Determination of some butyrophenones in body fluids by gas chromatography with surface ionization detection.

Haloperidol, moperone, pipamperone and bromperidol were found measurable with high sensitivity by gas chromatography (GC) with surface ionization detection (SID). The calibration curves using moperone as internal standard were linear in the range of 0.4-4 pmol on column for haloperidol, pipamperone, and bromperidol. The detection limit of the drugs was about 0.1 pmol on column. For their actual determination with forensic samples, a detailed procedure for their extraction from human whole blood and urine was established with Sep-Pak C18 cartridges. The recovery of the 4 drugs, which had been added to whole blood and urine, was more than 90%. Haloperidol in whole blood and urine of a schizophrenic patient, who had been administered with this drug (3 mg/day p.o.), could be quantitated by the present GC-SID, and the levels were 7.18 and 43.2 pmol/ml, respectively.

Rapid extraction of methamphetamine and amphetamine in body fluids with Bond Elut SCX cartridges before capillary gas chromatography.

A simple and rapid method for extraction of methamphetamine and amphetamine from human whole blood and urine with Bond Elut SCX cation exchanger cartridges is presented. Detection of the stimulants was made by capillary gas chromatography (GC) with flame ionization detection after derivatization with trifluoroactic anhydride. The compound-containing samples, after mixing with 0.05 M potassium phosphate buffer (pH 7.8), were directly applied to the cartridges, washed with 20 ml of 50% methanol in water and eluted with methanol-HCl (240:1). The recoveries of both methamphetamine and amphetamine were close to 100%. The background noises for Bond Elut SCX cartridge extraction were much smaller than those for Sep-Pak C18 cartridge and diethyl ether extractions.

[Primary chylopericardium: a case report].

A case of primary chylopericardium in a 54 year-old-man was reported. Despite 3 times of pericardiocentesis, the pericardial fluid accumulated rapidly. After the fenestration of pericardium, a small cannula was inserted for the thoracic ductgram which showed a clearly visible duct upward to the angulus venosus and an abnormal branch near the bifurcation of the trachea spilling some contrast material into the pericardial cavity. Ligation and division of the thoracic duct including an abnormal branch was performed from its entry to the thorax for a length of 13 cm upward. The postoperative course was uneventful and chest X-ray film taken 6 months after surgery showed no accumulation of chyle in the pericardium and pleural cavity.

[Simultaneous operation of Cabrol's operation and sternal turnover with rectus abdominal flap].

We performed simultaneous operation of sternal turnover and Cabrol operation for a 28-year-old man, who had 3rd grade funnel chest and annuloaortic ectasia with 3rd grade aortic regurgitation. We think when both annuloaortic ectasia and funnel chest are indicated for operation, simultaneous operation is to be considered and the combination of Cabrol operation and sternal turnover with rectus abdominal flap is the best procedure.

Trace levels of pyrroloquinoline quinone in human and rat samples detected by gas chromatography/mass spectrometry.

A detailed procedure for the assay of free pyrroloquinoline quinone (PQQ) in human and rat samples by gas chromatography/mass spectrometry (GC/MS) has been established with stable-isotopic PQQ as internal standard. PQQ was extracted from the samples, after addition of the internal standard, with butanol under acid conditions and with Sep-Pak C18 cartridges. After derivatization of PQQ with phenyltrimethylammonium hydroxide, molecular peaks at m/z 448 and 462 were used for detection of PQQ and [U-13C]PQQ by selected ion monitoring, respectively. Trace amounts of free PQQ were detected in eight organs, plasma and urine of the human, and in three organs of the rat. The PQQ level was highest in the human spleen (5.9 +/- 3.4 ng/g tissue, followed by the pancreas and lung, and it was below detection limits for human brain and heart. Trace levels of PQQ were also found in rat small intestine, liver and testis. Our data are far below those measured by the redox cycling method of Gallop's group for human plasma, adrenal and urine.

Determination of diphenylmethane antihistaminic drugs and their analogues in body fluids by gas chromatography with surface ionization detection.

Eleven diphenylmethane antihistaminic drugs and their analogues were tested for their detection by capillary gas chromatography (GC) with surface ionization detection (SID). The GC-SID response was highest for doxylamine, diphenhydramine and orphenadrine and lowest for terodiline, clemastine and pipethanate. The detection limits for drugs with the highest response were 2-5 pg (ca. 6-20 fmol) on-column (100-250 pg/ml of body fluid). The detection limits with GC-SID were 10-100 times higher than those with GC with nitrogen-phosphorus detection. A detailed procedure for the isolation of the antihistaminics from human whole blood and urine by the use of Sep-Pak C18 cartridges, prior to GC-SID, is also presented. The recoveries of the drugs (50 or 500 pmol), which had been added to 1 ml of body fluids, were > 60%. The baselines remained steady as the column temperature was increased and the background was clean, especially for whole blood extracts.

Sensitive determination of phenothiazines in body fluids by gas chromatography with surface ionization detection.

