Experience with the Bamberg obstetrical forceps.

The design and use of a divergent obstetrical forceps, which was developed at the Staatliche Frauenklinik und Hebammenschule in Bamberg by Sipli and Krone are presented. The major advantage associated with the use of this instrument is that it permits the exertion of a limited constant application force (max 300 g) on the fetal head. Thus, compression injuries are effectively prevented and slippage of the forceps with resultant trauma is precluded. The Bamberg forceps was evaluated at the Frauenklinik und Poliklinik der Technischen Universität München and at the Staatliche Frauenklinik und Hebammenschule Bamberg, Federal Republic of Germany. An evaluation of 483 cases where this forceps was used is presented. No serious complications directly attributable to the use of this instrument could be documented.

[Improved Bamberg divergence forceps. Modification for the prevention of soft tissue injuries of the pelvis].

Constant and limited (max. 300 g) force of application by the Bamberg divergent forceps helps to prevent compression injuries and slippage of the forceps with resultant trauma. No serious foetal lesions have been observed over a period of 9 years. Maternal soft-tissue injuries were somewhat above the range of vacuum extractions. This prompted us to consider modifying the Bamberg forceps with a view towards preventing such maternal injuries.

Comparison of the effects of betamethasone and L-carnitine on dipalmitoylphosphatidylcholine content and phosphatidylcholine species composition in fetal rat lungs.

Antepartum administration of L-carnitine to pregnant rats results in an increase of both total phospholipid (80 +/- 11 mg/g dry weight (dw); mean +/- SD) and dipalmitoylphosphatidylcholine (DPPC) content (7.7 +/- 2.1 mg/g dw) in fetal lungs as compared to controls (72 +/- 10 and 7.0 +/- 2.5 mg/g dw, respectively). On the other hand, no effect of L-carnitine could be demonstrated on the DPPC portion in the total phosphatidylcholine (PC) or on the portion of palmitic acid in the PC fatty acids. Betamethasone elevated the DPPC content in fetal lungs (8.1 +/- 2.0 versus 7.0 +/- 2.5 mg/g dw in the controls), while total phospholipid content remained unaffected (71 +/- 19 versus 72 +/- 10 mg/g dw). The portion of DPPC in the PC species increased significantly (p less than 0.01) from 27.6 +/- 4.5 in the fetal lungs of the control group to 34.2 +/- 3.3 in the lungs of the betamethasone-treated group, while the palmitic acid portion in the PC fatty acids was nearly unchanged (45.9 +/- 3.2 versus 43.9 +/- 2.6 in the controls). Further, after betamethasone treatment, a significant diminution (p less than 0.01) of the monoenic PC 32 species (palmitoyl-palmitoleyl PC and palmitoleyl-palmitoyl-PC) and the PC 34 species (consisting primarily of palmitoyl-oleoyl-PC) could be demonstrated both in absolute and relative terms. This is in agreement with a significant reduction of the portions of palmitoleic (p less than 0.01) and oleic (p less than 0.05) acids in the total PC fatty acids.(ABSTRACT TRUNCATED AT 250 WORDS)

[Current problems with intrauterine contraception].

Due to the high technological standard of modern IUDs further improvements are rarely to be expected. Parallel to the increasing spread of intrauterine contraception an increase of aggravating complications is registered. The course of complications in 136 cases treated at the Gynecological Clinic (Technical University Munich) shows a wide range of causes of complications still to be eliminated. We consider critical selection of patients, comprehensive information of the patient before insertion of the IUD, careful insertion technique as well as complete and continuous follow-up after insertion of IUDs to be effective measures to reduce the present rate of complications.

Anti-Bacterial Agents

[Results of the cytological examinations of cells from the uterine cavity (author's transl)].

A new method for obtained cytological material from the uterine cavity with the Exploret-Fatol is described. The results in 38 patients with atypical vaginal bleeding is reported. In 4 women the tight cervical canal necessitated general anaesthesia. In all cases adequate cytological material for examination was retrieved from the endometrium. It was not always possible to differentiate endometrium from endocervical cels by their morphology. Nevertheless all benign premalignant lesions and all malignant lesions of the endometrium were cytologically recognized. This method may find application in many high risk patients and therefore improve the early diagnosis of carcinoma of the endometrium. The methods success depends upon good knowledge of endometrial cytology by the examining cytologist.

Adenocarcinoma

[Gynecological laparoscopy. A 5-year retrospective study on laparoscopy at the Technical University of Munich].

In a retrospective study we evaluated the results of 598 laparoscopies which had been carried out in the years 1973 through 1977 at the Women's hospital of the Technical University in Munich. A considerable increase in pelviscopic tube sterilizations could be observed. Severe complications resulting in a laparotomy occurred in 0,5% of the cases. For laparoscopic operations the medical indication was restricted, whereas it was generously applied for the differential diagnosis of an ectopic gravidity. The sterilization techniques most frequently used at the moment are the bi-polar high-frequency technique and--to a smaller extent--the light-current coagulation technique. The uni-polar high-frequency technique can be regarded as obsolete. As far as a restoration of fertility is concerned, the individual application of the various sterilization methods can be expected for the future.

Does the administration of the calcium-antagonist verapamil in tocolysis with beta-sympathicomimetics still make sense?

