Intracavernosal injection of prostaglandin E1 as an alternative for papaverine in penile angiography.

Arteriography of the penile vasculature was performed after intracavernous injection of prostaglandin E1 in five patients. Penile tumescence was obtained in three patients with a dose of 10 micrograms and in the other two patients with a dose of 20 micrograms. Mean duration of penile tumescence was 1 h 36 min. Visualization of the penile vasculature was adequate in all patients. Local pain was a minor side effect in one patient. These preliminary results suggest that prostaglandin E1 might represent an alternative for papaverine in the angiographic study of male impotence.

Fertility in twins.

From the 474 twins born in 1950 in 100 Flemish cities and towns in Belgium, 85% reached the age of 20 yr, against 37% in Austria a hundred years ago. More female than male twins are married (P = 0.03), in particular nonidentical female twins (P = 0.02). Fertility seems to be lower in male, especially male-male twins (P less than 0.03), and most strikingly so in identical male twins (P = 0.05). There seems to be no influence of premature birth or low birthweight.

Solitary or predominantly right-sided varicocele: a possible sign of situs inversus.

Among more than 850 patients undergoing outpatient sclerotherapy of the internal spermatic vein, only 1 unilateral and 1 predominantly right-sided varicocele were encountered. In both patients with this finding a total situs inversus was present, with inversion of normal anatomy, enlarged "right" internal spermatic vein draining into the renal vein, and the "left" internal spermatic vein entering the inferior vena cava directly. Total situs inversus should be considered whenever right-sided varicocele is solitary or predominant.

Coital death.

Generally speaking one can state that there exists insufficient information to be alarmed about coital death; indeed, there are almost no studies known about coital death of heart patients. All the same an epidemiologic research is absolutely necessary for profylaxis of heart attacks caused by sexual activity, even more whilst the present population shows an increased rate of arteriosclerosis and disposes over more spare time. It is remarkable that apart from medico-legal practicians, cardiologists and revalidation doctors, other disciplines as psychiatry and sexuology, still haven't paid any attention to the problem of coital death. It certainly is high time to draw this phenomenon out of its taboo-sphere.

Pregnancy rate after homologous insemination.

Homologous insemination (AIH) was performed in 151 patients. The pregnancy rate was 27% in 143 patients treated with the cervical cap method. In cases of large ejaculate volume the split-ejaculate technique was applied to several types of sperm quality, resulting in a pregnancy rate of 43%. Of 36 women with diminished fertility, five (14%) became pregnant. The motility of the spermatozoa was correlated with the success rate, with 30% good progressive motility thought necessary to consider insemination.

[Modification of prolactin and stress by masturbation].

Fertility patients are in a stressful situation at the time of clinical examination. That condition of stress influences hormonal levels, especially prolactin levels, in blood plasma and in seminal plasma. After masturbation previously stress-induced hyperprolactinemia levels are reduced. This is probably due to the decrease of stressful tension during the period of clinical consultation and not likely due to the relaxation by masturbation. In conclusion, the stress condition of fertility patients must be taken into account while evaluating hormonal data.

Adrenal cortical hormones in the spermatic vein of 95 patients with left varicocele.

Adrenocortical hormones were measured in the peripheral and spermatic venous blood samples, taken simultaneously at the time of a high vasoligation for left varicocele in 95 patients. Our results do not confirm the hypothesis of MacLeod that the deleterious effects of a varicocele on male fertility are caused by an higher local concentration of adrenocortical hormones, due to venous reflux through incompetent valves. If this is the case it occurs only in a minority of patients.

Sexual impotence in man.

An analysis as to etiology and complaints of 486 cases of male sexual impotence was performed and is presented. Psychological factors are dominant in 68%. Physico-organic or iatrogenic causes accounted for deviations in 26% of the patients examined. Educational factors took an important part mainly in ejaculation disturbances. Primary anejaculation as a consequence of educational flaws occurred in 15% of the patients. Erection disturbances were often the consequence of physico-organic affections. The loss of libido was a real complaint in 19% of the patients.

Changes in areolar size in boys during the growth phase and pubertal development.

The areolar size was measured in a group of 755 school boys, aged 7 to 20 yrs. At the onset of puberty a sudden increase in areolar size has been noted, with a gradual increase in areolar size as pubertal development proceeds. The increase in areolar size must be seen as a pubertal phenomenon, resulting from a peripheral influence of sexual hormones, but a significant positive relationship to body size during this period of life must also be taken into account.

Separation of X- and Y-bearing human spermatozoa with the sephadex gel-filtration method.

By means of the Sephadex gel filtration method we were able to separate a fraction rich in X-bearing human spermatozoa, in contrast with the method of Ericsson et al. (1973), which allows to obtain a fraction rich in Y-bearing sperm. The method, which is simple and not time-consuming, may allow control of sex in cases of expressed sex desire in off-spring.
