[Surface properties of Lactobacillus strains. I. Hemagglutination and hydrophobicity].

Bacteria possess many surface membrane properties, both mechanical and biochemical, that allow them to interact with their environment These properties may affect a host in either positive (beneficial) or negative (pathological) ways. All surface properties of bacteria are yet unknown therefore we attempt to increase our knowledge regarding specific strains of lactobacilli, by examining the known properties including hemagglutination and hydrophobicity. The purpose of this study was to evaluate the cell surface properties of certain strains of Lactobacillus. These strains isolated from the human vagina and gastrointestinal tract were selected because of their antagonism toward aerobic and anaerobic bacterial pathogens. Part I discusses the hydrophobicity and hemagglutination abilities of these Lactobacillus strains.

[Surface properties of Lactobacillus strains. II. Adherence to tissue culture surfaces].

In the second part of this study, we evaluated the adherence properties of specific strains of Lactobacillus isolated from both the human vagina and gastrointestinal tracts. Lactobacilli taken from the vagina and GI tract were tested for their adherence to A431 vaginal tissue, and to CaCo-2 cells taken from the gastrointestinal tract. The Lactobacillus strains with the most marked adherence to the respective cell lines were examined under the electron microscope. These images revealed the presence of a substance morphologically resembling slime, which probably possess unknown active properties.

In vitro antibiotic susceptibility of Bacteroides fragilis strains isolated from excised appendix of patients with phlegmonous or gangrenous appendicitis.

Out of 34 studied after-appendectomy tissues of adult and child patients 86 different strains of anaerobes were isolated. The antibiotic susceptibility of 30 isolated B. fragilis strains was tested using E tests. All studied strains were sensitive to imipenem, clindamycin and penicillin/tazobactam. Sensitivity to penicillin and cefoxitin was variable among these strains. One strain resistant to metronidazole (MIC--256 mg/L) and 3 strains with increased MIC to metronidazole were detected. Most of isolated strains were beta-lactamase producers.

[Surveillance of hospital infections in Poland--current condition and prospects].

This paper outlines the current achievements in the control of hospital acquired infections in modern countries as well as in Poland in recent years. Studies related to the incidence of hospital-acquired infections have been conduced in western Europe, Scandinavia, the United States, and many other countries around the world. Because of these already completed studies, we are able to develop epidemiologic analysis specifically centered upon the incidence and increased costs involved with hospital-acquired infections. In Poland this type of surveillance was unable to be attained until long after its institution. However, over the past few years, the Polish Society of Hospital-Acquired Infection, has worked to bring recognition of the seriousness of this problem to a representative group of Polish hospitals. One of the most important conclusions reached was lack of departmental control which, using standards previously agreed upon by modern countries, could provide the necessary surveillance over hospital-acquired infections. Currently, surveillance is carried out by treating physicians without the help of specially-trained infection control nurses, epidemiologists and medical microbiologists. The basic challenge for the Polish health service is that of the development and realization of a professional infection control team. This professional team would work develop guidelines for the protection of patients and hospital personnel from hospital-acquired infections.

[Mortality associated with hospital acquired infections. Analysis based upon data from the Polish National Program for Control of Infection in 1998].

OBJECTIVE: The purpose of this study was to analyse patient mortality connected with hospital acquired infections in Polish hospitals during the year 1998. METHODS: Data was taken from 96 hospitals involved in the National Programme for the Registration of Hospital-Acquired Infections which was established in 1998. The data concerning the presence or absence of 19 specific infection forms was gathered using the passive method. RESULTS: The mortality of patients during 1998 in which hospital acquired infections were the direct or indirect cause of death was 6.9% Pneumonia and blood stream infections were the most frequent causes of death in patients hospitalised in Intensive Care Units. The most important risk factors for the development of hospital acquired infections, included old age, mechanical ventilation and urinary bladder catheterization. CONCLUSIONS: Selected epidemiological indicators for mortality rate were similar to those reported in countries with high quality infection control. A marked difference, however, was seen when comparing dating involving blood stream infections. The level of morbidity was found to be less in Poland than in developed countries. The authors feel that this low morbidity may be secondary to the decreased frequency of blood culture testing in Poland.

Actinomycotic osteomyelitis of the mandible: review of 15 cases.

The authors have analysed 15 cases of actinomycotic osteomyelitis of the mandible. Primary actinomycotic inflammation of the mandible was diagnosed in nine patients, secondary in six patients (following fracture or resection of the mandible). The diagnosis was established on the basis of the clinical examination confirmed by bacteriological and/or histopathological tests. The basic method of treatment was antibiotic therapy of long duration. Surgical treatment was performed in all patients. Twelve of fifteen patients who had received the therapy and proved asymptomatic for 1 year to 23 years were recognized to be cured of the disease. In three patients, a single recurrence of the disease was observed after 1.5-2 months.

[Results of prophylactic and curative antibiotic therapy in biliary surgery].

Prophylactic antibiotic regimen (gentamicin, clindamycin) was assessed in a group of 104 patients operated electively on biliary lithiasis (cholecysto- and choledocholithiasis). Antibiotic prophylaxis was performed in group of 20 patients, in 6 of them antibiotics were continued postoperatively. Intraoperatively antibiotics were given in a group of 20 patients--curative treatment. Control group consisted of 58 patients. We concluded that "one shoot" antibiotic therapy is valuable in controlling of wound infections. But the most effective method of reducing local septic complications rate combines prophylaxis with prolonged antibiotic administration.

Hemagglutination by Staphylococcus saprophyticus and other coagulase-negative staphylococci.

Hemagglutination tests were performed to specify surface lectins (hemagglutinins) of four coagulase-negative staphylococcal species: S. saprophyticus (31 strains), S. epidermidis (5 strains), S. haemolyticus (3 strains), and S. warneri (3 strains). All strains of S. saprophyticus agglutinated sheep red blood cells (RBC) and the hemagglutination was inhibited by N-acetylglucosamine (GlcNAc) plus either N-acetylgalactosamine (GalNAc, 15 strains) or N-acetylneuraminic acid (NANA, 16 strains). Those strains showing inhibition by GalNAc also agglutinated horse RBC while those inhibited by NANA agglutinated rabbit RBC. The former type was more common among urinary tract isolates (10/15) and the second one among respiratory isolates (9/14). The eleven strains of other staphylococci agglutinated rabbit (and not sheep or horse) RBC; this hemagglutination was never inhibited by GlcNAc but instead by NANA alone or together with another sugar (7 strains) or by other sugars (4 strains, 3 different patterns of inhibition).

[Occurrence of trichomoniasis in women with intra-uterine devices (author's transl)].

The authors investigated the biocenosis in the vaginas of 216 women with IUD and 124 controls without any contraceptive. The incidence of vaginal trichomoniasis in IUD probands was higher with significance than that among the controls. Culture testing is indispensable to rule out the presence of vaginal trichomoniasis. Parasites were not readily detectable from stained and native specimens unless infections were massive. The cytological findings recorded from the above probands were within the normal. Knowledge is inadequate on the risk of infection of women with IUD. This should be another reason for which to improve public health education.
