[Family, career and age as socialization factors of the middle and later adult years].

The authors deal with the connections of age, family and vocational activity. By using Marx term "Gratisdienst" they develop an ideal of family functions for the vocational education of grown-up children. Family in this sense is individually and socially of importance as a medium of transmission. The authors also show the importance for society and give examples how the so called post-parental (post-familial) phases in family and lifecourse is orientated towards professional integration of the children. Some aspects how to use and explain this potential funds under socialist conditions in the GDR are discussed.

[Localised aspergillosis of the lung caused by Aspergillus niger].

A 51-year old man with a history of pulmonary tuberculosis suffered an exacerbation without demonstration of tubercle bacilli. The mycological examination of sputum specimens discovered an infection with Aspergillus niger. Diagnosis was confirmed by the pure culture of this fungus in bronchial mucus and the cultural and histological identification in surgical material. As a histological feature strong double-refractile crystalline material was found in fungi and surrounding lung tissue (wall of cavern), probably oxalate crystals, developed from oxalic acid, produced by Aspergillus niger.

[Measurements in triggered CT imaging of moving objects].

This paper describes results of measurements characterising the image properties of a CT apparatus triggered by an "R-peak simulator". For this purpose, a phantom was constructed in which--as straightforward simulation of a cardiac wall movement--a plate made of plexiglass of 5 mm wall thickness moves to and fro with a frequency nu (1 s-1 less than or equal to nu less than or equal to 2 s-1) and an amplitude of A = 10 mm. The number of scanning cycles during a measurement could be varied between 2 and 9. It was evident that the measured density values as well as the positions of the plate which could be ascertained from the CT images in the various phases of movement, especially at low frequencies and a large number of cycles, agree well with the real values; however, it was also found that both at the measured density values and the positions of the plate as seen from the CT images, systematic deviations occur, especially at high frequencies and a small number of scanning cycles.

[Biometry, data registration and data processing in rheumatology].

In the research of rheumatism there appear subjective, semiobjective and objective findings, by the combination of which criteria of activity, function and progression are recognized. Their qualitative criteria are described. Apart from an economically justifiable recording of data, in which regulations of codification and lists of recording of data are separated, the establishment of important statistical numbers of measure (mean value, standard deviation, variation coefficient) is described and explained, by which methods comparisons of groups (t-test after Student) for normally distributed measuring sizes, chi2-test for qualitative characteristics and categories and correlations (correlation coefficient after Bravais-Pearson, rank correlation after Spearman) are performed. The methods are described thus that they may be repeated ubiquitously and not only in the research of rheumatism, be it with or without electronic data processing devices. The factor analysis, which cannot be realised without using data processing, but by means of which we succeed in investigating its structural connections, is also explained. This is done with regard to the possible reduction of extensive laboratory investigation and the connected with this economic advantages.

[Animal studies of quantitative renal occlusion scintigraphy (author's transl)].

Radio-active plastic microspheres with an average diameter of 15 mu were used in eight dogs to measure renal perfusion and the results compared with time-activity curves before and after constriction of the left renal artery. It was shown that the values obtained for renal blood flow by functional scintigraphy are not accurate, particularly for moderate or low blood flow. The reason for this is primarily the distortion of the intravenously injected radio-active bolus and the consequent deviation from the so-called delta function.

[Use of a microcomputer system in radiotherapy (author's transl)].

The authors report their experiences with the successful use of a microcomputers system in radiotherapy. A self-developed programme allowing the calculation of depth dose and irradiation times in telecobalt therapy is given as an example for the application of this system. The authors put stress on a simple handling (video dialogue) and a written documentation. The programme can easily be modified for the use of other telecobalt devices.

The office laboratory in family practice residency programs.

A survey of family practice residency programs explored several aspects of the educational and clinical work of the office laboratories in residency centers. Many residency laboratories were found to have limited equipment and undertook only a few of the common procedures. Only 56 percent of residency programs had identified a specific educator for laboratory teaching. Only 15 percent or programs had established a formal curriculum in laboratory medicine. Curricula were poorly developed in the areas of quality control, equipment purchase, laboratory design, and the training of laboratory personnel. Recommendations are made for improving residency laboratories resident education in laboratory medicine.

[Professional activity of people at an advanced age and of old people (author's transl)].

Professional activity and work at an advanced age take place under certain demographic and socia-conditions and increasingly in close connection with scientific-technological progress and the structural changes involved. For working people who are advanced in years it is incomparably more difficult to adjust themselves to changed working conditions and to changed job requirements and task than it is for their younger colleagues. Since it is left to these people to adjust themselves spontaneously to such developments, it is in many spheres of professional activity no longer possible to realize the necessary measure of adjustment to the conditions in the enterprise and, consequently, to job satisfaction with respect to the individual. This situation finds its expression in the fact that such working people are forced out of commercial activity, at the latest upon reaching the retirement age. A possibility of counteracting such consequences consists in the systematic and faresighted or prospective and preventive definition of job assignments and job layout while allowing for the concrete disposition of working people at an advanced age.

[Teratogenicity of anticonvulsant drugs (author's transl)].

Investigations were done on 111 children of epileptic mothers who used anticonvulsants in 93 pregnancies and none in 18 pregnancies. Hydantoinbarbiturate embryopathy was found in 7.1% after hydantoin monotherapy, in 17.6% after combination of hydantoin and barbiturates or primidone. No embryopathy was seen in children of untreated epileptic mothers. Children of untreated and treated epileptic mothers had an approximately equal frequency of marked single malformations and cerebral damage without dysmorphia. However, malformation and cerebral damage without dysmorphia was found significantly more frequently in children of mothers on anticonvulsant drugs with convulsions during pregnancy as compared to children of mothers without convulsions. Single manifestations and cerebral damage without dysmorphia are probably not caused by anticonvulsants but by convulsions during pregnancy.

