Bellak and coworkers' "Ego-Function" rating was performed at a neuropsychiatric outpatients department by having an interview that was recorded once on tape analyzed by the same rater at intervals of 16 +/- 6 months using an estimation scale divided into seven main points. The result shows a high degree of uniformity in assessing the following functions: Reality testing, judgement, feeling for reality of oneself and the world, control of drives and affects, relationship to objects, ability to think, use of fantasy for solving problems, protection from stimuli, defence function, autonomic function, synthetic integrative function and feeling of competence. The most important result is that the rating can be used to good effect by neurologists and psychiatrists working alone.
Fifty-four patients with carcinoma of the oesophagus and cardia were selected to have either a two-cavity radical operation or blunt oesophagectomy without thoracotomy. Squamous-cell carcinomas were randomized for postoperative radiotherapy or none. Neither the two-cavity radical operation nor additional radiotherapy prolonged life expectancy. The results cast doubt on the effectiveness of local radical or curative surgery for carcinoma of the oesophagus and cardia. The only objective in the choice of treatment must be to decrease operative mortality and to make the course of the disease tolerable to the patient.
In an attempt to relate karyotype evolution to clinical and hematological data serial chromosomal analyses were performed in 31 patients with chronic myeloid leukemia (CML), both in chronic and acute phases. Our results in Philadelphia chromosome (Ph1)-positive CML are in line with karyotype profiles described in the literature. In addition, we report on chromosomal findings in 4 cases of Ph1-negative disease, one presenting with an iso17q chromosome in the positive CML. The same chromosomal abnormality was observed in a small population of Ph1-negative cells present in one of two patients with mixed Ph1-positive/Ph1-negative CML. The first case of a female patient with the loss of a sex chromosome in Ph1-positive cells is reported. Two patients with unusually long and mild chronic phases despite the presence of trisomy 8 in their karyotypes are described. Our findings suggest that the order of appearance of additional chromosomal changes of CML is of prognostic significance for the progression and the clinical picture of the disease.
Contrast injection for angio-CT shows kinetic changes in patients with portal hypertension due to cirrhosis of the liver when compared with normals. Contrast-duration diagrams show a higher aortic peak, low contrast and delayed contrast maximum in the liver and portal vein in patients with liver cirrhosis and portal hypertension. On these criteria it is possible to distinguish patients with cirrhosis (n = 18) from normals (n = 16).
Between 1972 and 1981, 38 patients suffering from lung disease by atypical mycobacteria were admitted to our institute. 36 out of them were males and 2 females with an average age of 55 years. In 10 patients silicosis was present. The patients received chemotherapy at special combinations using Rifampicin, Ethambutol and one or two additional drugs. 2 patients could be healed by lung lobe resection. In 12 patients x-ray regression could be observed. In the remaining patients, stabilization of the lung lesions and sputum conversion--at least temporarily--could be obtained. Because of multiple drug resistance, the therapy of mycobacteriosis is complicated, and permanent conversion of sputum cannot always be obtained, especially in diseases caused by M. intracellulare-avium. Bacteriological relapses are to be expected in 30 to 40 percent.
The authors describe the application of the linear discriminatory analysis as a pattern of prognosis for a group of 83 irradiated patients with Hodgkin disease. They investigated which combination of primary characteristics allows the most accurate discrimination between a group of patients with favorable prognosis and a group with unfavorable prognosis. Stage, histology, age, and clinical symptomatology proved to be valuable discriminatory factors. The accuracy of the discriminatory criterium is 75%. The results are compatible with the temporally comparable observations about prognostic factors. The method is suitable for gaining further quantitative knowledge.
In 271 gynaecological or obstetrical patients a "combined anaesthesia method" was applied, consisting of lumbar catheter epidural analgesia and controlled ventilation with nitrous oxide/oxygen. We aim to show that by mere interaction of the local anaesthetic, with nitrous oxide, and the drugs used for premedication and during induction of anaesthesia a stage of sleep can be achieved, which, on the patient's behalf is identical with his idea of "full anaesthesia", while at the same time the use of centrally acting agents can be kept at a minimum. In incidents of sympatho-adrenergic-stimulation due to insufficient cephalad spread of the blockade and not amenable to additional doses of local anaesthetic, i.v. fentanyl or low concentrations of volatile anaesthetics were added. In the postoperative phase the patients showed a high degree of vigilance. Epidural morphine offered best pain relief with full mobility maintained. In cases with postoperative hypertensive crises, morphine was combined with bupivacaine injections. Even marked obesity or senile degenerative changes in the vertebral column proved not to constitute contraindications for this method.
