[Changes and progress in the treatment of fractures of the coxal end of the femur].

Consideration of technical developments, a review of the literature and of our own experience with 535 patients compared with nearly 20,000 patients of an ASIF population treated during the same period show that the indications for treatment of extra- and intracapsular fractures of the proximal femur have become much simpler. (1) Extracapsular fractures of the proximal femur fall into two groups: pertrochanteric fractures are fixed by means of the dynamic hip screw and intratrochanteric unstable fractures by means of either the dynamic hip screw (valgus type) or a 95 degrees condylar plate. (2) Intracapsular fractures are treated according to the patient's age group. In patients younger than 70 years femoral head preservation is achieved by compressing screw osteosyntheses, while in patients over 70 years head resection is performed an a total endoprothesis inserted. In patients with life expectation shorter than 3-5 years a femoral head prothesis is inserted.

[Experiences collected with a daylight system (author's transl)].

The Kodak Daylight System is space-saving. It consists of the Kodak-X-Omat cassette, the identification camera and the loading and reloading device (cassette multiloader). All three units of this system are notable for extremely safe operation. Handling is simple and easily appreciated. All brands of film can be used. The data window in the cassette is too large; in some cases, important sections of the image can be covered up by the window. Automatic loading and reloading considerably reduces soiling of the cassette, which is otherwise common, and also reduces abrasion of the intensifying screen. Introduction of this system has had a beneficial effect on overall working and has been welcomed by all concerned as a contribution towards facilitating routine work.

[Studies on a new hepatotropic contrast medium].

The contrast-material Biliscopin was clinically studied in 101 patients. No side-effects were observed when the injection time was stretched over a 5 min period. The visible heterotopic excretion which correlates with the iodine concentration was low in the case of Biliscopin. Optimal visualisation of the biliary tract was obtained 60--90 min after injection. The remarkably long visualisation of the biliary tract allows tomography beyond 90 min after injection; this is seen as an advantage for X-ray departments with limited tomographic facilities.

[Pacemaker and seat belt (author's transl)].

Although seat-belt-induced traffic accident injuries to wearers of pacemakers are rare, about 50,000 pacemaker patients have to "buckle up." A study of 450 pacemaker patients indicated that, as in the general population about 50% use their seat belts. Fifty percent of the patients felt uncomfortable or showed local damage of the area overlying the pacemaker. Studies with dummies revealed a 7-cm upward dislocation of the pacemaker caused by the negative acceleration and by the seat belt itself during the crash. This could be prevented by an external cap protecting the pacemaker.

[Clinical comparative study between the contrast media Biligram, Endomirabil and Biliscopin (author's transl)].

The contrast media Biligram, Endomirabil und Biliscopin are tested in comparative series in a total of 304 patients. The optimal time of exposure for visulaization of the bile ducts and gallbladder is between 60 and 90 minutes in combined filling with Biloptin and the contrast medium used. The contrast media do not differ from one another in a significant manner in respect of contrast densification. All three constrast media have low side effects for injection times of 5 minutes. The side effect quota increases with the amount of injected contrast medium. Of all three contrast media, Biliscopin has the lowest side effect quota.

[Radiographic examinations of the colon -- results of 2400 double contrast enemas (author's transl)].

A technique for the double contrast method was described which was used in our radiology institute on 2400 patients. It takes into account economic factors (amount of film used) as well as the lowest possible radiation exposure (mean screening time: 6.4 minutes). Typical lesions from malignant processes in the colon were illustrated. Today, radiographic examination of the colon should be conducted only with the double contrast method. This rule need not be followed in acute cases, e.g., an ileus produced by a tumor. The examining techniques must be adapted to the individual case. Today, the single contrast technique should be used only in such special cases.

[The effect of proteinase inhibitors in the early stage after experimental myocardial infarction (author's transl)].

Our experiments have shown that in cats in the early phase after experimental myocardial infarction, the volume of extravascular fluid increases in the lungs while the intravascular volume decreases. A hemoconcentration developed with increased hematocrit and a fall in plasma volume. The thrombocyte count fell continuously during the first 4 hours after the infarction. The CPK activity in the region of the myocardial infarct was reduced in comparison to uninfarcted tissues. Intravenous injection of 50,000 KIU/kg Trasylol within 20 minutes after the infaction caused a significant reduction of the extravascular fluid in the lungs. All the other parameters were not changed by Trasylol administration, with the possible exception of the fall in thrombocytes which seemed to be slowed after Trasylol.
