[Experiences with a new double-lumen venous catheter in anesthesia and intensive care medicine].

A new double-channelled catheter with one main lumen and one accessory small lumen is described. The different ranges of application and the advantages are discussed. Flow rates through the two channels are measured and compared with the well-known "Cavafix"-system. Reactions of incompatibility and possible changes in efficacy of drugs and infusions can be provided with this kind of central venous catheter.

Antihypertensive action of dietary polyunsaturated fatty acids in spontaneously hypertensive rats.

The experiments were carried out in order to clarify the mechanisms of attenuation of hypertension development by means of diets enriched with polyunsaturated fatty acids (PUFA) in spontaneously hypertensive rats (SHR). Female SHR were fed a linoleic acid rich (LAr) diet (13.3 cal % LA, sunflower oil), a linolenic acid rich (LNAr) diet (18.8 cal % LNA, 3.9 cal % LA; linseed oil) and a PUFA deficient diet (0.5 cal % LA, hydrogenated palm kernel fat), respectively, during the last week of pregnancy and during the suckling period. Corresponding diets were given to the male offspring up to an age of 16 weeks. Our results demonstrate that the attenuation of hypertension development in LAr and LNAr fed male SHR was paralleled by an increased in-vitro uptake of 14C-norepinephrine into cardiac and aortic tissues as well as an increased degradation rate of 14C-norepinephrine in cardiac tissue. Ex vivo prostaglandin (PG) formation was reduced after LNAr diet in the aorta (PGF2 alpha, PGI2-like material) and in the kidney medulla (PGE, PGF2 alpha). It is concluded that an increased catecholamine inactivation may play a role in the attenuation of hypertension development in LAr and LNAr diet fed SHR.

[Use of etomidate within the scope of neurosurgery].

The effect and the applications of Etomidate pro infusion (125 mg/ml) are discussed under special consideration of neurosurgical patients. Besides the application for anaesthesia, the administration of Etomidate pro infusion in the intensive-medical range for sedation, reduction of cerebral pressure and as an anticonvulsant is dealt with. The possibility of an adrenal insufficiency under the action of Etomidate pro infusion is discussed on the basis of the literature controversial to this subject.

[Amrinone in catecholamine refractory heart failure in septic shock].

The effect of the new positive inotropic and vasodilatator bipyridine-derivate Amrinon on catecholamine-refractive heart insufficiency in septic shock is described. A bolus dose of 1 mg/kg b.w., followed by continuous infusion of Amrinon 10 micrograms/kg b.w./min improved the haemodynamic parameters of all seven patients. The severe tachycardia before therapy was diminished more than 30%, the blood-pressure increased about 25-30%. RAP, PAP and PCWP showed a diminution of 35-45%. The cardiac output increased nearly 100% under therapy. All patients had IPPV with high inspiratory oxygen concentration, with inversed-ratio-ventilation and high positive end-expiratory pressure. Under Amrinon-therapy the initial pulmonary insufficiency diminished. The oliguria/anuria existing before Amrinon-therapy was improved also. Amrinon was given over 24-36 hours, the total dose was between 800 and 1440 mg. Six of the seven patients survived their severe illness; one patient died of pulmonary embolism, confirmed by autopsy, four weeks after Amrinon-therapy.

Perinatal linoleate deprivation impairs learning and memory in adult rats.

Rats receiving polyunsaturated fatty acid (PUFA) deficient diets during the perinatal period showed in adult age undisturbed acquisition of a footshock motivated brightness discrimination task, but a significant impairment of retention. The same effects on retention were obtained in rats receiving the PUFA deficient diet in adulthood, when the behavioral parameters were investigated at the end of the dietary treatment.

C-reactive protein and clinical kidney transplantation.

50 patients were serially monitored for C-reactive protein (CRP) in serum after kidney transplantation. Compared with a control group the post-operative peak at days 1 and 3 was strongly depressed in the immunosuppressive-treated transplant group (p less than 0.01). In 29 out of 30 patients CRP concentration showed an average rise from 9.4 to 30.0 micrograms/ml between the 5th and 2nd day before onset of the rejection episodes (p less than 0.01). The dynamics of CRP concentration before, during and after prednisolone anti-rejection bolus therapy was considered in 22 cases. The first bolus led to a greater or smaller decrease in CRP concentration in almost all patients. After the second bolus treatment CRP dropped to zero already in more than half of the patients with reversible rejection episodes (p less than 0.01). In case of irreversible rejection crises CRP never reached zero.

[Use of alfentanyl in short-term anesthesia. Initial results].

The administration of the new opiate-analgetic alfentanil in combination with etomidate supplemented by N2O/O2 for short anaesthesia is described. The influence on blood pressure, heart rate, paO2, paCO2, rate-pressure-product (RPP) and glucose rate has been investigated. In the intraoperative and early postoperative period, the cardiac and respiratory system was unchanged. After anaesthesia all patients were very cooperative.

Chronobiologic and metabolic characterization of rats fed low or high linoleic acid diets.

The influence of a modified endogenous prostanoid synthesis on circadian rhythmicity at different functional levels (prostanoid formation, behavioural parameters, circulatory indices, hematological parameters, energy metabolism) was investigated. The daily rhythmic changes observed for all parameters were partly modified by the variation of dietary linoleate intake.

[Etomidate as an anticonvulsive agent].

Experiences with etomidate as an agent with hypnotic, brain protective and anticonvulsive effects are discussed. The application of etomidate is described at one patient with epilepsy and withdrawal symptoms after resuscitation.

Myocardial biosynthesis of prostaglandins with special consideration of prostacyclin and the influence of dipyridamole and propranolol.

Experiments with isolated perfused hearts of guinea pigs and rats showed that cardiac action is linked to formation of prostaglandinlike substances (PLS) and prostacyclin (PGI2). Perfusion of the hearts with arachidonic acid or pretreatment with a linoleic-acid-supplemented diet significantly increased the content of PLS and PGI2 and exerted an economic effect on the heart performance. Dipyridamole induced a marked increase in the coronary flow and PGI2 formation of the hearts but decreased the enhanced myocardial PGI2 biosynthesis after perfusion with arachidonic acid. Propranolol also caused a rise in PGI2 efflux but did not show any influence on PGI2 formation after arachidonic acid. Dipyridamole and propranolol prevent decreased PGI2 formation after acetylsalicylic acid, supporting the view that a combination of these drugs exerts a preventive effect in patients with angina pectoris and heart infarction.
