Expression of 7F7-antigen, a human adhesion molecule identical to intercellular adhesion molecule-1 (ICAM-1) in human carcinomas and their stromal fibroblasts.

The 7F7-antigen is a widespread 85-kDa membrane adherence molecule involved in heterotypic adhesion between PHA-blasts and fibroblasts. Immunofluorescence analysis of COS cells transfected with clone pICAM-I indicated that 7F7-antigen is identical with ICAM-I, the ligand for the contact molecule LFA-I. We have investigated the expression of ICAM-I on several human carcinomas. Tumor cells in most carcinomas, with the exception of squamous-cell carcinomas, expressed very little ICAM-I or none at all. In contrast, marked expression of this molecule was observed on fibrous tissue in the vicinity of carcinoma cells, its intensity correlating with lymphatic infiltration in these tumors. We also examined the expression of ICAM-I on carcinoma cell lines and its induction by treatment with interferon-gamma (IFN-gamma). The inducibility of ICAM-I expression on cultured fibroblasts by several lymphokines suggests that the expression of ICAM-I in the vicinity of carcinoma cells is effected by lymphokines produced by activated lymphocytes/macrophages within the tumor.

Incidence of septic and thromboembolic-related deaths after splenectomy in adults.

In a review of 37,012 autopsies over the last 20 years 202 deceased adults who had had a splenectomy were investigated. The incidence of infections and thromboembolic complications related to death in these patients was compared with that of a matched deceased population (n = 403) who had not undergone splenectomy. Death-related pneumonia was diagnosed frequently in the splenectomy group and to a lesser extent in the control group (57.9 versus 24.1 per cent, P less than 0.001). Lethal sepsis with multiple organ failure occurred in 6.9 per cent of the splenectomy group and in 1.5 per cent of the controls (P less than 0.001). Purulent pyelonephritis was observed in 7.9 per cent of the splenectomy group and was significantly more frequent than in the control group with its rate of 2.2 per cent (P less than 0.001). Finally, pulmonary embolism was the major or a contributory cause of death more often in the splenectomy group than in the control group (35.6 versus 9.7 per cent, P less than 0.001). We conclude that splenectomy generates a considerable life-long risk of severe infection and of thromboembolism.

[The clavicular fracture--a questionable achievement in modern obstetrics].

The increase in fractures of the clavicle as a birth trauma at the Innsbruck University Gynecological Clinic in the last decades, which was substantial in contrast to the improvement in perinatal mortality and morbidity, indicated an analysis of the reason. With the exception of the high average birth weights, the causal factors crystallised out, in the broadest sense, as being related to the control and direction of labour. The anticipated fetal distress condition at the end of the expulsion period proved to be of primary importance. This resulted in a too early and traumatic delivery, which could not be justified by the good navel arterial pH and Apgar values. By observing certain procedures, it was possible to reduce the frequency of fractures of the clavicle in spontaneous births from 2 to 1.25% maintaining the same fetal outcome.

[The intraperitoneal installation of gamma interferon for the treatment of refractory ovarian carcinoma].

Patients with refractory ovarian cancer were treated intra-peritoneally with interferon-gamma. The maximum tolerated dose was achieved at 2 mg/m2. The substance was administered 3 times per week every second week. Interferon-gamma treatment activated locally the macrophages and induced a rise in neopterin urine, serum, and ascites levels. The tumor marker CA-125 showed marked fluctuations of more than 100% during interferon treatment and this was not correlated with neopterin. A flu-like syndrome and especially fatigue were the dose limiting side effects. Two of 3 evaluable patients died on tumor progression whereas one is now 18 months clinically free of disease.

Steroid hormone receptors and prognosis in breast cancer.

The importance of steroid receptors for the prognosis of mammary carcinoma has been evaluated by investigating the course of disease in 163 patients for a median follow up time of 66 months after mastectomy. Multivariate analysis including estrogen receptor (ER), progesterone receptor (PgR), the presence of 8S and 4S ER together or 4S ER only, and the lymph node status revealed only the latter to have significant (p less than 0.001) predictive potency. Lymph node positive (N-pos) patients had a 3.3 (1.7-6.2) fold risk of death and 2.8 (1.7-4.7) fold risk of recurrence relative to node negative (N-neg) patients. When we compared overall survival (OAS) and disease-free survival (DFS) in the various receptor-positive groups with the groups that displayed neither ER nor PgR, significant differences in prognosis were only seen in N-neg patients. PgR did not turn out to be a better prognostic factor than ER, nor was the 8S ER a sign of increased OAS and DFS compared to total ER. However, the number of patients in this group was too small to allow a definite statement.

Correlation between steroid hormone receptors, histological and clinical parameters in ovarian carcinoma.

Estrogen (ER) and progesterone (PR) receptors in ovarian tumors of 62 patients (51 carcinomas, 11 benign tumors) were estimated by the dextran-coated charcoal method using Scatchard plot analyses. 63% of carcinomas were ER-positive (greater than 10 fmol/mg cytosol), 38% were PR-positive (greater than 25 fmol/mg cytosol), whereas in benign tumors only 45% were ER-positive and 36% were PR-positive. We found no statistically significant correlation between receptor content and stage of disease, menopausal status or age of the patient. The highest concentration of ER and PR was observed in patients between 61 and 70 years of age. Life table analysis for patients with advanced ovarian carcinomas showed no significant difference in survival time in the group with higher ER and PR content. This study also reports the results obtained in a group of patients with receptor-positive ovarian carcinomas treated with a combination of chemotherapy and antiestrogen therapy. In comparison to treatment with cytotoxic chemotherapy alone, no significant difference in the time of survival or duration of remission could be found.

[Status of surgery in therapy of cervix cancer].

Between 1969 and 1980 296 patients with cervical cancer were treated by surgery in our department and some were given postoperative irradiation. The five-year survival rate was 96.2% in stage I a cases and 85.2% in stage Ib. The surgically obtained specimens were evaluated for tumour extension and lymph node spread. These parameters were examined for their prognostic value. The tumour size correlated with the five-year survival rate. Lymph node spread was found in 1% of stage Ib cases. Intraoperative complications were seen in 2.4% of patients. Postoperative complications appeared in 44%. Late complications were found in 19.1%. Diagnostic conisation is of predominant value for the adequate surgical management of cervical cancer stage Ia and borderline stage Ib cases. Our own results and comparable data of other authors allow the following guidelines to be laid down for the operative treatment of early invasive cervical cancer: In stage I a: When the tumour volume is less than 1 mm3 (early stromal invasion according to Lohe) simple hysterectomy (in special cases possible only conisation) is sufficient. Microcarcinoma (tumour size less than 10 X 10 X 5 mm) is best treated by the conventional Wertheim operation. When tumour invasion of blood or lymph vessels is diagnosed, additional lymphadenectomy must be performed. In stage Ib-IIa: Radical operation according to the Latzko technique is advisable.