Characteristics of a cell line established from a child with familial acute myeloid leukaemia.

The properties of an unusual new cell line from a child with acute myeloid leukaemia (AML) is described. The patient, his mother and grandmother all died within 2 years of AML. Karyotypic analysis suggests that the cells which grow in vitro represent a proliferation of the child's leukaemic cells. The presence of myeloid alkaline phosphatase in these cells suggests that they represent a proliferation of malignant myeloblasts. During the first year in culture a significant percentage of the cells formed E-rosettes, but the cells failed to react with monoclonal anti-T-cell antibodies. The cells are SmIg, Fc, C3 and EBNA negative. Ultrastructural examination of the cells revealed cytoplasmic inclusion bodies which contained virus-like particles. These inclusion bodies and particles were similar to those observed by us in two other human leukaemic cell lines and not unlike those in fresh human leukaemic cells, as reported by others.

Cell surface labeling of proteins and glycoproteins of some human cultured lymphoid cells.

Cell surface proteins and glycoproteins of some human lymphoblastoid- and neoplastic hematopoietic cell lines were labeled by three different techniques: a) lactoperoxidase catalyzed iodination, b) galactose oxidase-tritiated sodium borohydride and c) reductive methylation of free amino groups by tritiated sodium borohydride--a modification of the technique known for the radiolabeling of soluble proteins. Electrophoretic analysis (SDS-PAGE) of labeled surface proteins and glycoproteins resulted in comparable and complementary electrophoretic patterns obtained by these three techniques. Electrophoretic patterns of studied lymphoblastoid cell lines were essentially similar, with an intensively labeled group of glycoproteins (gp44, gp31, gp24). These glycoproteins were markedly reduced on studied neoplastic cell lines. Further differences in several large proteins were observed between lymphoblastoid- and neoplastic cell lines.

[Differential diagnostic criteria in cervico-brachial psychalgia].

In 100 patients with only for a short time existing pains in the region of neck, shoulder and arm and inconspicuous laboratory and X-ray findings by means of the galvanic test of the muscular function and Janda's test of the muscular function a vastly intact nerve-muscle-apparatus was proved. In the ENR-test after Brengelmann and Brengelmann clear introversion values, significantly high neurodizism values and slightly increased rigidity values were shown. Also the VELA-values were significantly higher than in the normal comparative group. In an additional inquiry predisposing biographical references for the existence of actual conflicts were found. As to the inclusion of further biographical data, 68% of intense affect reactions, 16% of functionally fixed neurotic reactions, 9% of primary and 7% of secondary neurotic maldevelopments are concerned. After a 4-week- hard-and-fast date (15 mg/a day) the complaints improved without a clear retrogression of the neurotic constellations. At the same time a change of the initially slightly increased conducting values of the skin and of the vegetative complaints appeared. A cervico-brachial psychalgia is present, when an organic muscle disease was excluded, when in the ENR-test high values to neuroticism, to introversion and rigidity are found and when a temporary connection to actual conflicts are the result.

[Changes in thyroid function after application of iodinated contrast media to normal subjects (author's transl)].

In 39 patients without thyroid disorder the influence of radiographic contrast media (Conray 60, Biligram) on the thyroid function was studied. 6 weeks after the application of the iodine compounds a significant increase of T3 in blood serum could be established being accompanied by decreased TSH level and reduced response of TSH rpoduction to TRH injection. These findings showed to be even more marked 6 months later. It is assumed that the observed hormonal changes may possibly be due to a persistent Wolff-Chaikoff block induced by diffusion of free iodide and subsequent shifting of the intrathyroidal MIT/DIT ratio, which is finally followed by an increase of T3-levels in serum.

[The effect of drugs on laboratory diagnosis. The effect of ascorbic acid on selected automatic laboratory methods].

The paper presents further investigations for a critical survey on the influences of drugs on laboratory methods. In controversion to the meanings you can find in the literature that ascorbic acid is most one of the important drugs to interfere with laboratory results we couldn't see in our systematical experimental investigation such results. Only in very extremly cases it seems to be right. Selected parameters of clinical chemistry (glucose, lactic dehydrogenase, aspartal-aminotransferase, alanine aminotransferase, alkaline phosphatase, protein, albumine, creatinine, butanol extractable iodine, ferrum) show under therapeutic conditions no influences of ascorbic acid, which can lead to diagnostic or therapeutic false interpretations. Above all the often mentioned example that glucose estimations in blood (reduction methods) can disturb if ascorbic acid is present, is abstracted in an uncritical manner how our experimental results may show.

[Quantitative gated nuclear cardiography for noninvasive evaluation of left ventricular function].

In 43 patients with angiographically proven coronary heart disease and in 24 patients without heart disease, gated blood pool scintigraphy was done under resting conditions with 15 mCi 99mTcHSA in LAO projection. Scintigraphic data were collected in list mode by means of a gamma camera connected to a computer system (Siemens 330). A cumulative scintigram sequence with a time resolution of 100 frames/s was constructed using several hundred heart cycles of equal duration. After background correction volume curves of the left ventricle were obtained and the following parameters were calculated: EF, dV/dt max,dV/dt min. In all patients with coronary heart disease, these parameters were reduced depending on the severity of the stenotic lesion. The changes of dV/dt min and to a greater extend of dV/dt max were more pronounced than those of the EF. These results are in keeping with invasively obtained data which show that a decrease of myocardial compliance is one of the earliest manifestations of CHD. Quantitative gated nuclear cardiography allows, therefore, noninvasive evaluation of left ventricular function even under resting conditions.
