The role of randomization in clinical trials.

Random assignment of treatments is an essential feature of experimental design in general and clinical trials in particular. It provides broad comparability of treatment groups and validates the use of statistical methods for the analysis of results. Various devices are available for improving the balance of prognostic factors across treatment groups. Several recent initiatives to diminish the role of randomization are seen as being potentially misleading. Randomization is entirely compatible with medical ethics in circumstances when the treatment of choice is not clearly identified.

Theory and practice in medical statistics.

The discipline of statistics generally, and that of medical statistics in particular, has evolved through two traditions, the theoretical and the practical. Their history, traced briefly here, shows recurrent points of contact and a process of gradual merging which is now almost complete. However, the current tendency to build complex mathematical and computing models may lead to over-confidence in their conclusions, when some crucial aspects of the data cannot easily be modelled satisfactorily. Illustrations are drawn from the use of formal stopping rules in the monitoring of clinical trials, and from the treatment of missing data and incomplete compliance with the trial protocol.

Changes in long-term mental health nursing.

This article tells the story of a group of people with severe and persistent mental disorder and the team who cared for them. A long-term research project into their needs and potential for changes in their care was conducted over 12 years (1, 2). Its findings and innovations present examples of fundamental changes in care management that can be replicated elsewhere.