Fourteen phenothiazine derivatives were tested for their detection by gas chromatography (GC) with surface ionization detection (SID). The sensitivity of GC-SID was highest with trimeprazine and levomepromazine, which contain aliphatic tertiary amino side chains, and lowest with thiethylperazine and thioproperazine, which contain sulphur residues. Chlorpromazine, trimeprazine and promazine showed excellent linearity between the SID response and the drug amount in the range 0.25-3.0 pmol on-column. Their detection limits were as low as ca. 5-10 pg (15-30 fmol) on-column (250-500 pg per ml of body fluid). A detailed procedure for isolation of phenothiazines from human whole blood and urine using Sep-Pak C18 cartridges, before the GC with SID, is also presented. The recoveries of the drugs (100 pmol), which were added to 1 ml of whole blood or urine, were more than 79%. The baselines remained steady as the column temperature was increased.

Stimulation of muscarinic cholinoceptive neurons in the hippocampus evokes a pressor response with bradycardia.

The injection of neostigmine into the hippocampus of anesthetized rats increased the mean arterial blood pressure (17% of baseline after 60 min injection) and decreased the heart rate (24% of baseline after 60 min injection). These changes were blocked by the co-administration of methylatropine into the hippocampus. Intrahippocampal injection of neostigmine stimulated the secretion of epinephrine and norepinephrine. Adrenodemedullation did not suppress the increase in blood pressure and the decrease in heart rate. It is concluded that the stimulation of muscarinic cholinoceptive neurons in the hippocampus evokes a hypertensive response via an increase in sympathetic drive to the heart and peripheral vasculature, with bradycardia possibly mediated via the parasympathetic system.

Rapid isolation with Sep-Pak C18 cartridges and capillary gas chromatography of triazine herbicides in human body fluids.

A simple and rapid method, for the isolation of eight triazine herbicides from human serum and urine, using Sep-Pak C18 cartridges is presented. After mixing with distilled water, serum and urine samples containing the herbicides, were loaded on Sep-Pak C18 cartridges and eluted with either chloroform only or chloroform/methanol (9:1). The herbicides were detected by capillary gas chromatography with both flame ionization detection (FID) and nitrogen-phosphorus detection (NPD). Separation of eight triazine herbicides from each other and from impurities was generally satisfactory with the use of a non-polar DB-1 capillary column. Recovery of most compounds was excellent for both chloroform and chloroform/methanol (9:1) as elution solvents. Backgrounds were cleaner and evaporation time was shorter for the chloroform only than for the chloroform/methanol (9:1). The NPD gave sensitivity more than 10-20 times higher than that of FID.

Primary hepatocellular carcinoma with severe hypoglycemia: involvement of insulin-like growth factors.

We report a case of severe hypoglycemia and hepatic masses suspected to be an insulin-like growth factor-II (IGF-II)-producing hepatocellular carcinoma. A 62-year-old man presented with mental disorder in the night and early morning associated with extremely low blood sugar levels (less than 21 mg/dl). Computerized axial tomography and ultrasonography revealed a massive tumor in the right lobe of the liver with multiple secondary nodules, and a tumor thrombus in the portal vein. At autopsy 107 days after admission, the liver weighed 3070 g, histologically showing an Edmondson type II tumor with liver cirrhosis. IGF-II in plasma (899 ng/ml) and tumor tissue (2.4 micrograms/g) was higher than that in normal plasma (374-804 ng/ml) and non-tumor liver tissue (0.2 micrograms/ml), while IGF-I (14 ng/ml) was significantly reduced. IGF-II, probably produced by the liver tumor, appeared to be involved in the mechanism of hypoglycemia.

Long-term survival in aged patients with corrected transposition of the great arteries.

Corrected transposition of the great arteries is a rare condition, and few patients with this abnormality survive past 50 years of age because of associated congenital defects or the subsequent development of atrioventricular valvular insufficiency or heart block or both. We describe four men with uncomplicated C-TGA. Our patients are of interest for the following reasons: (a) their condition is very rare; (b) the diagnosis of C-TGA traditionally has been verified through invasive cardiac catheterization procedures; however, in our latest two patients, recently developed noninvasive diagnostic techniques played the decisive role in the diagnosis of C-TGA; (c) in these modalities, they presented as a "natural experimental model" that the right ventricle submitted to a high systemic pressure load is capable of increasing muscle mass over long-term adaptation. Our four patients illustrate that patients with C-TGA, even with the associated cardiac anomalies, may live a normal life span with proper management.

[Trace analysis for drugs and poisons in human tissues].

