In order to study, autoradiographically, DNA synthesis in tumors, kidneys containing renal carcinomas were for the first time perfused with normothermic oxygenated blood with an addition of radioactively labelled thymidine. Tumor cells preserve the ability of synthesizing DNA in spite of an unavoidable postoperative ischemia. In most cases the incorporation of 3H-TdR was inhibited significantly after preoperative irradiation with a total dose of 1,600 rad applicated 48 h before operation.
Preoperative lymphography is a basic condition for uniform tumor classification of renal carcinoma, according to the T, N and M categories. The comparison of 148 lymphographic findings with 170 intraoperative and histopathological lymph node results showed a satisfactory correlation. The total frequency of lymph node metastasis was of 21.6%. Carcinomas with positive lymph nodes showed a significant increase of far distant metastasis up to 50%. Lymph node metastases have to be considered in 10% of all carcinomas of the T1 and T2 categories and in 15.5% of the cases with unclear preoperative lymphographic diagnosis. These results point out the necessity of a radical lymphadenectomy in all T categories.
The moral justification for substitutive sterilization presupposes a critical verification of the sterilization wishes and an exact explanation to both partners. With the acquisition of the proper consent, the explanation of the absolute necessity of the postoperative spermiogram check-up cannot be neglected. Comparison of the spermiograms of unalterably vasectomized men with findings from additional rinsings with physiological saline solution and nitrofurantoin showed that the instillation of the vas deferens leads to a swift, mechanically dependent, emptying of the distal sperm depot. Through instillation of the spermicidal substance nitrofurantoin and the resulting spermatozoa immobilization, this effect can be considerably increased.
Four cases of vaginal metastases of renal carcinoma are reported. This is an incidence of 1.3% in 313 operated patients (from 1/1/70 to 12/31/76). A surgical treatment of primary renal carcinoma and vaginal metastasis seems to be reasonable in there cases. Since in accordance with the literature metastatic involvement of vagina and vulva seems not to be a very rare finding, the diagnostic in renal carcinoma should imply a gynecologic examination. Although in carcinoma of the left kidney metastatic spreading into the external female genitalia most probably occurs by a retrograde venous pathway (left ovarian vein), the way of dissemination in carcinoma of the right side is unclear.

A case of a residual kidney and polycystic renal degeneration is reported in which postrenal anuria developed from a cluster of cysts filling the entire renal pelvis. The anuria was repired by removing the intrapelvic cysts and creating a temporary renal fistula. A sudden onset of anuria and the corresponding symptoms (e.g., acute abdominal pain) and also the familiar progressive polycystic degeneration of the kidney should arouse suspicion of an obstruction caused by cysts.

In order to prevent intraoperative metastasis, venous invasion and infilitration of the regional lymph nodes by renal carcinoma, the thoraco-abdominal surgical approach is recommended. The vascular pedicle can in this way be explored and ligated before manipulation of the tumour. After total nephrectomy en bloc with the fatty capsule and adrenal gland, paracaval, paraaortal and intravasal lymphadenectomy is performed. Tumorous protrusions into the vena cava can mostly be removed at the same time. Inoperability rate could be reduced to 2.1 per cent as opposed to an earlier 11.3 per cent with the lumbar approach. TNM classification followed the recommendations of the UICC and was supplemented with a four-stage system. No improvement over the lumbar approach in respect of survival has been achieved in the first three postoperative years. However, local recurrence is expected to diminish as a result of improved radicality.
The operative treatment of two patients with male pseudohermaphroditism is reported. The patients reported were girls whose psychosexual attitude indicated a male sexual change. During several procedures the vagina was removed, the urethra was extended according to the method of Denis-Browne and a functioning penis was formed using a skin flap plasty supported by rib cartilage or an artificial prosthesis.
