[Sequestral osteitis with fracture of the mandibular angle occurring secondarily to the extraction of a wisdom tooth. Apropos of a case].

The authors report on a case of sequestrated osteitis complicated by a fracture of the angle of the mandible, following the avulsion of a wisdom tooth in a 20 year old man. The treatment consisted of an intermaxillary fixation of the mandibular arche lasting 40 days accompanied by wide-spectrum antibiotic therapy for 15 days, until such time as the ESR and blood count values returned to normal. In three months resorbtion of the sequestrated necrotic bone and disappearance of the osteitic cavity was noted, all of which was indicative of a satisfactory result. From the aetiological point of view it was possible to confirm by way of a routine X-ray taken 5 days after the extraction that the lesions had developed of their own accord, thus eliminating surgery and trauma as causes. The problem of what causes maxillary osteitis remains, and in particular, the role played by local anaesthetics containing adrenaline.

[Bilateral parotid enlargement from malignant lymphoma (author's transl)].

A rare case is reported of a 32-year-old man presenting with bilateral enlargement of the parotids only, later found to be due to a malignant lymphoma of the non-Hodgkin's type. Diagnosis was impossible at this stage because of the absence of other clinical signs, except for asthenia, and the negative results of complete investigation. Diagnosis became possible after the appearance of cutaneous lesions followed by a parotid nodule, which were shown by biopsy, over a period of two years, to be extensions of a non-Hodgkin's type 2 malignant lymphoma, according to the classification of Bryon et al. from Lyon. The initially prescribed corticoid treatment was replaced by multiple chemotherapy, and parotid enlargement, asthenia, and skin lesions were no longer present eight months later. The authors emphasize that no lesions occurred in the hematopoietic organs during the course of the disease, which clearly differentiates this case from the parotid lesions seen fairly frequently in blood diseases and Gougerot-Sjögren syndromes complicated by lymphoid leukemia.

[Treatment of large mandibular cysts (author's transl)].

Apart from certain particular types of lesions, the diagnosis of a histologically benign "large mandibular cyst" (paradental cyst, epidermal cyst, ameloblastoma...) should lead to an attempt at conservative treatment: this does not alter mandibular continuity and if possible, the inferior dental neurovascular pedicle, the soft tissues of the buccal lining, and the dental organs. It is finally the gross pathological findings and if necessary the results of an extemporaneous histological examination that will supply the information leading to the choice of the best type of treatment: marsupialization, enucleation, curettage, "pressure curettage", uninterrupting resection. This attitude requires prolonged postoperative radiological supervision and does not exclude the need for an interrupting resection in unfavourable cases at a later date.

[Sialorrhea. Value of isotopic examination (author's transl)].

Three types of sialorrhea can be distinguished: -- deglutition disorders, especially in patients with Parkinson's disease and those with anxiety, -- ptyalomania, -- primary hypersecretion at rest. Functional scintigraphic studies enable a physiological approach to be made, and this tends to group together, under the name of primary hypersecretion at rest, various clinical manifestations occurring in two main groups: -- elderly women complaining of a bitter taste in the mouth, -- plethoric men with large salivary glands. The scintigraphic recording apperances of secretion at rest, which occurs early and without obvious food stimuli, are also noted in other clinical types. It remains to be demonstrated whether they have a common physiopathological basis arising from disorders of the normal sensitivity of the buccal cavity.

[Hypertrophic sialosis: general review and report on 13 cases (author's transl)].

The authors describe 13 cases of hypertrophic sialosis and discuss possible mechanisms which could explain their appearance. In spite of different etiologies there is uniformity in the reactions produced. The authors therefore compared the clinical findings with those reported from animal experiments in which such lesions were artificially produced. Whether the processes of cellular hyperplasia and hypertrophy observed are related to an autonomic nervous system disorder, is open to question.

[Several cases of emergency repair of loss of skin substance in the child (author's transl)].

Traumatic loss of skin substance in the child is uncommon. There are three main causes: dog bites, scraping lesions and bullet wounds. In all cases, great attention is paid to initial and early treatment of the wounds (exploration, wound care) under general anaesthesia. Provided the operator is competent, dog bites may be completely repaired from the outset. Lesions due to scraping or bullet wounds should be treated more cautiously with emergency treatment not necessarily seeking to achieve closure at any price.

[Hereditary enamel dysplasia. Therapeutic approach apropos of 2 cases].

Two cases of hereditary dysplasia in the same generation of a family are presented. The genetic line is suggestive of a dominant transmission. The therapeutic attitude in each case: --Orthodontic, endodontic and prosthetic rehabilitation in the 1st case. At the patient's strong urging, a total fixed denture was made, which made it possible to correct moderate protrusion of the incisors and to obtain the aesthetic result desired by the patient. This work was carried out in the Stomatological Clinic of Lyon, by students working for their specialty certificate in stomatology. --In the case of the younger sister, the presence of a markde skeletal open-bite and the refusal of any surgical solution led to the total extractions requested by the patient. A mobile denture was made by her own dentist. Thus complete assessment and knowledge of the patient's precise wishes make it possible to adopt the most desirable therapeutic approach in each case.

[Radioisotopic examination and diagnosis of dryness of the mouth].

The clinical and sialographic opposition between true xerostomia (with continuous disorders) and paradoxical asialia (with inter-prandial disturbance, but sparing mealtime) led the authors to analyze these patient's scintigraphic scannings. A kinetic study of excretion was carried out with gamma-camera recording of intensity and glandular activity: the information thus obtained was set down in a numerical integration block and dealt with by computer. A graph of activity was plotted for each gland showing: - a phase of ascending concentration followed by a plateau corresponding to excretion at rest; - a phase of excretion provoked by a gustatory stimulus (physiological) with a very rapidly descending graph. It is possible to establish a ratio of glandular activity/blood activity at maximum concentration and maximum excretion. From the graphs thus obtained it is possible to perfectly differentiate true xerostomia (no excretion even with stimulation) from paradoxical asialia (no plateau of excretion at rest, but stimulus effective). A better knowledge of "dryness of the mouth" should enable better adapted therapy.
