[Early diagnosis of endocrine ophthalmopathy using eye movement tonometry].

In a certain number of cases (about 30%) of autoimmune hyperthyroidism the pattern of an endocrine ophthalmopathy was observed which is difficult to determine especially in early stages of Graves disease. Already prior to measurable morphological changes, being determined by sonography and computertomography (41.6%), functional discrepancies could be assessed by means of temporary increases of intraocular pressure due to eye movement in 64.5% of cases. Such changes in ocular pressure were established on the basis of eye movement tonometry. Results obtained suggest that by means of the method described an early detection of eye signs associated with thyroid diseases is possible and thus enabling an early differentiation between autoimmune and non-immunogenic thyroid disorders.

[Effect of celiprolol compared to propranolol on thyroid metabolism].

In series of 20 patients suffering from atrial arrhythmias, hypertension or hyperkinetic heart syndrome in euthyroid subjects the effect of the new cardioselective beta-blocker celiprolol was examined when orally administered in doses of 300 mg per day against propranolol in a dosage of 160 mg per day. Celiprolol caused a significant reduction (p less than 0.01) of both systolic and diastolic blood pressure while propranolol showed simply a marked decrease in systolic blood pressure. A significant decrease in heart rate was observed with both drugs (p less than 0.01). TT3 and TT4 decreased only slightly when administering either celiprolol or propranolol. A further not significant drop in FT4 due to celiprolol is contrasted to a similarly not significant increase in FT4 when administering propranolol. A striking decrease in FT3 was induced by both drugs, although in the case of celiprolol this reduction became apparent only within a period of 3 weeks. Since only in cases of hyperthyroidism the number of beta-receptors has increased, the reduction of 5'deiodinase is possibly not so pronounced in euthyroidism. In this way an explanation may be found for a mild inhibition of conversion and a consequently missing increase in RT3. Neither celiprolol nor propranolol caused significant changes of laboratory findings, especially when considering triglycerides and creatinine values. Side effects could be lessened in nearly all cases and deterioration was not observed so that therapy never had to be discontinued. Celiprolol has to be considered as a further favourable cardioselective beta-blocker with intrinsic sympathicomimetic activity, causing a reasonable reduction in blood pressure and heart rate and moreover a propranolol-like effect in the peripheral metabolism of thyroid hormones in euthyroid patients.

[Urinary excretion of iodide and incidence of goiter in Styria--studies 20 years after the introduction of compulsory iodinated salt prophylaxis].

20 years after introduction of iodized salt prophylaxis by law in Austria the iodine supply of 6 to 19 year-old school children in Styria was examined. In this study we compared the goitre incidence and urine iodine excretion in children from regions with low iodine intake (locations Graz and Oberwölz) with those of an area where iodine supply was augmented by drinking local iodine containing mineral water (area of Sicheldorf). A modified ceric ion arsenate method was applied for determining urinary iodine excretion. Both in the city of Graz and the rural locality of Oberwölz there still exists an iodine deficiency, grade I (Urine iodine excretion of 50 and 65 micrograms I/g Cr respectively). The goitre incidence in 6 to 10 year olds in Graz amounted to only 4%, while Oberwölz records 8%. This figure in juveniles was relatively high already with 12% in Graz and 15% in Oberwölz. The age group of 15 to 19 years at the Graz secondary school showed a goitre incidence of 12%. The results of the 6 to 10 year olds in the Sicheldorf area recorded a goitre incidence of 4%. [Determination of the free thyroxine concentration (FT4) in serum using the FT4 fraction and total thyroxine concentration].

A new equilibrium assay for the determination of serum free thyroxine was evaluated in 514 patients. The assay comprises a two-vial-procedure to measure total thyroxine and free thyroxine fraction by use of monoclonal antibodies. Free thyroxine concentrations are calculated from fT4-fraction and total thyroxine concentration readings. In euthyroidism the average free thyroxine fraction (%fT4) was 0.011%, in hyperthyroidism this fraction was elevated, in hypothyroidism it was below normal. In patients with TBG anomalies, TBG values were inversely correlated with fT4 fraction readings. The "euthyroid reference range" of FT4 (SPAC ET) was between 0.70 to 1.78 ng/dl. This euthyroid range of FT4 was determined from TT4 concentrations measured by T4-RIA (SPAC T4 MONO) which were 30% above TT4 values measured by conventional T4-RIA (SPAC T4 POLY; polyclonal antibodies). However, a different euthyroid range of FT4 between 0.55 to 1.30 ng/dl was observed as well as by other investigators when conventional T4-RIA measurements were used for calculation of FT4 values. Our results indicate that calculated FT4 concentration values are highly dependent on the methods used for determination of total thyroxine concentrations. Precision and reproducibility of this two vial equilibrium assay did not meet the requirements mandatory for the application as a clinical routine diagnostic procedure, and its general use for this purpose can as yet not be recommended.

[Vitamin A and carotene in thyroid diseases].

From 190 goitrous patients (106 euthyroid, 53 hyperthyroid, 31 hypothyroid) serum levels of vitamin A and carotene were obtained. The serum levels of vitamin A were significantly decreased in both hyperthyroidism and hypothyroidism, the serum levels of carotene in hypothyroidism only. Remarkably, vitamin A levels almost never drop to subnormal values in hyperthyroidism. There is evidence, that a sufficient dietary protein supply enables the liver cell to produce enough amounts of retinol binding protein and prealbumin to overcome the increased clearance observed in hyperthyroid conditions.

[Determination of triiodothyronine in serum with a heterologous enzyme immunoassay: results of a group survey].

This paper summarises the results of the evaluation of a heterologous enzyme immunoassay in antibody coated tubes for the determination of triiodothyronine (T3) by a group of 11 laboratories. The assay procedure is analogous to an established heterologous thyroxine enzyme immunoassay. The evaluation demonstrated that 1. the specificity of the assay and its analytical range between 0.46 and 9.2 nmol/l meet diagnostic requirements, 2. the intra- and interassay precision was in accordance with that commonly found in radio immunoassays, 3. the recovery of added T3 was between 90 and 103%, 4. the results agreed well with those of self-established radio immunoassays and were comparable with commercial RIA kits, 5. there was no interference by hyperbilirubinaemia or by high concentrations of bile acids, but interference can occur in very haemolytic and lipaemic samples. The great advantage of T3 enzyme immunoassays lies in the absence of restrictions and any authorization needed for working with radioactive substances.

[20 min 99mTc thyroid uptake (author's transl)].

The 20 min 99mTc thyroid uptake compares favourably with the more time consuming 24 h 131J uptake because it results in a lower radiation dose to the thyroid gland. The reliability of both methods is nearly the same. In all conditions in which the 131J uptake is altered, a similar deviation of the 99mTc uptake has been found. A single exception are patients treated with antithyroid drugs (together with thyroid hormones), who show frequently an elevated 99mTc uptake (in contrast to the 131J uptake). Prognostic conclusions have been drawn by other autors from the 99mTc uptake in similar cases.