In tocolysis with beta-sympathicomimetics Verapamil is administered universally to reduce the subjective and objective cardiovascular side effects. The efficacy of the calcium-antagonist has not yet been substantiated. This has now re-emerged as an important issue due to the increasing occurrance of cardiopulmonary complications in tocolysis, some fetal. Nine mongrel German shepherds were anesthetized and relaxed, and their breathing was controlled at constant volume. After infusion of Fenoterol (0.06 microgram/kg/min) and subsequent administration of Verapamil (2 microgram/kg/min) a continuous record was made of the parameters of the pulmonary and systemic circulatory systems, the cardiac dynamics, the acid-base balance, the blood gases with O2 absorption and CO2 output, electrolytes and the colloid osmotic pressure. Furthermore, the intrapulmonary fluid volumes in the vascular system and the interstice were measured. During infusion with Fenoterol the changes expected from a beta-adrenergic were observed in the form of a hyperkinetic cardiac situation, metabolic acidosis and increased exchange of gases. Also pronounced fluid shifts in the lungs occurred from the intravascular space to the interstice. Subsequent administration of the calcium antagonist showed no influence on the beta-mimetic-induced changes demonstrated in the present work. The results agree with previous clinical investigations. According to the known dosage curve, the dosage of Verapamil would have to be increased multifold to achieve an effect, which is hardly possible due to the cardiodepression and peripheral circulation drop with hypotension that this would entail. The administration of Verapamil in tocolysis no longer appears expedient.

Adrenergic beta-Agonists

[Examination of plasminogen-activators and plasmin-inhibitors in maternal and fetal rat-lung-tissue after administration of DL-carnitine-hydrochloride (author's transl)].

In the formation and resolution of fibrinous pulmonary hyaline membranes of the type occurring in perinatal respiratory distress, the plasminogen activators and plasmin inhibitors contained in the tissue assume central importance. Carnitine does not change plasminogen activator activity and reduces the plasmin inhibitor content in the foetal rat lung. Hence, no inhibiting action on local fibrinolytic processes can be established for carnitine.

[Pharmacologic influencing of fetal phospholipid synthesis. Part IV: influencing the DPL(dipalmitoyl-lecithin) content of fetal lung by betamethasone (author's transl)].

Dipalmitoyllecithinic (DPL) is the quantitatively largest and most active portion of the surfactant system. Hence only a quantitative DPL determination allows a relevant statement of fetal lung maturity. The antenatal prophylaxis of RDS should result in an increase of the DPL-level in a fetal lung. A new gaschromatographical method for determination of DPL was developed. The results were secured by gaschromatography-mass-spectrometry-couplings. After treatment with betamethasone an increase of the relative DPL-amount in fetal rat lung lecithin species was found, but not an increase of the lecithin-level in the lung-tissue. Corticosteroids probably induce transesterification of the lecithins.

[Induction of fetal lung-maturity. Part III: surfactant-parameters in amniotic-fluid and fetal lung].

In animal experiments we determined surfactant-specific parameters of premature fetal lungs and of samples of amniotic fluid taken fromthe same animal, and compared them with each other. These parameters were, among others: total phospholipids, lecithine and palmitic acid. In these experiments, 75 gravid Wistar rats were treated with NaCl (controls) or drugs in clinically relevant doses for stimulating the fetal synthesis of phospholipids like betamethasone, bromhexine metabolite VIII and D-L-carnitine hydrochloride. A relation between the amniotic fluid and the fetal lung with regard to the quantity of surfactant substances was found - with a security of 95% - with the controls only. The value of amniotic fluid analysis is discussed under this aspect.

[Pharmacologic influencing of fetal phospholipide synthesis. Part II: Carnitin - a new way of dyspnea syndrome prophylaxis? (author's transl)].

Carnitine is a body-owned Betaine which is wide-spread in living nature in the form of a physiologic metabolit. On account of its central metabolic function, Carnitine has various effects in the intermediary metabolism. Toxicity of Carnitine is very low. DL-Carnitine hydrochloride (30 mg/kg body weight) was injected into 25 gravid Wistar rats and 24 gravid rabbits, 4 days before the Cesarean section. Compared to the group of controls, the group treated with Bromhexine metabolit VIII, as well as to the group treated with Betamethason, significantly higher values with regard to the content of total phospholipids, lecithine and palmitic acid could be found in the fetal rat lungs. Effects of Carnitine on the phospholipid metabolism are discussed.

[Pharmacologic influencing of fetal phospholipide synthesis. Part I: Effect of betamethasone and bromohexine metabolit VIII (Ambroxol) (author's transl)].

The effect of Betamethasone and Bromhexine-Metabolite VIII, commonly used in RDS-prevention, was studied in pregnant rats and rabbits. 73 rats and 78 rabbits were treated for 4 days with doses, similar to those used in men. Cesarean section was performed in rats from the 18th-21th day of gestation (23 days), in rabbits from 27th-30th day (32 days). In the amniotic fluid, in fetal and maternal lungs the total phospholipid content, percentage of lecithins in total phospholipids and palmitic acid were determined and used as parameters for the surfactant content. There was no sign of induction with regard to synthesis. Premature deliveries and malformed fetusses were frequently observed under steroid-therapy. Clinical application should therefore be observed more critically.

Ambroxol

[The practicability of the gravlee-jet-wash-technique--results of a laboratory (author's transl)].

The matter of examination were 171 jet-wash-proofs of 166 patients. In working up the material we aimed to get "sedimentsmears" and "specimens" by the aid of the cellbloc-technique. In the case of seventeen patients there was no material to be evaluated. Counting the 13 cases which were found necessary to be repeated because the quantity of material was too little, 17,5% of the cases finally could not be diagnosed. Using the histology of curettage, or the surgery preparations of 148 patients, we had something to compare with the results of the jet-wash-proofs. 11 out of 13 endometrial carcinomata (84,6%) were registered by the aid of the jet-wash-technique; this was contrasted with two false positive results out of 148 cases. The reasons of false positive and false negative results are being discussed. The efficiency of the jet-wash-technique depends on the prooftaking method, the way the material is worked up, and the kind of the dammage of the endometrium. The results show, that the use of a jet-washer is now, after a phase of learning, a diagnostical completion for the recognition of the endometrial carcinoma in high risk patients.
