131I-hippuran renography for control of patients with ureterolithiasis.

The value of the 131I-Hippuran renography for control of patients with ureteroliathiasis was investigated in a consecutive series of 58 patients with radiologically verified unilateral ureteroliathiasis. Intravenous pyelography (IVP) and renography (RNG) were performed on admittance and 4-5 weeks later, the interval between the IVP and the RNG being 0-4 days. From 80 comparisons of the results of IVP and RNG (prevalence of abnormal IVP 73%), the predictive value of an abnormal RNG in the control situation was found to be 88% with a 95% confidence interval of 75-93%. The predictive value of a normal RNG was 88% (62-98%). If renography was combined with an X-ray of the abdomen at the control examination, the predictive value of an abnormal RNG was unchanged and the predictive value of a normal RNG increased to 100% (77-100%). On the basis of these high predictive values, a new examination protocol for control of patients with unilateral ureterolithiasis is put forward.

Skeletal status in patients with chronic renal disease.

The bone mineral content (BMC) was measured in 17 patients with chronic renal failure. The patients as a group had a mean BMC of 89% compared with 127 normal subjects. The investigation failed to disclose any significant correlation between BMC and the stage of renal diseases.

Residual kidney function after unilateral nephrectomy. Pre- and postoperative estimation by renography and clearance measurements.

The residual kidney function was predicted from preoperative renography and determination of the glomerular filtration rate (GFR) in 57 patients undergoing unilateral nephrectomy for cancer, postrenal obstruction or renovascular hypertension. Postoperative GFR measurements were carried out 6-36 months after the operation. In eleven patients where the kidney removed had no function, no significant difference was found between pre- and postoperative GFR values. In 46 patients where the kidney removed had some function, the preoperative estimate was a little too high in only two patients. In this group, the postoperative GFR on an average amounted to 42% above the preoperatively predicted value. We conclude that the combination of 51Cr-EDTA clearance and renography is a reliable, non-invasive method for determination of the minimum residual kidney function before unilateral nephrectomy is carried out.

Predictive value of liver scintiphotography for demonstration of metastases in malignant gastrointestinal disorders.

In a prospective investigation, the predictive value of liver scintiphotography was evaluated in 107 consecutive patients operated on suspicion of intra-abdominal cancer. Scintiphotography was performed by means of a Pho/Gamma HP scintillation camera after intravenous administration of 99mTc sulfur colloid. The final diagnoses were obtained within 10 days after scintiphotography by laparotomy or autopsy. The predictive value of an abnormal scintiphoto was found to be 57%, and the predictive value of a normal scintiphoto was 91%. It is concluded that scintophotography is helpful in the preoperative evaluation of patients with suspected intra-abdominal cancer, especially in order to rule out the presence of liver metastases.

Actions of vitamins D2 and D3 and 25-OHD3 in anticonvulsant osteomalacia.

In 54 epileptic outpatients treated for at least one year with anticonvulsants the bone mineral content (B.M.C.), an estimate of total body calcium, and serum calcium were measured before and during treatment with three doses of cholecalciferol (vitamin D3; 200, 100, and 50 mu-g daily) and 25-hydroxycholecalciferol (25-OHD3; 40, 20, and 10 mu-g daily) for 12 weeks. The results, when compared with the effects of calciferol (vitamin D2; 200, 100, and 50 mu-g daily) in 40 epileptic outpatients, showed different actions in anticonvulsant osteomalacia of vitamin D2 on the one hand and vitamin D3 and 25-OHD3 on the other. In the patients who received vitamin D2 an increase in B.M.C. was found whereas serum calcium was unchanged. The patients who received vitamin D3 or 25-OHD3 showed an increase in serum calcium but unchanged values of B.M.C. The results suggest that liver enzyme induction cannot alone explain anticonvulsant osteomalacia.

[131I]Hippuran renography in normal subjects and in patients with essential hypertension.

Seventeen normal subjects and 21 patients with essential hypertension were examined by [131I]hippuran renography. Both groups were studied twice on the same day, first in a state of moderate dehydration and then after rehydration. From these data, four sets of reference intervals were established for several renographic parameters. Differences between the two groups were defined. The most impressive observation was a greater functional disparity between the two kidneys of one individual in the group of hypertensive patients, particularly when in a state of moderate volume depletion. This was reflected in a greater variance of certain kidney-to-kidney ratios derived from the renographic tracings. This article presents basic data to be utilized in clinical renography, as described in an accompanying article.

Diagnostic value of renography for detection of unilateral renal or renovascular disease in hypertensive patients.

In an effort to find an optimal set of renographic reference parameters for the study of hypertensive patients, the renographic data from 50 hypertensive patients with unilateral renal or renovascular disease were assessed against 95% confidence intervals obtained in normal subjects and in patients with essential hypertension in two different states of hydration. The highest all-over nosographic sensitivity and specificity were obtained by using the data from hydrated normal subjects as reference parameters. The nosographic sensitivity and specificity found in the selected series of hypertensive patients were converted to the clinically relevant predictive values of a positive and a negative test by assuming the prevalence of unilateral renal or renovascular disease in hospital populations of hypertensive patients to be 5% or 10%. In these model populations the predictive value of a postive was 33% and 51%, respectively. The predictive value of a negative test was higher than 99% for both populations. We find that predictive values of that order of magnitude are satisfactory for a test to be used in the initial phase of a diagnostic process.

Fractional intestinal absorption and retention of calcium measured by whole-body counting. Application of a power function model.

By application of a power function model, fractional intestinal calcium absorption was investigated with a new technique involving whole-body counting after successive oral and intravenous administration of standard doses of 47Ca. The fractional calcium retention 7 days after the oral load of 47Ca was also measured. Fractional calcium retention averaged 30.3% in normal subjects and 11.5% in 11 patients with intestinal malabsorption. In the same groups fractional calcium absorption averaged 46.6% and 16.4%, respectively. Fractional calcium retention and intestinal calcium absorption were significantly correlated to body surface area, and there was a well-defined relation between fractional retention and absorption of calcium...