Despite continuous developments of analytical techniques in terms of sensitivity and accuracy, uncountable increase in number of chemical substances are brought into the field of forensic sciences to be analyzed. Based on the idea that progress in analytical technique to cope with the change of situation is always required, a research team including 10 members was organized to set up the most advanced methods at present to analyze the important drugs and poisons in biological materials for forensic purposes. Stimulant drugs such as methamphetamine and amphetamine were studied on the improvement of extraction procedure prior to mass spectrometric analysis. A conventional solvent extraction method was replaced by an extraction technique using Extrelut column. The technical procedure was simplified and the accuracy of measurement was improved. The changes in CO-Hb concentration in the whole blood in storage was examined with regard to the lapse of time and temperature, where a differential spectrophotometry was used. Useful information could be obtained from practical aspects. Analytical conditions of gas chromatography were revised on volatiles including alcohols, especially as to column conditions. The use of capillary column was recommended for sensitivity and peak separation. The optimum conditions for detecting barbiturates in the blood were examined. The combination of Sep-pak C18 cartridge with a capillary column for gas chromatography using nitrogen phosphorus detection was found preferable. Mass spectrometry of various kinds of local anesthetics was studied. Quantitative analysis of the drugs was examined on gas chromatography with a surface ionization detector. Three types of insecticides including organophosphorus, chlorinated and fluorine compounds were analyzed by gas chromatography/mass spectrometry in order to establish a sensitive and selective method. The detection limits, calibration and reproducibility were examined. Herbicide, paraquat, was examined on the sensitivity, recovery, required time and costs in connection with methods of pretreatment and analytical procedure. A secondary spectrophotometry was found useful practically. Antimony in biological tissues was analyzed, using a flameless atomic absorption spectrometer with carbon tube atomizer. The time for analysis was reduced, and sensitivity was improved. Immunoassay method was examined from general aspects on drugs and hormones. Using antibodies specific to haptens, the dynamics of such antigens as drugs and hormones in the body tissues were observed. Interfering substances at the time of toxicological analysis were checked in order to obtain reliable information. Exact identification was found possible by using gas chromatography/mass spectrometry. As described above, the research was made from the practical aspects of forensic toxicology, and meaningful results could be obtained from each study.

[Benefit of the autoperfusion balloon catheter for emergency coronary artery grafting after failed percutaneous transluminal coronary angioplasty].

A 61-year-old man was operated on an emergent basis because of abrupt closure of left anterior descending coronary artery (LAD) following failed coronary angioplasty. At once abrupt closure occurred, Stack autoperfusion balloon catheter was placed across the coronary lesion to reestablish blood flow to the ischemic myocardial area with intraaortic balloon counterpulsation. Despite a longtime of 12 hours to cardiopulmonary bypass, the patient had been managed with excellent resolution of symptoms and ST elevation. Under a stable hemodynamic state, saphenous vein grafting to LAD was performed with minimal myocardial infarction. Stack catheter made it possible to perform the subsequent coronary bypass operation as a controlled, optimal revascularization procedure because of its passive autoperfusion effect.

[Long-term patency of aorto-coronary saphenous vein grafts].

One hundred eighty-two cases with 814 aorto-coronary saphenous vein grafts were studied according to coronary risk factors (smoking, hypertension, hyperlipidemia, diabetes mellitus, obesity and family history). The patency rates of all cases were as follows, early-term (within one year after operation): 92.3%, mid-term (within 5 years after operation) 80.7%, long-term (more than 5 years after operation): 66.0%. Coronary risk factors have great influence upon the mid- and long-term patency, especially upon the latter. The long-term patency rate of the grafts complicated with hyperlipidemia was 57.4% and that without hyperlipidemia was 81.8% (p less than 0.01). Hyperlipidemia, complicating 55.5% of all cases, was one of the most influential factors on the patency and also the most difficult one to be controlled. In the United States and Europe, many cases were complicated with hyperlipidemia, and it was considered that the poor patency of the saphenous vein grafts in those countries was due to this fact. Pathological studies revealed that hyperplasia of intima and media, characteristics of venosclerosis, appeared frequently in the saphenous vein grafts having more than three risk factors, and that the factors had effect not only upon arteries but also upon veins. So we conclude that saphenous vein grafts are the materials of good long-term patency, and that the control of the risk factors, particularly hyperlipidemia, is the key to improve the patency.

[Surgical techniques of ventricular assist device].

Since October 1982 to April 1990, we had 16 ventricular assist device (VAD) cases in our institute. Using this VAD system that was produced by Nippon Zeon company, surgical techniques for cannulation and de-cannulation could be made very easily, and connection between cannulae and pump also could be performed quickly. In this article, the actual surgical techniques and the know-how were illustrated in detail. If this techniques can be more accustomed, the clinical results of VAD cases will be improved.

Determination of local anaesthetics in body fluids by gas chromatography with surface ionization detection.

Ten local anaesthetics were tested for their detection by gas chromatography (GC)-surface ionization detection (SID). Lidocaine, mepivacaine and bupivacaine were detected with the highest sensitivity; their detection limit was 5-10 pg in an injected volume. The sensitivity of other drugs, such as procaine, dibucaine tetracaine and oxybuprocaine, was an order of magnitude lower than that of the above three local anaesthetics. A detailed procedure for isolation of local anaesthetics from human whole blood and cerebrospinal fluid (CSF) by the use of Sep-Pak C18 cartridges, before the GC-SID, is also presented. The recovery of lidocaine, mepivacaine and bupivacaine, which had been added to 1 ml of whole blood or CSF, was close to 100%.
