Maternal plasma and urinary hormone levels during and after a successful abdominal pregnancy.

In a successful abdominal pregnancy, urinary oestriol levels were about the fifth centile, but other hormone concentrations suggested that placental function was normal until 32 weeks gestation. After that, plasma oestrogen concentrations levelled off and plasma placental lactogen concentrations declined. Pre-eclampsia developed at 34 weeks and necessitated the delivery at 36 weeks and 2 days of a live normal female infant by laparotomy. The placenta was not removed. All hormone levels fell rapidly during the first weeks of the puerperium and then more slowly during the next six weeks to non-pregnancy levels. Plasma progesterone, urinary pregnanediol, and plasma oestradiol were the slowest to return to non-pregnant levels.

Single dose metronidazole prophylaxis in gynaecological surgery.

A double blind study of 200 patients was undertaken to evaluate the prophylactic effect of 2g metronidazole rectal suppositories given prior to major gynaecological surgery. The patients receiving metronidazole were found to have a significant reduction in the quantity of postoperative pyrexia as assessed by the fever index. Metronidazole prophylaxis was associated with a dramatic reduction from 18 percent to 1 percent of major postoperative wound and pelvic infections. Four patients in the study developed proven Bacteroides infections, all were in the placebo group.

Simplified antibiotic prophylaxis for vaginal hysterectomy.

Single dose administration of antibiotics prior to vaginal hysterectomy was found to be an efficient and safe way of providing prophylaxis against infection. Metronidazole was as efficient as ampicillin in achieving a significant reduction in major morbidity, pelvic sepsis, and the "quantity" of postoperative fever. Antibiotic prophylaxis reduced hospital stay by an average of 3 days.

Social skills training and cognitive restructuring with alcoholics.

Social skills training (SST) has been shown to produce significant increases in skills immediately after training. However, persistence of skills has not been demonstrated during the follow-up period. The present study investigated this issue using SST and cognitive restructuring (CR) methods. Twenty-four hospitalized "alcoholics" were allocated to one of the following three group: (i) SST, (ii) CR, and (iii) traditional supportive therapy (TST). Twelve 2-hour sessions were conducted with each group. Before, following, and three months after treatment (i) behaviour ratings through structured interview, (ii) nurses' ratings, (iii) self-reported ratings of social skills and (iv) alcohol intake were recorded. Analysis of variance indicated SST to be significantly superior on all measures compared to CR, which was superior to TST immediately after training. However, 3 months after treatment, the subjects in the CR group indicated greater skill increments than the SST group subjects who, in turn, showed greater improvement than subjects in the TST group. These results led to the conclusion that CR was more effective than SST and TST in producing long-term skill increments. The significance of these findings in providing support for the social learning model of alcohol dependence and cognitive restructuring theory of social skills modification is discussed.

Meckel's syndrome (dysencephalia splanchno-cystica) in two Pakistani sibs.

A Pakistani couple, who were first cousins once removed through their fathers, and whose mothers were also related, had two liveborn children, a boy and a girl. Both children died within 2 hours of birth with occipital encephalocele, microcephaly, polycystic kidneys, and cystic distension of intrahepatic bile ducts. Both children had normal karyotypes. These abnormalities constitute Meckel's syndrome (dysencephalia splanchno-cystica); this is the fifth report of parental consanguinity, adding further support to the evidence for autosomal recessive inheritance of the disorder.

Intramolecular disorder and its relation to mesophase structure in lipid/water mixtures.

NMR spin-half pair dipolar echo measurements are reported for the lamellar (dispersions and multibilayer stacks) and hexagonal phases of potassium palmitate/2H2O mixtures. In the lamellar Lbeta and Lgamma (gel) phases the alkyl chains are rigid and perfectly ordered, while in the lamellar Lalpha and hexagonal phases they are flexible and disordered. In particular, the measurements show that in the fluid lamellar Lalpha phase the chain is "bent" at the C9-C10 segment; but is "straight" in the hexagonal phase.

Radioimmunoassay kits for thyroid hormones.

Characteristics of the T4 and T3 radioimmunoassay kits that are commercially available in the UK are described. Useful comparative data and criticism are given about many of these kits. Some unacceptable variation in accuracy is demonstrated.
