Fixation of extradural catheters by means of subcutaneous tissue tunnelling.

In 100 patients treated for chronic pain, extradural opioids were administered by means of catheters which were tunnelled subcutaneously. Seventy-three per cent of patients needed only one catheter and 15% had a catheter in place for more than 100 days, the longest being for 434 days. In only 18 patients was the catheter displaced accidentally from the extradural space. There were no instances of extradural infection, although in three patients the tunnel became infected. We conclude that fixation of extradural catheters by subcutaneous tunnelling is a simple method which provides adequate security in clinical practice.

Comparison between midazolam and thiopentone-based balanced anaesthesia for day-case surgery.

In a randomized study of 100 women (ASA 1-2) undergoing termination of pregnancy as outpatients, the combination of midazolam-fentanyl-nitrous oxide was compared with thiopentone-fentanyl-nitrous oxide. The induction time (the time recorded until disappearance of the palpebral reflex) was significantly longer in the midazolam group (40 s) than in the thiopentone group (31 s). There were no significant differences in heart rate or arterial pressure between the two groups. Thirty minutes after the termination of surgery the degree of awareness, estimated by means of Glasgow Coma Scale, was lower in the midazolam group. After 60 and 180 min the scores were equal. During recovery more patients experienced side-effects in the thiopentone group than in the midazolam group, the difference being statistically significant 72 h after discharge when 66% and 34% of the patients complained of side-effects, respectively. Midazolam is as suitable as thiopentone for the induction of anaesthesia in day-case surgery.

[Treatment of ulcus simplex of the bladder and ulcerating radiogenic cystitis with superoxide dismutase].

Treatment of ulcus simplex vesicae and post irradiation ulcer of the bladder with superoxide dismutase (SOD) is reported. Its antiinflammatory effect depends on the elimination of cytotoxic superoxide radicals from tissue. SOD is injected into the bladderwall during general anesthesia using a transurethral access. We treated 8 women, 4 of them suffering from ulcus simplex vesicae and 4 of them suffering from post irradiation ulcer of the bladder. In most cases we saw prompt relief from subjective complaints and healing of the ulceration. When necessary the injection was repeated once or twice in a fortnight. We did not see any side effects.

[Course and prognosis of polycystic kidney degeneration].

Polycystic disease of the kidneys was found in 80 patients, who were members of 18 families. The disease could be demonstrated in 62 of 65 tested persons of these families. By ultrasound examination cyst formations could be detected already in the age of 6 years. The most common complications were urinary tract infections (46%), hypertension (30%) and nephrolithiasis (29%). Cysts of liver and pancreas were observed in 30%, cerebral hemorrhage in 6%. The frequency of hypertension and urinary tract infections as well as the occurrence of uremia increased in the fifth decennium.

Analysis of proliferative compartments in human tumors. I. Renal adenocarcinoma.

Vascular perfusion of 16 renal adenocarcinomas with radioactive DNA precursors provides a possibility of characterize proliferative compartments of this tumor type. Immediately after resection of the tumor-bearing kidney, the organ is perfused via renal artery with dextran-diluted, heparinized oxygenated blood at physiological temperature, pH, flow, and pressure in a recirculation system. DNA synthetizing cells are labeled by addition of 3H- or 14C-thymidine or both isotopes at different intervals. Beta camera scans and whole-tumor autoradiograms disclose a striking proliferative heterogeneity of the tumor. Cell proliferation depends on intratumoral localization, cellular differentiation, histological structure and vascular supply. Subpopulations of high proliferative activity are found at the invasive borderline near normal kidney, focally in subcapsular areas and in intrarenal metastases, but also immediately adjacent to necrotic areas in the tumor center. Quantitative evaluation of autoradiograms yields, at the cellular level, a significantly higher labeling index in granular cells (3.21%) than in clear cells (0.65%), with a large variability dependent on the histological structure. The highest number of DNA synthetizing cells is seen in papillary and mixed solid-tubular zones and at peripheral parts of solid areas, whereas in central parts of solid tumor cords and in highly differentiated tubular areas lower labeling indices are observed. The labeling index decreases exponentially as a function of the distance from the supporting blood vessel. In solid cords, no labeled cells are seen at a distance of more than 200 micron from the capillary. The ts determined by 3H/14C-thymidine double labeling is between 9.9 and 16.8 hr for granular cells and about 9.2 hr for clear cells. Potential population doubling time calculated for various subpopulations yields values between 4 and 50 days. It is concluded that cell loss is high, for granular cells in particular. Besides cell loss, a large nonproliferating compartment contributes to a delay of the tumor volume doubling time. Proliferative heterogeneity of advanced human tumors, as exemplified by the renal adenocarcinoma, bears important implications for therapy and prognosis.

[Comparative studies of radioisotope uroflowmetry (author's transl)].

Radioisotope uroflowmetry allows continuous recording of urinary stream with determination of maximum and mean urine flow rate, duration of voiding and residual urine without urethral catheterisation. By the aid of twenty simulated tests and nine examinated patients the precision of this method was investigated by comparing our results with the findings of simultaneously recorded mictiographic measurement. The flow curves and the maximum flow rate show a high correlation.
