[Cystinuria therapy by ascorbic acid (author's transl)].

At the beginning of the four chapters on phenomena, analysis, pathophysiology and therapy of cystinuria the essentials of the published literature are summarized. The frequency of cystinuria is in the order of 1:10,000. Besides the cystine lithiasis occurring in nine tenths of all cystinuria patients neurological diseases may also be observed. All commonly applied methods to analyze cystine detect the sum of cystine and cysteine. Cystinuria is characterized by a higher cystine excretion, up to the 100-fold of the normal. Also the concentrations of lysine, arginine and ornithine in the urine of cystinuria patients are elevated, caused by intestinal and renal transport defects. Inevitable damage of renal parenchyma by multiple operations can drastically be reduced by the therapy with D-penicillamine or alpha-mercaptopropionylglycine. The disadvantages of that formation of soluble asymmetric disulfides are the side effects, such as nausea, gastric difficulties and dermatosis, occurring in up to 50% of the patients. Using the especially developed method with HPLC separation and electrochemical detector with a mercury electrode, cystine and cysteine are analyzed simultaneously. In the urine of healthy persons the molar concentration of cysteine is in the same order as cystine. But in cystinuria the cysteine concentration in urine is about a thousand times less than that of cystine. These results are evidence that a shifted redox-equilibrium of cystine-cysteine is also typical of cystinuria. The molar cysteine percentage of cysteine in healthy persons is increased from 30 to 50% by oral ascorbic acid administration. Therefore a vitamin C therapy for cystinuria is developed. 31 cystinuria patients who receive 5 g of vitamin C a day show a decrease in the cystine concentration of about 40%. Up to now, no side effects have been observed. The most obvious sign of the positive effect of the proposed vitamin C therapy for cystinuria is the missing cystine sediment in fresh urine.

[Problems of hyperparathyroidism in urology (author's transl)].

The review of 340 own cases of hyperparathyroidism surgery shows, that only the concentrations of calcium and parathormone in serum are of interest for the diagnosis. Further biochemical analyses and function tests are not specific enough to serve as indicators for hyperparathyroidism and serve only as secondary reinforcements of the diagnosis. There are many methods for the preoperative localisation but only the computer tomography with simultaneously given contrast medium or a refined sonography seem to be reproducible. 94% of the cases are cured by surgery. The further development of urolithiasis is certainly reduced after the successful operation, however, in about one third of the cases with normocalcemia, a recurrent and marked increase in size of the stones present was observed postoperatively. The possibility of such a continuous growth gives reason to observe closely the development of urolithiasis in order to be able to react promptly by means of lithotomy.

Staging and surgery in testicular cancer.

Both staging and surgery in testicular cancer are thoroughly analysed. Extensive experiences from various authors are reported and compared. The essentials for staging are suggested and a new detailed stage grouping is proposed. The role of surgery in diagnosis, staging and therapy is examined. It is stressed that in non-seminomas, retroperitoneal lymph node dissection is not only a staging procedure, but it may be also a curative operation in several cases of patients with lymph node metastases and a prospective randomized clinical trial is advocated in order to clarify the usefulness of adjunctive chemotherapy in these cases. Surgery is also useful as adjuvant to chemotherapy in several patients with advanced non-seminomas and it is underlined that it should be undertaken early, after only 2 or 3 cycles of chemotherapy, in order to avoid the strong fibrosis that often occurs after long-term chemotherapy and which makes surgical dissection very difficult or even impossible. Lastly, even if extensive, surgery seems to be a safe procedure in these young patients.

[The Laferma reflux family, a case of multiple familial reflux].

We substantiate that five of total seven examined members of a family including the mother showed a reflux disease. Two of them had a monolateral, three a bilateral, two a vesicoureteral and three a vesicorenal reflux. Three of the examined members suffered from enuresis nocturna. All of the reflux-positive members of the family endoscopically showed gaping ostia of the ureter at the reflux-positive side. We found no criteria for an intravesical obstruction. All reflux-positive members showed a low-pressure reflux. Interestingly, only the female members exhibited a reflux. We don't know whether this case of an increased incidence of reflux disease within one family will represent a known hereditary pattern.

[Urinary calculi analysis using X-ray diffraction: selection, use and advantages].

Analysis of urinary calculi by X-ray diffraction: The evaluation of X-ray diffraction for the analysis of urinary calculi led to the substitution of the analysis by qualitative-chemical reactions. This instrumental method is performed in the routine laboratory in Zurich since two years. The interlaboratory quality assurance programme of 100 laboratories showed the advantage of the new method, producing correct results. On the contrary the old fashioned method by qualitative-chemical reactions yields questionable results.

[Clinical significance of urinary calculi analysis].

Base of real metaphylaxis in the renal stone disease are the analysis of stone and the research of the metabolism in blood plasma and urine. The greatest part of the happened stones is classifying in the four groups: oxalate, phosphate, uric acid and cystine. The metaphylaxis by whewellite and weddellite is the same for both species, but there is a distinction in growth and recurrence. In the phosphate stones, the section with the most different composition, a therapy postoperatively is only possible with the distinction in "acid" and "alkaline" stones. The uric acid and cystine stones need a tight supervision and metaphylaxis for all the life. On the basis of 1200 analyses with X-ray diffraction the difference of the renal stones is discussed and the deduction for a real metaphylaxis is shown.

Extratesticular gonadal stomal tumor in the pelvis. A case report with immunoperoxidase findings.

A 40-year-old man with two normally descended testicles and no signs of hormonal anomalies developed a fist-sized extraperitoneal tumor to the left of the urinary bladder. Histologic diagnosis was that of a probably malignant gonadal stromal tumor because of considerable mitotic activity and widespread vascular invasion. Retroperitoneal lymphadenectomy showed no metastatic disease, and the patient is well two years after surgery. By use of an immunoperoxidase method we could demonstrate estradiol and testosterone in the tumor cells. The findings in this case are unique in regard to the extratesticular location of the tumor and support the current view that gonadal stromal tumors arise from a primitive cell with a multidirectional potential for differentiation.

[Medical support in surgery for pheochromocytoma].

The occurrence of hypertensive crises, arrhythmias and hypotensive crises during surgery for pheochromocytoma can be prevented both preoperatively and intraoperatively by adequate alpha-adrenergic blockade and volume replacement. Only in a few cases is additional beta-adrenergic blockade necessary. Preoperatively the author uses phenoxybenzamine (Dibenicyline) as alpha-blockade in a dose of 20-30 mg daily, and propanolol (Inderal) as beta-blockade in a dose of 4 X 20 mg daily, both given orally. Intraoperatively he gives preference to alpha-adrenergic receptor blockers with a short halflife, i.e. phentolamine (Regitin) and lidocaine or pranolol as the beta-adrenergic blocker.

[Experiences with instrumental methods for urinary calculi analysis].

To reduce the urinary calculi incidence by calculi formers, it is important to know the composition of these stones. Unfortunately the chemical analysis does not give very reliable results. Looking for a better method to analyse urinary calculi, three instrumental methods were tested: infared spectroscopy, thermal analysis and X-ray diffraction. The experimental results and economical considerations show that the X-ray diffraction analysis of urinary calculi would meet the goal of improved care of patients with stones.
