The authors advocate an elective approach to the lower part of the humeral (shoulder) joint by an original axillary route. This route is external to the short head of the biceps brachii muscle which is retracted together with the neurovascular bundle. The articular capsule is opened through the space between the subscapular and latissimus dorsi muscles. The skin incision is cosmetic. No muscle is cut, and in surgery for anterior instability of the shoulder the crucial area is automatically exposed. This technique, however, requires two assistants and may be arduous in very muscular subjects. In addition, the visibility of the rest of the humeral joint is limited. With a few precautions, the neighbouring vessels and nerves are well protected.
Three patients with POEMS syndrome were studied: two of them had myeloma, whereas the third had M protein without myeloma. Peripheral nerve biopsies showed no deposits of anti-Ig sera at direct immunopathological examination. Ultrastructural study revealed myelino-axonal degeneration in all three cases. This was associated in the first case with a dramatic loss of myelinated fibers and there were lesions consistent with acute degeneration in the second case. In addition, all three had some fibers showing uncompacted myelin lamellae. This peculiar modification has previously been reported only in cases of dysglobulinemia, lymphoma and inflammatory demyelinating polyneuropathy, but its mode of formation remains unclear.

A 74 year old woman without previous cardiovascular disease presented with pericardial effusion and tamponade. Two-dimensional echocardiography displayed a pericardial mass, but a diagnosis of tumour was not envisaged. Pericardiocentesis produced an exudative fluid with lymphocytes but no malignant cells. Pericardial biopsies through axillary thoracotomy indicated chronic progressive pericarditis. As the patient's haemodynamics deteriorated, sternotomy was carried out showing diffuse pericardial sclerosis corresponding, at histology, to a mixed, predominantly fibroblastic pericardial mesothelioma. The authors describe the main clinical characteristics of pericardial mesothelioma, always discovered belatedly at surgery or necropsy. They insist on the value of non-invasive methods (two-dimensional echocardiography, computed tomography) to diagnose a pericardial tumour, and on the poor prognosis of mesotheliomas, treatment still being uncertain due to the small number of cases and to the early occurrence of cardiac complications.
The authors report a case of hemangioendothelioma of the penis. They emphasize the value of immunohistochemical markers for precise pathological diagnosis in strictly localised disease. They improvement of the prognosis by adjuvant treatments has not been demonstrated.
Four patients with idiopathic livedo reticularis and multiple cerebral ischemic events leading to a severe dementia are reported. Imaging (CT, MRI, angiograms) and pathological features (in one case) are presented. Similar clinical features have been described previously in Divry-van Bogaert's disease and Sneddon's syndrome. Data from our cases and a review of the literature did not find distinctive characteristic between these entities. Thus, for practical purposes, considering them as one group appears at present warranted.
12 cases of spinal osteotomy in patients with ankylosing spondylarthritis are reported. It concerns 3 cases of cervical osteotomy and 9 cases of lumbar osteotomy with severe kyphotic deformity. The techniques and the operated cases are analyzed. The indications of this surgery, which is not devoid of neurological risks, are rare. It is only justified in severe deformities in order to restore a normal visual field for the patient.

Sequential measurements of plasma alkaline phosphatase activity were made in 84 pre-term infants delivered before 38 weeks gestation. In 67% of infants, without evidence of rickets but in whom three or more measurements were made and the peak activity was less than 10 times the adult reference range, the activity rose to a peak and declined to previous levels, or lower. The time taken for these changes varied greatly. The range of values was inspected at each week of age, and in all but six cases the level did not exceed 10 times the upper limit of the adult reference range. Three infants had radiological evidence of rickets, and of the six cases in whom higher levels were found, only one had radiological evidence of rickets; the remainder did not. It is recommended that in the latter, the high plasma alkaline phosphatase activity should be regarded as evidence of subclinical bone disease.

In the assessment of the bacteriological quality of water from 1,500 drills World has not in Burkina-Faso, three methods have been analyzed comparatively. The first, relying upon bacterial production of H2S as indicator of faecal pollution and proposed by Indian workers, has been found unreliable for determining water potability. The second, based upon the nitrate content of water, is unable to reveal water contamination by human and animal excreta. The third method, employing a membrane filtration, is to be commended since it is well adapted to the Sahelian conditions. Simple, reliable, of low cost, this method is providing the quality assurance of water potability, a requirement which must accompany the drilling programs.
Two cases atlanto-occipital dislocation with survival are reported. The displacement was anterior. One case was associated only with some pyramidal signs and was diagnosed fourteen months after the injury. The other was associated with immediate tetraplegia. Both cases were treated by occipitocervical fusion with plate fixation. The second case was reduced during operation. A review of the literature has shown that dislocation with anterior displacement is more severe than with posterior displacement. The diagnosis is always difficult. The authors show the importance of certain radiological signs such as the curved anterior line joining the odontoid process to the occiput. The need to reduce the displacement in old lesions is discussed.

A 49 year old caucasian female with Hashimoto thyroiditis, developed during two years a neurological disorder with tonic-clonic and myoclonic seizures and confusional states. Some attacks were followed by a transient postictal aphasia. Some parallelism was noted between the clinical state and TSH levels. Neurological events disappeared with the normalisation of thyroïd functions. This association of Hashimoto thyroiditis and myoclonic encephalopathy has been rarely published. Pathogenesis could be double. Focal signs could be due to an auto-immune mechanism, perhaps through a vasculitis. A non-endocrine central action could explain diffuse signs: tonic-clonic seizures, myoclonus and confusional episodes.
Three patients developed a progressive flaccid paraparesis without sensory or sphincter disturbances, following radiotherapy for lymphoma in two cases and carcinoma of testis in one case. The course was progressive with stabilization between two and four years. Electrophysiological study suggested anterior horn cell damage the mechanism of which remains unclear.
