Experimental determination of transfer coefficients of 137Cs and 131I from fodder into milk of cows and sheep after the Chernobyl accident.

Following the Chernobyl accident in April 1986, the transfer of 131I and 137Cs from feed to milk was studied under experimental and common agricultural conditions. From measurements in different dairy farms in Southern Bavaria, equilibrium transfer coefficients for cow's milk were calculated to be 0.003 d L-1 (range 0.0015 to 0.005) for 131I and 0.003 d L-1 (range 0.0025 to 0.004) for 137Cs. In feeding experiments with cows and sheep under more controlled conditions, milk transfer coefficients of 0.007 d L-1 (range 0.0055 to 0.0081) for 131I and 0.003 d L-1 (range 0.0023 to 0.0053) for 137Cs were obtained for cows, while for sheep the 137Cs transfer coefficient was higher: 0.06 d L-1. The kinetics of the Cs transfer from fodder to cow's milk can be described by two exponential terms assuming biological half-lives in milk of 1-2 d and 10-20 d. The use of a fast component with 1.5 d and a fraction of 0.8, and a slow component with 15 d, gives a good approximation to the kinetics for all cows in this experiment.

Improving patient care through problem-solving groups.

In a competitive healthcare environment where as much attention must be given to quality of care as to cost containment, hospitals must learn to involve their lowest-paid paraprofessionals in decision making, since often these people have as much or more contact with patients than do physicians and nurses. One effective employee-involvement technique is the problem-solving group (PSG), which works to identify, analyze, choose, and implement solutions to various problems that influence the quality of patient care. A PSG project involving a typical paraprofessional unit was begun in spring 1985 at Emory University Hospital, Atlanta. The project's developmental stages were the following: Gathering data; Identifying problems; Providing feedback; Selecting PSG members; Focusing on problems; Generating solutions; Choosing the best solution; Implementing solutions; Managing successful outcomes. The PSG project was successful in addressing almost all the 15 identified problems in the department. Involving the staff in the PSG project resulted in higher morale, greater satisfaction with management, and a more favorable attitude toward their jobs.

Structure of the amino acid-containing capsular polysaccharide (K54 antigen) from Escherichia coli O6:K54:H10.

The structure of the K54-antigenic polysaccharide (K54 antigen) of Escherichia coli O6:K54:H10 was elucidated by determination of the composition, 1H- and 13C-n.m.r. spectroscopy, periodate oxidation, and a study of the oligosaccharides obtained by partial hydrolysis with acid. The K54 polysaccharide consists of----3)-beta-D-glucosyluronic acid-(1----3)-alpha-L-rhamnosyl-(1----repeating-units. Of the glucuronic acid residues, approximately 85% are substituted in the ratio 9:1 with L-threonine and L-serine amidically linked to the carboxyl group. The K54 polysaccharide has a molecular weight of approximately 160,000, corresponding to approximately 380 repeating-units.

Structure of the fructose-containing K52 capsular polysaccharide of uropathogenic Escherichia coli O4:K52:H-.

The chemical structure of the K52 antigenic capsular polysaccharide (K52 antigen) of Escherichia coli O4:K52:H- was elucidated by composition, nuclear magnetic resonance spectroscopy, methylation, periodate oxidation before and after graded acid hydrolysis and by oligosaccharide analysis. The polysaccharide consists of a backbone of alpha-galactose units interlinked between C1 and C3 by phosphodiester bridges. This poly(alpha-galactosyl-phosphate) is substituted at C2 of each galactose unit by beta-fructofuranose residues. About 80% of the galactose units are O-acetylated at C4 and about 10% of the fructose units are both O-acetylated and O-propionylated at C1. The K52 polysaccharide has an average molecular mass of 34 kDa, thus consisting of approximately 65 fructosyl-galactosyl-phosphate repeating units.

[Stability of beta-HCG in old blood stains].

Pregnancy was diagnosed from old blood stains by means of radioimmunoassay of beta-HCG. Up to 45 days, the age of the blood stains did not influence values, and extraction by ultrasound did not improve the results.

Structure of the 3-deoxy-D-manno-octulosonic acid-(KDO)-containing capsular polysaccharide (K14 antigen) from Escherichia coli 06:K14:H31.

The chemical structure of the K14-antigenic polysaccharide (K14 antigen) of Escherichia coli 06:K14:H31 was elucidated by determination of the composition, 1H- and 13C-n.m.r. spectroscopy, periodate oxidation, and study of the oligosaccharides obtained by partial hydrolysis. The polysaccharide consists of [O-(2-acetamido-2-deoxy-beta-D-galactopyranosyl)-(1 leads to 5)-O-(3-deoxy-beta-D-manno-octulopyranosylonic acid)-(2 leads to 6)] repeating units, approximately 60% of the octonic acid units being O-acetylated and approximately 10% O-propionylated at O-8. The sequence of acetylated and propionylated residues is not known. The serologically-specific part of the K14 antigen residues in the polysaccharide part.

[Differentiation of functional knee movement in hemophilic patients by means of ultrasonic topometry (author's transl)].

