[Cancer of the right kidney and tumor of the right liver].

The true benign nature of a right side of liver tumor was revealed only after postoperative histopathology in a patient with cancer of right kidney. This raises the problem of kidney tumors with liver metastases. The preoperative diagnostic value of liver biopsy under ultrasound guidance is discussed, together with reasonable limits of hepatic excision combined with nephrectomy as a function of single or multiple metastases, their location, and their degree of surgical accessibility.

[Effect of theophylline and theobromine on cell respiration and calcium transport in isolated rat liver mitochondria].

The influence of theophylline and theobromine on cellular respiration and on membrane transport of calcium has been studied in isolated rat liver mitochondria, using oxygen and Ca2+ selective electrodes. A linear decrease in respiratory coefficients, in the total amount and rate of "extra" oxygen consumption induced by ADP is observed with drug concentration. Theobromine does not show any appreciable effect on these respiratory parameters, but this result is similar to that observed with theophylline for the same concentration range. Calcium uptake coupled to respiration is inhibited by both drugs depending on their concentrations. Theobromine is more effective than theophylline. Calcium saturation of the mitochondria takes place in all cases after 36 +/- 2 s but only a 20% of the maximum calcium uptake observed in the absence of the drugs is determined in the presence of 15 mM theophylline or only 1.8 mM theobromine. Comparative studies show direct correlation between the pharmacological activities as stimulants of caffeine, theophylline and theobromine and their behaviour as inhibitors of calcium uptake coupled to respiration by mitochondria.

Microvascular anastomosis for the treatment of seizures in patients with cerebrovascular ischemia: a report of five cases.

The authors report five patients who had either generalized or focal seizures in association with occlusion of the internal carotid artery. Two of them developed a motor deficit after having had several seizures. The other three did not develop a fixed neurological deficit, but their seizures had continued in spite of anticonvulsant treatment. After an extracranial to intracranial (EC-IC) bypass, the seizures either stopped or became less frequent. Based on our experience with these patients and our review of the literature, we think that seizures may be the initial or at least an early manifestation of cerebrovascular insufficiency in some patients. An EC-IC bypass may be of help when angiographic evidence of cerebrovascular occlusive disease is found.

Personal experience in the treatment of 178 cases of arteriovenous malformations of the brain.

We have analysed our personal series of 178 consecutive cases of arteriovenous malformation (AVMs) treated since 1954. A surgical approach was used in 146 cases (82%), with direct attack on the malformations in 132 cases (74%). There was total extirpation of the malformations in 124 of the operation cases (94%). In the remaining cases, ligation of the carotid artery was performed in three, and ligation of the afferent feeding artery in five. Embolization was used in five cases, and stereotaxis cryocoagulation in one. The decision-making process was based on the size of the malformations and the clinical presentation, which was subarachnoid haemorrhage (SAH) in 64% of the cases, and epilepsy in 15%. The results were considered good in 117 of the cases (89%), and the mortality rate was 11%. As in other series, a high incidence of AVMs was encountered in patients under 40, and men predominated (74%) over women. Different criteria of operability are discussed, including a "New Look" at the venous components of the malformations as an important assessment.

Pneumoscrotum after diagnostic peritonoscopy.

Peritonoscopy is a widely used procedure in diagnosis and surgery with few post-procedural complications. We report a case in which pneumoscrotum occurred as a procedural complication of peritonoscopy. Anatomic considerations are considered. The condition resolves without therapy, and only patient reassurance is indicated.

[Personal experience in monitoring examination of intracranial pressure].

In our Department of Neurosurgery, during the last two years, we have measured the intracranial pressure (IPC) with an extradural transducer, in cases of head trauma, subarachnoid haemorrhages, pseudotumor cerebri, dilated ventricles and microcephalies. We remark the importance of the continuous measuring of the ICP and its value in relation with the use of hyperosmolar solutions in the diagnosis, treatment and outcome of this patients.

Cloning and expression of a rat D2 dopamine receptor cDNA.

Dopamine receptors are classified into D1 and D2 subtypes on the basis of their pharmacological and biochemical characteristics. The D2 dopamine receptor has been implicated in the pathophysiology and treatment of movement disorders, schizophrenia and drug addiction. The D2 dopamine receptor interacts with guanine nucleotide-binding proteins to induce second messenger systems. Other members of the family of receptors that are coupled to G proteins share a significant similarity in primary amino-acid sequence and exhibit an archetypical topology predicted to consist of seven putative transmembrane domains. We have taken advantage of the expected nucleotide sequence similarities among members of this gene family to isolate genes coding for new receptors. Using the hamster beta 2-adrenergic receptor gene as a hybridization probe we have isolated related genes including a cDNA encoding the rat D2 dopamine receptor. This receptor has been characterized on the basis of three criteria: the deduced amino-acid sequence which reveals that it is a member of the family of G-protein-coupled receptors; the tissue distribution of the mRNA which parallels that of the D2 dopamine receptor; and the pharmacological profile of mouse fibroblast cells transfected with the cDNA.
