Relief of pain following upper abdominal operations by thoracic epidural block with etidocaine.

Bupivacaine and etidocaine were compared in 0.375% and 0.5% solutions (without adrenaline) in a double-blind study in thoracic epidural analgesia following upper abdominal surgery. Special regard was taken to duration and adequacy of analgesia and changes in motor function. Duration of analgesia was roughly comparable for all four solutions. Bupivacaine 0.375% and etidocaine 0.5% seemed to be appropriate concentrations for adequate pain relief. Motor function, as assessed by changes in FVC, FEV1 and PEFR was not influenced to any greater extent. A progressive fall in FVC with successive injections, indicating increasing motor weakness, did not occur.

Early intracellular events in the replication of bacteriophage T4 deoxyribonucleic acid. VI. Newly synthesized proteins in the T4 protein-deoxyribonucleic acid complex.

Shortly after infection of Escherichia coli B with T4 phage, the phage deoxyribonucleic acid (DNA) can be isolated as a fast-sedimenting, proteinaceous complex. Formation of the complex is inhibited by the addition of chloramphenicol between 3 and 4 min after infection, suggesting that phage-coded proteins are necessary to form the complex and may contribute to its structure. To determine whether the phage DNA is associated with a random collection of proteins after infection or whether the complex contained a specific set of proteins, total protein from phage-infected cell lysates was compared to complex protein isolated from similar lysates by gel acrylamide electrophoresis. The proteins obtained from complexes exhibited a distinctly different pattern of separation, indicating that the complex contained a specific set of those proteins newly synthesized after infection. The proteins of the complex appear to be associated directly with the DNA rather than with some other component which could impart the characteristic of fast sedimentation to the complex. Fast-sedimenting complexes were isolated from a (3)H-leucine-labeled cell lysate. Part of this material was treated with pancreatic deoxyribonuclease. Deoxyribonuclease-treated and untreated complexes were resedimented in sucrose gradients. Virtually all the untreated complex remained fast-sedimenting, whereas most of the (3)H-leucine label of the deoxyribonuclease-treated material was located toward the top of the gradient. These data suggest a direct association of DNA and protein in the complex.

Development of a scale for the measurement of symptom change in an outpatient clinic.

A brief 21-item symptom rating scale, the Psychiatric Outpatient Rating Scale (PORS), was developed for use in outpatient clinics. On the basis of its initial use with 86 patients, it was shown to have high internal and interjudge reliability and evidence of concurrent and construct validity. Scores on the PORS correlated significantly with the Global Assessment Scale and with the number of sessions of psychotherapy. For a subsample of 45 patients rated on the PORS at the beginning and termination of psychotherapy, seven symptoms revealed highly significant improvement. The PORS appears to be a potentially useful measure of change in outpatient clinics.

Relationships between psychological mindedness and personality traits and ego functioning: validity studies.

Two studies further investigated the psychometric properties of the self-report Psychological Mindedness Scale (PMS), which has been shown to have good internal consistency and evidence of predictive and divergent validity. Its construct validity was investigated by assessing (1) whether total scores on the PMS of a group of medical students attending a personnel clinic in a psychiatric outpatient setting would be related in theoretically expected ways to their scores on a personality test; and (2) whether total scores on the PMS of a group of regular clinic patients would have the expected theoretical relationships to scores on a measure of ego functions. The findings of both studies provided support for the construct validity of the PMS.

Cortical localization of human sustained attention: detection with functional MR using a visual vigilance paradigm.

PURPOSE: Our goal was to determine whether functional MRI on a standard 1.5 T system can localize activation during a visual vigilance sustained attention task and whether this corresponds to results described in a PET investigation of a similar task. METHOD: Sixteen volunteers were studied on a 1.5 T system using a gradient echo technique. A single axial section was oriented within a stereotaxic coordinate space, 40 mm superior to the anterior-posterior commissure line. Images with eyes closed were followed by images during subject concentration on a small dim spot. Motion correction and pixel-by-pixel statistical analysis were performed. Talairach grids were applied for summary statistical analysis and comparison to PET data, with analysis using a series of planned contrasts within a repeated measures analysis of variance. RESULTS: Predominantly right-sided frontal and parietal activation was observed, with statistical significance across subjects in the right frontal lobe (F > or = 5.9, p < or = 0.041). Comparison with previously reported PET data yielded a very similar pattern of activation (F = 13.2; df = 1,8; p = 0.007). CONCLUSION: Activation of the right middle frontal gyrus and right parietal lobe during visual vigilance is detectable across functional imaging modalities.

A comparison of anterior compartment pressures in competitive runners and cyclists.

Anterior compartment pressure was measured in 10 competitive runners and in 10 competitive cyclists who were asymptomatic for compartment syndrome. Pressures were measured at rest, after exercise at 80% VO2max, after maximal exercise, and 15 minutes after both exercise bouts. No difference in compartment pressure was found after exercise at 80% VO2max in runners and cyclists. Total creatinine phosphokinase enzyme levels measured before and after exercise at 80% VO2max showed a 10-fold increase in runners as compared to cyclists. Anterior compartment pressure measured after maximal exercise was significantly greater in runners as compared to cyclists. Compartment pressure showed no increase from resting values during cycling at 80% VO2max or maximal exercise. These findings suggest that patients with chronic anterior compartment syndrome may be able to cycle without elevation of compartment pressure and concomitant pain as an alternative exercise to maintain a continued degree of fitness and training. Compartment pressures should be measured during cycling in patients with chronic compartment syndrome to determine its efficacy as a method for maintenance of cardiorespiratory fitness.
