Thrombo-embolic occlusions of the renal arteries are usually a result of co-existing cardiac disease, in most cases cardiac arrhythmias or ischaemic heart disease. Similar findings have been observed in mesenteric infarction and embolism into the lower extremities. The duration of ischaemia determines the prognosis. Surgical intervention has been the mainstay of previous therapy. However, the application of local thrombolysis, which is an extension of the diagnostic catheter studies, provides an alternative form of treatment. As in the case of peripheral artery disease, the choice of treatment can only be made in consultation with the surgeon. This report presents experience with the use of local thrombolysis of renal artery emboli.
Radiologic heart volume measurement (HV) and two dimensional echocardiographic estimation of left ventricular volume and ejection fraction (EF) were carried out in 108 healthy subjects and 68 patients with hypertony, dilatative cardiomyopathy and hypertrophic-obstructive cardiomyopathy (HOCM). Healthy subjects show a close relationship between echocardiographic determined volume and absolute heart volume (HV), in patients with hypertony, dilatative cardiomyopathy and HOCM there is also a significant correlation. However, no correlation exists between EF and HV and HV/kg in patients with hypertony and HOCM, whereas patients with dilatative cardiomyopathy show a high significant inverse relationship. It is concluded that these results are caused by great differences in muscle mass. The clinical value of radiologic heart volume as a parameter of left ventricular systolic function seems to be low and depends on cause of the heart enlargement.
Percutaneous transhepatic biliary drainage is a proven method for treating obstructive jaundice due to malignancy. However, the rate of complications for primary internal drainage is relatively high. Consequently, various types of biliary endoprostheses which can be introduced transhepatically or endoscopically have been developed. These prostheses also have serious disadvantages: there is a high risk of dislocation or obstruction. The present paper describes the technique of combined endoscopic-radiological introduction of large biliary prostheses and discusses the advantages of this method.
We herein report a case of a retroperitoneal lymph node metastasis from a malignant teratoma with yolk-sac admixtures originating from the right testicle. The metastasis was missed on CT scanning due to a vena cava crossing over with the aorta in the region of the metastatic disease, but typical tumor marker elevation was seen on follow-up.
A 39-year-old woman was evaluated for acute midepigastric pain. The patient was never severely ill; her only medication was an oral contraceptive (OC) which she took regularly over the last 12 years. Abdominal ultrasonography and computed tomography revealed bleeding into a liver tumor. At laparoscopy, 3 adenomas of the liver were detected. Bleeding was restricted to 1 tumor and did not require surgical intervention. OC medication was discontinued, and the patient was followed over more than 3 years. The patient was doing very well; ultrasonographically, the liver tumors became gradually smaller.
Disorders of the skeleton in the thoracic area after percutaneous irradiation of malignant lymphomas are relatively rare. The most common site for pathologic changes is the clavicula. Local overdosage and, in case of malignant alterations, an additional chemotherapy might be the reasons. Among our patients seven lesions occurred, three of them were malignant: two osteogenic sarcomas and one chondrosarcoma.
Six cases of posttraumatic hematomas in areas of the chest wall are reported, four of them were found subpleural and two extrapleural. Radiological signs and differential diagnostic possibilities are discussed.
Endoscopic sclerotherapy of esophageal varices is a widely used procedure. It reduces the frequency of rebleeding and improves the survival of cirrhotics with portal hypertension. The intravariceal or paravariceal injection of sclerosing agents causes structural changes of the esophageal wall recognisable radiologically. Stricture is a late complication which occurs in about 10 percent. In residual dysphagia balloon dilatation is recommended.
Vascular disorders of the liver except portal hypertension are rare. They can be visualised via ultrasound, as demonstrated by two cases of cavernous transformation of the portal vein, one case of Osler's disease of the liver, and one case of Budd-Chiari's syndrome. The cavernous transformation and the occlusion of the hepatic veins can be diagnosed by ultrasound, while Osler's disease cannot be differentiated with certainty from other disorders.
Experiments in 12 dogs have shown that intra-arterial injection of hyperosmolar contrast into the superior mesenteric artery normally produces a 52% increase in blood flow lasting more than two minutes. Following digitalis-induced mesenteric ischaemia, vaso-dilatation is equally marked, but lasts two and a half times as long. In view of the prolonged vaso-dilator effect of hyperosmolar contrast, arterial spasm could be missed during angiography.

A 46-year-old male patient with rapidly progressive Klebsiella pneumonia and abscess formation is presented. The infection resulted from a mediastinal-pulmonary fistula that developed during irradiation of a widespread carcinoma of the oesophagus.

In patients with otherwise therapy-resistant ascites operative methods for reinstillation of ascitic fluid have gained importance in recent years. Most often peritoneovenous shunt systems in the modification "LeVeen" or "Denver" are set in. Clinical evidence for insufficiency of the shunt system can force the radiologist to check the function of the drainage system. The investigation has to be performed under sterile conditions on the fluoroscope table, thus permitting the angiographic assessment of thrombotic complications as well as check of the position and function of the shunt valve and drainage tubes.
