distinct from the congenital H-type urethro-anorectal fistula.
A detailed analysis of host-tumor factors and interstitial physical factors influencing the disease-free control in carcinoma of the tongue was carried out. Twenty-eight cases of carcinoma of the tongue T1-3, NO-1, MO were treated radically with combined external irradiation and 192Iridium interstitial brachytherapy (one patient received brachytherapy only). The teletherapy dose ranged from 44 Gy to 56 Gy (average 48 Gy), the brachytherapy dose ranged from 16 Gy to 55 Gy (average 22 Gy). The interstitial practice involved loop technique in all the cases. Dose distribution analysis to assess factors influencing local control included indices of dose rate, source activity, inter-planar distance, and discontinuity in the prescribed isodose in other planes when compared to mid-plane. Two-year actuarial disease-free survival (DFS) was 46% with primary treatment and 63% when salvage treatment was also included. DFS was significantly poorer when the interplanar distance at mid-plane exceeded 10 mm (p < 0.05). Similarly, tumor control was poorer (p < 0.008) when there was discontinuity in the prescribed isodose in 1 or 2 planes (1 cm superior and inferior to mid-plane). Interplanar distance of around 10 mm, prescribed isodose continuity in all three planes and limiting the dose maximum within a factor of 2 will optimize the results of interstitial implants.
