Reviewing the outcome of 82 patients with differentiated carcinoma of the thyroid gland operated upon and followed up for more than 5 years, the authors were unable to find any difference between "younger" *under 45) and "older" (above 45) patients. They wonder whether the poorer prognosis of differentiated thyroid gland carcinoma in older patients could be due to to the tumour being usually more advanced in such patients at the time of diagnosis rather than to age itself. II this were the case, there would be no need to tailor the operative procedure to the patient's age.

Whenever an excess of blood lipoids is revealed by laboratory tests, investigations should be conducted along two lines: etiological, aimed at detecting an underlying disease amenable to treatment, and dietetic, aimed at determining the patient's calorie intake. In every case, dietetic measures should begin with a normal-lipid, alcohol-free diet providing a number of calories adapted to the patient's weight. Calorie restriction is achieved at the expense of carbohydrates. This adaptation phase usually lasts 2 months. It is made easier by dietetic note-books where the amounts of each food are expressed as parts containing either 10 g of lipids or 10 g of glucides, and where fats are divided into predominantly saturated and unsaturated and glucides into slowly and rapidly absorbed. The problem of adapting the diet to the various types of hyperlipaemias is discussed. In most cases serum lipoprotein levels return to normal after 2 months of adequate diet, but it is essential to reeducate the patients in their eating habits, since successful re-education may help to reduce cardiovascular morbidity and mortality.

Follow-up of 102 patients operated upon for autonomous thyroid nodules is reported. Retrospective study of operative findings and aftermath of surgery clearly demonstrates that the patho-physiological concept of autonomous nodule does not snugly fit practical problems experienced with the management of such patients. The autonomous nodule can be a malignancy. Ipsilateral thyroid lobectomy seems more suitable than nodulectomy. Gross appearance of the thyroid remnant makes sometimes post-operative hormonotherapy unexpectedly advisable. From a theoretical point of view, post-operative thyroid scan, TSH assay and TRH test are mandatory, to assess recovery.

A new direct colorimetric procedure for uric acid assay in serum or urine is described, utilizing a 3,5-dichloro-2-hydroxybenzene sulfonic acid/4-aminophenazone chromogenic system in the presence of horseradish peroxidase and uricase from Aspergillus flavus. This chromogen system has a high absorptivity, affording useful results with sample/reagent volume ratios as low as 0.025. The procedure is applicable to serum, plasma, or diluted urine. A single working reagent is used; the reaction is complete in less than 15 min at room temperature. The red dye formed is measured at 520 nm; a blank sample measurement is not needed. The standard curve for the method is linear for uric acid concentrations up to 1500 mumol/L. Average analytical recovery of uric acid in human sera and urine exceeded 99%; within-run and between-run precision studies showed CV's of less than or equal to 1.2 and less than or equal to 2.2%, respectively. The new procedure correlated well with the uricase/catalase and uricase/ultraviolet methods. The method is suitable for automation.

Results of a retrospective study of 674 frozen sections in thyroid surgery, demonstrated on over all reliability of 98.2%, when compared with final pathological findings, with 651 confirmation in 674 cases. False negatives of malignancy represented 1.6% of lesions thought to be benign, while false positives were obtained in 4.8% of lesions thought to be malignant on frozen sections. Prior to frozen sections, 221 patients had needle punctures and cytological smears studies of the thyroid lesion. When comparing the results after using the two methods, no false positive for malignancy, assessed by cytology, was found on frozen sections, which also corrected but two false negatives of malignancy given by the 177 smears affirmative of benignancy, but reduced the number of equivocal results of cytology from 36 to 1. Both pre-operative cytological examination by needle puncture and intra-operative frozen sections appear, therefore, to be very reliable methods for studying thyroid lesions. Frozen sections are complementary to cytological studies. These results suggest that, in surgical thyroid diseases, frozen sections, need perhaps not to be systematically conducted, but should be selective and reserved for cases in which pre-operative needle cytology affirms malignancy, or raises suspicion.

Serum and seminal gonadotropins were evaluated in 30 normozoospermic and 30 oligozoospermic patients. Mean values of basal serum gonadotropins were higher in the oligozoospermic group than in the normozoospermic group: 3.8 versus 1.8 ng/ml and 3.3 versus 2.2 ng/ml for FSH and LH, respectively. Seminal FSH levels were comparable in the normozoospermic and oligozoospermic groups (0.8 versus 0.8 ng/ml, respectively) and were constantly lower than serum FSH levels in both groups. Seminal LH values were constantly higher than serum LH values in the normozoospermic group, whereas in the oligozoospermic group seminal LH values were higher, similar, or lower than serum LH values. However, no significant difference was noted between the two groups for seminal LH values: 3.6 (normozoospermic group) versus 4.1 ng/ml (oligozoospermic group).
By using immunohistoenzymatic techniques, cells of 4 pituitary adenomas are revealed with beta MSH, alpha and beta endorphins antisera but do not react with 1--24 and 17--39 ACTH antisera. These data suggest that adenomatous cells are able to synthesize and LPH-like molecule without ACTH. The levels of plasmatic LPH and beta MSH, studied by RIA, stay normal. These results are discussed.
