[Lead detoxication effect of a combined calcium phosphate and ascorbic acid therapy in pregnant women with increased lead burden (author's transl)].

In former papers it was stated that the fetus can be exposed a considerable burden by traces of heavy metals and that a combined therapy with calcium phosphate and ascorbic acid has a good detoxifying effect. In fourty lead-burdened mothers who were treated in this way the excretion of 5-aminolevulinic acid in urine decreased by 65%, the lead content of placenta by 90% and the lead content of mother's milk by 15% compared with a control group without treatment. The cadmium content of the placenta was reduced to 4% of the content in untreated mothers.

[The inguino-vaginal fascial loop repair in women with relapsing urinary incontinence (author's transl)].

This review comprises the results obtained over the past 20 years in the surgical treatment of incontinence by the method of Aldridge. The follow-up examination was carried out two years at least and 13 years at most after surgical intervention and consisted of case history evaluation, clinical assessment of urinary incontinence, as well as urodynamic investigation by cystometry and sphincterotonometry. Follow up was performed in 142 patients from the total number of 200 surgically treated cases. 84.5% of the patients showed good results, 7% were markedly improved, whilst 8.5% suffered recurrence of urinary incontinence. It must be taken into consideration that this kind of surgical intervention took place only after one or more operative failure for this indication. Hence, this method of surgical management can be recommended for the treatment of relapse of urinary incontinence.

Black urine.

A patient is described in whom spontaneous blackening of the urine, due to melanin formation, resulted from treatment with alpha-methyldopa. The colour change became apparent only after the correction of a severe postoperative hyponatraemic alkalosis, the urinary pH had become alkaline, as melanin formation in acid urine is very slow.

[Caesarean section - morbidity of mother and child (author's transl)].

In the years 1974, 1975, 1976, 1977 and 1978 641 cesarean sections were performed in the 1st Department of Gynecology and Obstetrics of the University of Vienna. All these were analysed in respect of diagnosis in each case as well as morbidity of mother and child. Especially the increase of the preventive indications in care of the child was shown. Vital and preventive indications were related to each other like 60,6% to 39,4%. The preventive indications were increasing. Obviously is this development by the childish indications, especially by children with extended legs. For this reason the vital indications concluded 69,1% in 1974, only 53,8% in 1978. In the same period the preventive indications increased from 30,9% to 46,2%. At the further indications we expect an addition of the diagnosis: st. p. sectio due to the general increasing of section-frequency, which causes the fact, that there will be more pregnant women who had been sectioned. Fever, parametritis and endometritis according to the morbidity of the mother contain 38%, by these and other complications a prolonged stationing more than two weeks is caused. The low Apgar-score of the newborns is due to the greater number of children below 2.500 g (at last: 19,18%). The perinatal mortality of the observed period was 3,06% by all cesarean sections; 12,8% belonged to the group with less than 2.500 g.

[Late results after surgery in chronic pancreatitis (author's transl)].

122 patients were operated upon because of chronic pancreatitis within 15 1/2 years. Mortality during and immediately after surgery was 9.8%, later mortality was 4.9%. 104 patients were still alive at follow up. In 40 patients most of the pancreatic parenchyma was left during surgery. Late results were good or satisfactory in 67.5%; carbohydrate tolerance was impaired in 35%, and exocrine insufficiency of the pancreas was present in 32.5% of these cases. In another group most of the pancreas was removed; 86% of these patients improved permanentely, 46% had impaired carbohydrate metabolism, 83% exocrine insufficiency of the pancreas. Good results can be achieved, if impaired carbohydrate metabolism, 83% exocrine insufficiency of the pancreas. Good results can be achieved, if indications are carefully choosen. Personality traits of the patients have to be taken into account when choosing the surgical approach. Advantages of a two session surgical approach combined with conservative treatment are discussed.

[Perinatal problems in breech presentation].

543 cases of breech presentation at the 1st Clinic of Obstetrics and Gynecology of the University of Vienna were investigated. The rate of Cesarean section showed during the last years a distinct increase (1969 to 1973: 16,6%, 1974 to 1977: 27,9%, 1977: 39,7%). A significant decrease of perinatal mortality in cases of breech presentation was not observed. On the other hand an increase in Cesarean sections diminishes the rate of perinatal mortality. In this publication the findings concerning the obstetrical results using abdominal or vaginal delivery are compared. No significant difference in perinatal mortality could be found. A better result could only be achieved in cases of mature infants. Our point of view is, that at present the indication of Cesarean section for cases of breech presentation should be very liberal. On the other hand vaginal delivery in some cases is still of importance.

[Serum oestriol: a parameter of the function of the feto-placental unit (author's transl)].

The determination of serum oestriol is known to be a useful parameter of the function of the feto-placental unit. In order to derive the normal range on application of the radioimmunoassay technique, the serum oestriol level was determined in 242 gravidae during the second half of pregnancy. The pregnancies were all uncomplicated and ended with the delivery of a healthy baby at term. The statistical mean and 4 confidence intervals were calculated for each gestational week individually. During the second half of pregnancy there was a continuous gradual increase in serum oestriol; between the 34th and 38th gestational week there was a further steep increase, followed by a decrease from the 39th gestational week onwards. The usefulness of this method is illustrated on the basis of case reports of 10 high-risk pregnancies in which oestriol determination is compared with the clinical findings and HPL determination.

A casuistic report on the Gruber or Meckel syndrome.

A case of Meckel or Gruber syndrome is reported, together with a survey of the relevant literature of recent years (1971-1977), in reference to a probably autosomal recessive inheritance of this malformation.

[Comparative evaluation of methods for predicting fetal lung maturity based on serial amniotic fluid tests (author's transl)].

12 pregnant females in the last trimester were subjected to serial amniocentesis. Although there was medical indication for amniocentesis the pregnancies were normal. All samples were evaluated with four different methods. We used two physical (Wilhelmy balance and foam test) and two chemical (L/S ratio densitometrically and planimetrically measured) methods. The Curves of individual pregnancies were compared with the reference values of 109 samples of amniotic liquid of uncomplicated pregnancies. Reference values for the various techniques reported in the literature are generally based on aggregate data collected from a number of pregnant females, which are then consolidated to obtain mean values for each gestational week. We found a continuous rise of all parameters, both in the reference value and in eight individual pregnancies. Besides we found a strikingly extraordinary run of the curves in four individual pregnancies. Here we recognized a rapid ascent and descent of surfactant concentration within short periods, which were not identical with the reference values. All four methods showed results with a statistical significant conformity, only the foam test showed a little lower significance.

[Intraoperative sphincterotonometrie during plastic repair of the urogenital diaphragm (author's transl)].

The method of using sphincterotonometry during the operation, which is described in this paper, adds further importance to this means of examination besides the pre-operative classifying of urinary incontinence and the postoperative control of the surgical success. By drawing up a urethral profile and by performing simultaneous impact palpation the functional effect of the operation can be checked during the same, so that a possibly necessary correction can follow at once.--Up-til now this test has been carried out during 16 operations on urinary incontinence (diaphragma plastic by Kelly with at least one years of postoperative control).--According to our hitherto existing examinations a positive closing pressure of more than 10 mmHg is necessary for achieving a functionally satisfying operation result.
