[Morphologic correlations of anti-tumor immunity. II. Reactions of the tumor stroma].

In three groups of oncological patients (with oral, stomach, and breast cancer), standard testing of the tumour stromal reaction was made in relation to life expectancy. The quantification of stromal reaction, cell infiltration topography and representation as well as necroses of tumour tissue were analysed. A direct correlation between the stromal reaction intensity and the life expectancy could be shown for the stomach cancer only for the time being. A close contact of the stromal and tumour cells in this cancer could be also interpreted as a favourable prognostic sign. When using this simple testing for oral and breast cancer there were some not quite positive hints of correlation.

Breast Neoplasms↗

[Morphologic correlations of anti-tumor immunity. III. Reactions of regional lymph nodes].

Regional lymph nodes were examined in 4 groups of oncologic patients (with cancer of oral cavity, breast, stomach and lung). Histological types of the lymph node reaction were correlated to patients' survival. Favourable prognosis in all four groups was related to lymphocytic predominance, especially when combined with sinus histiocytosis. The predominance of germinal centres (combined with sinus histiocytosis) was a favourable sign but in stomach cancer. A marker of unfavourable prognosis was lymphocytic depletion, especially when sinus histiocytosis was absent.

Carcinoma↗

[Prognostic significance of reactive changes in regional lymph nodes in stomach and breast carcinoma].

The regional lymph nodes have been examined in 80 cases of carcinoma of the stomach and in 70 cases of cancer of the breast, and their reactive type has been determined and correlated with the duration of survival of the patients. A prognostically favorable sign in both groups appeared to be the finding of a predominance of lymphocytes combined with a sinus histiocytosis, in the case of cancer of the stomach, then also a reaction of the type of prevalence of germinae centers together with sinus histiocytosis. Prognostically unfavorable was apparently the finding of a depletion of lymphocytes, especially if sinus histiocytosis was also lacking. The isolated finding of a single lymph node metastasis, in itself, if we do not take into account the reactive type, could not be interpreted in the sense of a worse prognosis from the viewpoint of survival time.

Adenocarcinoma↗

Prognostic significance of reactive changes in regional lymph nodes in gastric and mammary carcinomas.

Regional lymph nodes in 80 cases of gastric and 70 cases of breast cancer have been examined and the reactive types have been correlated to the survival periods of the patients. In both groups of patients, a prognostically favorable sign was the detection of lymphocytic predominance combined with sinus histiocytosis, moreover, in patients with gastric cancer also the reactive type of predominance of germinal centers with sinus histiocytosis. Prognostically unfavorable sign was the detection of lymphocytic depletion, in particular if sinus histiocytosis was absent. The presence of metastases in the lymph nodes--apart from the reactive type--cannot be regarded as a sign of bad prognosis.

Role of phagocytic cells in cancer.

Partial manifestations of the anti-tumor defence in the organism will sometimes lead to surprising changes in the clinical picture of the disease. This must be taken into consideration and, if possible, such therapeutical procedures must be chosen which do not interfere with the course of defence reactions. Therefore, the study of the organism's anti-tumor defence mechanisms is of theoretical as well as of practical value. In Hodgkin's disease a significant increase in phagocytic activity could be observed when the disease was in progress; during clinical remission the values returned to normal ones [181]. The phagocytic activity may be supposed to reflect the dynamic situation between the host and the tumor in man [196]. The determination of phagocytic activity may be useful as an indicator of the extent or activity of the disease [181]. Phagocytic function may be affected by a surgical intervention [46], radiotherapy [179, 183] and by anti-tumor chemotherapy [3, 209]. On stimulating MPS during the course of radiotherapy, improved clinical results have been observed [115]. When examining patients in the course of treatment and after it, a need was felt for methods that can easily be applied and that enable the state of the defence capacity of the organism to be taken into account. The measurement of the phagocytic activity of MPS in clinical practice by using the 131I labelled aggregated human albumin [115] can only be used at selected workplaces. Today, however, drugs and therapeutical procedures with immunosuppressive effects are virtually used in all workplaces where there are patients with malignant tumors. Therefore, an original method of phagocytosis of latex particles in vitro has been elaborated. This method is primarily aimed at orientation, but it may be employed in every hematological laboratory. It determines the intensity of phagocytic activity of leukocytes. The differences between the values obtained in cancer and those obtained in healthy subjects and patients with non-malignant disorders are highly significant. This statement has been made on the basis of 363 examinations performed in clinically healthy subjects, in patients with cancer and in patients with non-malignant disorders. The results were analyzed statistically and it has been found out that the phagocytic capacity of leukocytes is significantly activated in the presence of a pathological process in the organism. This activation is mush more evident in cancer patients than in patients with non-malignant disorders. Strikingly low values were observed in the terminal stage of the disease when the defence capacity of the organism has broken down. These findings induced us to consider the suitability of this kind of examination. It can be used alone or in combination with other methods for estimating the defence capacity of the organism, or for choosing the right therapeutical measures...

Prognostic significance of reactive changes in regional lymph nodes in cancer of the oral cavity.

Investigated were lymph nodes in a group of patients with spinocellular carcinoma of the oral cavity. As regards patient survival, lymphocyte predominance and sinus histiocytosis manifest prognostically the most favorable changes. A predominance of germinal follicle centers in our group of patients cannot be interpreted as indicator of an improved prognosis. Sinus histiocytosis has never been found in nodes with metastases.

Nucleoli of lymphocytes in the peripheral blood of patients with periodontal disease (preliminary report).

A group of 104 patients with various grades of severity of the periodontal disease and a control group of healthy volunteers were examined. On the basis of an examination of the nucleoli, the activity of the RNA synthesis in the lymphocytes of peripheral blood was established. Patients with a very advanced periodontal disease showed a statistically highly significant increase in the ratio of active and reversibly inactivated lymphocytes. Thus, the significance of immune factors in the development of the periodontal disease was confirmed.

[Regional lymph nodes in carcinoma of the oral cavity].

A re-examination was made of bioptic investigations of lymph nodes in a group of patients with spinocellular carcinomas of the oral cavity. With regard to the patients' survival, lymphocytic predominance and sinus histiocytosis appear to be changes with the most favourable prognosis. The predominance of embryonic centres of follicles cannot, in the author's group, be interpreted as signs of better prognosis. Sinus histiocytosis was never found in nodes with metastases.

[Attempt at the evaluation of stromal reactions in orofacial tumors].

A group of 130 patients with malignant tumours of the orofacial region were examined. Each patient was investigated for tumour tissue stromal reaction in terms of topography, quantity and quality of the infiltrate, and with a view to the presence of necrosis. A comprehensive analysis found no direct correlation between stromal reaction and the patients' survival times.

Adenocarcinoma↗

Cellular reaction to autologous tumor tissue followed by "skin window" technique in orofacial tumors.

In a sample of patients with pseudotumors and benign and malignant tumors of the orofacial region there has repeatedly been investigated, after radical surgery, the cell reaction to autologous tumor tissue applied at indicated time intervals on a small skin lesion. The prognostically favorable picture of a cellular-type hypersensitivity reaction with massive participation of lymphoid cells and basophil granulocytes was only found in some basaliomas and leukoplakias. Conversely, spinocellular carcinomas are, irrespective of localization, characterized by areactivity.