144 patients with bacteriologically positive results were analysed out of 258 relapses of pulmonary tuberculosis within one year (1979/80) to find the causes which are favouring relapses. It has been found that in 28% an inadequate chemotherapy, in 16% uncontrolled use of drugs, and in 13% of cases uncooperative attitude of patients were supposed causes of relapse. Causes of relapse could not be established in 23% of patients. Silicosis and chronic alcoholism were observed in 22% of patients with relapse.

A sample of 169 first- and third-grade children, selected because of their high exposure to television violence, was randomly divided into an experimental and a control group. Over the course of 2 years, the experimental subjects were exposed to two treatments designed to reduce the likelihood of their imitating the aggressive behaviors they observed on TV. The control group received comparable neutral treatments. By the end of the second year, the experimental subjects were rated as significantly less aggressive by their peers, and the relation between violence viewing and aggressiveness was diminished in the experimental group.
In this communication the experiences obtained in 221 patients with a newly developed immersion method for scrotal sonography are analysed. The specific criteria for testicular neoplasms are derived from 22 patients with 100% true-positive diagnosis. In this context, the diagnostic difficulties in defining and identifying the benignity of an affection are discussed. The typical sonographic alterations of scrotal structures in diseases like epididymitis, epididymo-orchitis, spermatocele, varicocele and trauma are also described. An approach to calculating the testicular volume and hydrocele volume is given. The increase in diagnostic reliability makes this method suitable for pre-invasive diagnosis. The simple and non-hazardous procedure can be recommended as a routine method.

The arterial system can be demonstrated after intravenous contrast injection by means of DVSA. The abdominal aorta, pelvic and lower limb arteries were examined in this way in 152 patients. Occlusions, stenoses and areas of dilatation could be recognised. In 8% of the patients the aortic bifurcation or common iliac arteries were not adequately visualised. In other respects, the results of DVSA were comparable with conventional arteriography. Because of limited image size, DVSA is particularly indicated where the vascular changes can be localised accurately by clinical means. The method is well suited for observing the results of surgery or percutaneous angioplasty. The indications for arteriography have been reduced as a result of DVSA.
The CT findings in 80 patients with confirmed portal hypertension due to intrahepatic block were analysed retrospectively. The findings included ascites, hypo-dense areas in the liver, nodular or irregular liver contour, the inability to define intrahepatic portal vessels with narrow window settings, the so-called "kissing" sign and evidence of hepato-fugal collaterals and thickening of the wall of the oesophagus. Electronic measurements of various organs (liver, spleen) and vessel diameters (portal vein, superior mesenteric vein) provide parameters which permit classification of patients into the cirrhotic group, as compared with a control group. The statistical significance of this classification was checked by the Kolmogoroff-Smirnoff test.
The results of a discriminant analysis for the objective diagnosis of cirrhosis of the liver are reported. Eight quantitative parameters seen on CT which, by themselves, are not diagnostic, were studied, and three relationships found to be of discriminatory significance; they are the diameter of the superior mesenteric vein, the ratio of the diameter of the caudate lobe and the right lobe of the liver and a quotient derived from the splenic and hepatic index. Cirrhosis of the liver with portal hypertension could be distinguished with a specificity of 100%, an accuracy of 96% and a sensitivity of 94%.
The demonstration of contrast-filled vessels by means of digital subtraction angiography has already proved a valuable clinical method (1 to 14). The present paper provides data on measured opacification as related to vessel calibre and contrast concentration. It is further shown in which degree narrowing of vessels can be ascertained dependent on vessel size and contrast concentration. Finally, the spatial resolution of the method has been investigated by using phantoms. It was shown that the resolution depends on the direction of the vessels within the projected area.