First experiences using UST (Ultra-Sound-Topometry) in functional analyses of the loco motor system show: The motor patterns in gait analyses can be analyses can be visualized in such high steric- and time-resolution, that even slightest disturbances can be seen and localized. The high resolution of the procedure allows exact analyses of the movement patterns within the joints. In the knee, for example, we could demonstrate, how in slight to marked pathology the stress in the cartilagenous layers is disturbed in different ways. Out of this, conclusions, concerning the intraarticular destructions and their effects on the kibernetic chain, can be made.

[Operative treatment of hemophilic arthropathy in the knee (author's transl)].

Within the knee occur 27.7% of all hemophilic bleeding episodes. Therefore it is the organ in which hemophilia is likely to be manifested. Changed in the sense of hemophilic arthropathy made 117 operative interventions necessary in the years 1973 through 1981. The osteotomies, performed to correct axial deviations (38 high tibial head osteotomies and 22 supracondylar osteotomies) formed the biggest group, followed by 34 hamstring-release operations, 6 synovectomies, 5 temporary epiphysiodeses, 4 ventralisations of the tibial tuberosis. An endoprosthetik replacement was necessary in only one case. A follow-up study after osteotomies and hamstring-release procedures after one to eight years, correspondingly six months to eight years, gives good results, concerning improved motility, subjective satisfaction and reestablishment of stabilizing muscular performance. In presence of sufficiently controlled substitution of the missing clotting factors, there is no increased operative risk, compared with non-hemophiliacs.

[Hemophilic pseudotumors].

A review of the literature was made with special interest in morphology, pathogenesis and treatment of hemophilic pseudotumors. 110 reports of hemophilic pseudotumors were evaluated and compared with 25 own cases. The separation according to the inflicted site and the primarily involved anatomical structure revealed three types of hemophilic pseudotumors: 1. Intramuscular hematomas which enter secondarily into the periosteum. a) In the diaphyses of long tubular bones (femur 49, lower leg 14, humerus 4, forearm 2). b) In the iliac bone (37). 2. Intraspongious cysts in short cancellous bones (calcaneus 14, talus 5, scapula 3). 3. Direct subperiostal hematomas in the phalanges and metacarpals in the hand (thumb 5, finger 1, metacarpal 1). Concerning treatment, there is to state, that in general a conservative approach (substitution of the missing clotting factor and immobilization) is promising. The operative indications are demonstrated and determined, using case reports.

[Conclusive significance of gait analyses at the pelvis-leg skeleton (author's transl)].

Functional analyses of the gait of healthy male volunteers show approximately identical coordination of movement under standardised conditions. In contrast thereto, marked differences can be seen if the joint functions are disturbed and if there is an associated muscular insufficiency. These changes can be seen even in very minor joint affections in pain-free condition. The standardised course of the functions is suitable for describing characteristic changes in gait and to reveal some of the causal links. If these standardised conditions are employed, they offer the only possibility of assessing normalisation or progressive degeneration of pathological movement patterns.

[BMT 9141--new foetal monitor (author's transl)].

The BMT 9141, a foetal monitor, is described in this paper. It has succeeded the BMT 504, an earlier model which has worked well. It has been designed for alternative external or combined internal foetal monitoring. An automatic labour evaluation unit has been added as a new element. The monitor has stood its clinical test with good success.

[Regional anaesthesia in haemophiliacs (author's transl)].

Since the middle of 1976 spinal and peridural anaesthesia are also being used for orthopaedic surgery of the lower extremities in haemophilic persons, the most important pre-requisites for the use of these techniques are: that it is a case of genuine haemophilia, that the deficiency is fully made up before and during anaesthesia and that the most important coagulation parameters are closely watched in co-operation with the Hemophilia Centre during the entire period. 46 haemophiliacs were given spinal and epidural anaesthesia via a catheter. A great advantage of these long-acting techniques is that less analgesics are needed during the postoperative period. Continuous peridural anaesthesia ensures freedom from pain without interfering with motoricity. The treatment of contractures of the joint by active and passive exercises has thus a better chance of success. Provided factors VIII/IX were kept normal there have so far not been any haemophilia-related haemorrhages into the spinal canal or cord or neurological disturbances that could be attributed to these anaesthetic techniques.

Hypocalcemia during restraint stress in rats. Indication that gastric ulcer prophylaxis by exogenous calcium interferes with calcitonin release.

The influence of restraint stress on serum calcium (Ca) and phosphate was studied in normal and thyroidectomized rats. In addition the response of gastric stress ulcer index, blood gastrin and glucagon to exogenous Ca was investigated. In intact as well as in thyroidectomized animals serum total, ionised and previously injected radioactive Ca decrease during an 8h stress period, whereas inorganic phosphate increases. Together with a constant specific activity these findings are consistent with hypoparathyroidism and calcitonin independent hypocalcemia during stress. Intragastric infusion of 45 mg/kg Ca-gluconate per 8h proves to be a potent anti-stress ulcer regimen in intact and neck-sham operated, but not in thyroidectomized rats without and with additional adrenal demedullation. Gastrin and glucagon were not correlated with calcemia during either stress alone or stress combined with intragastric Ca infusion. It is suggested that the development of gastric stress ulcerations can be prevented by a Ca-mediated release of endogenous calcitonin.
