A nurse-managed multiple sclerosis clinic: improved quality of life for persons with MS.

Multiple sclerosis (MS) is a complex disease, the symptoms of which may wax and wane on a daily basis. Conventional treatment for MS patients may involve numerous trips to a variety of clinics, physicians, and rehabilitation settings. This article describes a nurse-managed MS clinic which employs an interdisciplinary and holistic approach to treatment of MS patients with the goal of improving their quality of life and coordinating their health care. In this article, the services of the clinic are described, emphasizing the roles of the nurse practitioner (clinic manager), rehabilitation nurse specialist, and nursing director of the hospital-based home care program. A case study example of the interdisciplinary nature of this clinic's program with one MS patient is included.

Microwave-stimulated immunogold silver staining.

The effect of microwave-generated energy for the demonstration of immunoglobulins in periodate lysine paraformaldehyde dichromate fixed paraffin sections of human reactive tonsil using the immunogold silver staining technique has been evaluated. Results of the study show that microwave stimulation permits incubation times of both the primary antibody and the immunogold reagent to be greatly reduced. Specific staining of equal intensity to that produced using the longer standard procedure together with minimal background staining is achieved.

A neural circuit analysis of visual recognition memory: role of perirhinal, medial, and lateral entorhinal cortex.

Using a continuous recognition memory procedure for visual object information, we sequentially presented rats with eight novel objects and four repeated objects (chosen from the 8). These were selected from 120 different three-dimensional objects of varying sizes, shapes, textures, and degree of brightness. Repeated objects had lags ranging from 0 to 4 (from 0 to 4 different objects between the first and repeated presentation). An object was presented on one side of a long table divided in half by an opaque Plexiglas guillotine door, and the latency between opening the door and the rat moving the object was measured. The first presentation of an object resulted in reinforcement, but repeated presentations did not result in a reinforcement. After completion of acquisition training (significantly longer latencies for repeated presentation compared with the first presentation of an object), rats received lesions of the perirhinal, medial, or lateral entorhinal cortex or served as sham operated controls. On the basis of postsurgery testing and additional tests, the results indicated that rats with perirhinal cortex lesions had a sustained impairment in performing the task. There were no sustained deficits with medial or lateral entorhinal cortex lesions. The data suggest that recognition memory for visual object information is mediated primarily by the perirhinal cortex but not by the medial or lateral entorhinal cortex.

Comparison of enamel microabrasion techniques: Prema Compound versus a 12-fluted finishing bur.

This clinical study was done to compare the effectiveness of microabrasion with Prema Compound versus a high speed 12-fluted bur with water spray in removing incisor hypomineralized defects. Twenty-one patients had two incisors with enamel surface defects. It was randomly decided to use Prema Compound on one of the incisors and a 12-fluted bur on the contralateral incisor. One operator did all the procedures, but the tooth's color changes were rated by three observers who rated color photographs pre- and postoperatively, and one of the three rated each patient's teeth in person. The data revealed that there was no significant difference between Prema Compound and the 12-fluted bur in removing incisor hypomineralized defects. Both microabrasion techniques significantly improved the tooth's color, but the Prema method took over 9 minutes longer. The 12-fluted bur caused no clinically identifiable marks on the enamel surface. Both techniques were very reliable in removing the brown hypomineralized enamel defects, but were far less reliable in removing the white mineralization defects. The 12-fluted bur appeared to be an easier and equally effective alternative to Prema Compound for mineralization defects in incisors.

Youngsters get an introduction to sexual health clinics.

This paper describes the development of mock sexual health clinics for young people in an area with high rates of teenage pregnancy and poor awareness of sexual health services. The multi-disciplinary initiative was developed in partnership with local schools and aimed to promote access to sexual health and contraceptive services by breaking down barriers and misconceptions. Subsequent registrations of young people at local family planning clinics have increased dramatically and a full evaluation is being undertaken.

Regulation and deregulation of G2 checkpoint proteins with cisplatin.

BACKGROUND: The G2 checkpoint has a key role in the response to DNA damage. G2 arrest following DNA damage is associated with inactivation of the protein kinase cyclin B-cdc2. The role of changes in protein expression in enzyme inhibition is controversial. METHODS: Expression of cyclin B1, cdc2, cdc25c, total protein and DNA content were examined by flow cytometry in two lung cancer cell lines. Changes in protein expression were compared to cell cycle matched controls under conditions associated with and without G2 arrest. RESULTS: Total protein increased with G2 arrest and decreased with cell death. Changes in cdc2, cdc25c and p16 paralleled changes in total protein. A larger increase in cyclin B1 was seen which was due to cell cycle redistribution. Deregulation of cyclin B1 was seen with a sublethal dose of cisplatin. CONCLUSIONS: Our results do not support the model that changes in expression of cyclin B1 or other checkpoint proteins mediate G2 arrest after cisplatin.
