[Cell kinetics of human lung small cell carcinomas transplanted into nude mice].

Three human lung small cell carcinoma (SCC) xenografts serially transplanted into BALB/c nude mice were used for cell kinetic analysis. SCC strains, Lu-24, Lu-130 and Lu-134, were inoculated into the backs of nude mice, and when each tumor reached more than 300 mg, 50 microCi of 3H-thymidine per mouse was administered ip. The percentage labeled mitosis curve was obtained from the autoradiographic specimens which were labeled by the pulse-chase method. Cell cycle phase, growth fraction (GF) and cell loss factor (CL) were assessed by the methods of Quastler, Fujita and Steel, respectively. These cell kinetic parameters were compared with those of six control human gastrocolic and breast carcinoma xenografts which were previously reported by us. It was noticed that the cell cycle times (Tc) of SCC were statistically shorter than those of controls and this short Tc was found to be dependent on their short post-mitotic resting phases. GFs and labeling indices of SCC were observed to be statistically lower than those of controls, suggesting an incomplete adaptation of SCC xenografts to the host nude mice. Whereas some modifications by the host mice on the cell kinetics were supposed, the characteristics of SCC cell kinetics were thought to be essentially preserved in nude mice and these kinetic parameters were observed to be stable throughout the serial transfers. Accordingly, the SCC xenograft-nude mouse system was considered to be useful as an experimental therapeutic model of human lung small cell carcinomas.

Hyperproduction and gene amplification of the epidermal growth factor receptor in squamous cell carcinomas.

Human squamous cell carcinoma tissue was screened to determine the epidermal growth factor (EGF) receptor level. In 9 out of 15 cases, increased EGF binding was observed relative to normal adjacent tissue. In two tissue samples (SCE1 and SCL1), amplification of the EGF receptor gene and increased mRNA level were also observed. In two other cases (SCE2 and SCL3), EGF receptor gene amplification did not occur. We propose that there is an alternative mechanism for EGF receptor hyperproduction, independent of gene content, active in these tissues. A possible role of EGF receptor hyperproduction is envisioned in human neoplasia.

Numerous mast cells in an 11-deoxycorticosterone-producing adrenocortical tumor. Histologic evaluation of benignancy and comparison with mast cell distribution in adrenal glands and neoplastic counterparts of 67 surgical specimens.

Pathologic study of a rare 11-deoxycorticosterone-producing adrenocortical tumor causing primary aldosteronismlike signs and symptoms, revealed several characteristic features as follows: (1) fairly large size with histologic features corresponding to those of benign zone glomerulosa-type aldosteronoma, (2) lack of spironolactone (S) bodies despite S administration, and (3) heavy mast cell infiltration. In order to explain this rare histology, the localization of mast cells in the adrenal glands and functioning adrenocortical tumors of 67 surgical specimens were investigated. The results of the study supported the view that detection of mast cells helps in the differentiation of mineralocorticoid-producing tumors from cortisol-producing ones, and that the observed mast cell infiltration was due, in part, to its production of 11-deoxycorticosterone.

[Clinical investigation of oral high-dose medroxyprogesterone acetate (MPA) in advanced breast cancer--double-blind controlled study: Japanese Group for MPA Treatment of Breast Cancer].

A randomized controlled trial was made to compare the therapeutic result of oral high-dose medroxyprogesterone acetate (HD-MPA) versus mepitiostane (MS) in the treatment of postmenopausal breast cancer. MPA (1200 mg) was given p.o., b.i.d. to 47 patients and produced objective response in 19 of them (40.4%). Objective response was seen in 14 of the 40 control patients given MS p.o., b.i.d. (35.0%). Among patients with bone metastases, 6/19 (31.6%) for HD-MPA and 2/23 (15.4%) for MS showed objective response. The other merits of HD-MPA suggested in the study were improvement in performance status, anabolic effect and myeloprotective effect.

[Tumor markers of pancreatic cancer].

To determine the clinical usefulness for diagnosis and monitoring of pancreatic tumor markers, serum levels of CA 19-9 (RIA; less than or equal to 37U/ml), POA (EIA; less than 11 U/ml), CEA (EIA; less than or equal to 4.4 ng/ml) and TPA (RIA; less than or equal to 110 U/ml) were measured in 57 patients with pancreatic cancer and 25 patients with benign diseases of the pancreas. Frequencies of elevation for pancreatic cancers were 73.9% for CA 19-9, 69.2% for POA, 48.9% for CEA and 73.2% for TPA. Furthermore, the frequencies of elevation at relatively early stages of pancreatic cancer were not high. It was therefore thought that these markers were unsuitable for early diagnosis of pancreatic cancers. Although serum levels of CA 19-9 and POA in cases of pancreatic cancer were distributed up to on extremely high level, those for benign diseases of the pancreas were located up to twice the values of the cut-off levels. Therefore, these two markers seem promising for differentiating preoperatively between pancreatic cancer and benign diseases of the pancreas. CA 19-9 and POA were also useful for monitoring the recurrence of pancreatic cancer. In particular, patients with high preoperative levels seem good candidates for postoperative monitoring.

[Diagnostic usefulness of serum levels of pancreatic oncofetal antigen (POA) and CA 19-9 in pancreatic cancers: monitoring of postoperative recurrences].

Serum levels of POA (EIA; less than 11.0 U/ml) and CA 19-9 (RIA; less than or equal to 37 U/ml) were measured in 54 patients with pancreatic cancer and 19 patients with benign diseases of the pancreas. Although serum CA 19-9 and POA were elevated in 72.7% and 69.4% of patients with pancreatic cancer, respectively, patients in relatively early stage showed low percent of elevation. Therefore, these markers are unsuitable for early diagnosis of pancreatic cancers. However, these markers seem promising in differentiating preoperatively between pancreatic cancer and benign disease of the pancreas. These markers are also useful for monitoring the recurrence of pancreatic cancers. Of particular, patients with preoperatively high level seem good candidates for postoperative monitoring.

[Randomized study of long-term adjuvant chemotherapy with Futraful and mitomycin C in gastric cancer. A second study (fourth report)].

In view of the usefulness of long-term adjuvant chemotherapy, the Cooperative Study Group of Surgical Adjuvant Chemotherapy for Gastric Cancer adopted as a second cooperative study a randomized controlled trial including three groups; MMC alone, Futraful alone and a combination of MMC and Futraful. In the groups given Futraful alone and the combination therapy, the survival and disease-free rate were improved at five years, in patients with stage III. Moreover, the survival and disease-free rate for the group given combination therapy were higher than those for the group given Futraful alone. Also, in the groups given Futraful alone and the combination therapy, toxicities probably due to the long-term Futraful therapy were minor, without serious side effects. Therefore, a long-term combined adjuvant chemotherapy with MMC and Futraful seems to be an effective chemotherapy regimen for resectable gastric cancer. A more potent regimen and cyclic therapy may be needed to improve the result of treatment.

[A multi-institutional study on postoperative adjuvant immunochemotherapy of gastric cancer].

An analysis of 3,630 gastric cancer cases from 412 institutions who were treated with 6 randomly-assigned protocols was carried out to study the effect of daily PSK and Futraful administration and the adjuvant effect of PSK and/or Picibanil (OK 5 KE) combined with MMC and FT. Evaluation of immunochemotherapy was made on the basis of three-year survival rates and immunoparameters. The most positive results were found in survival rates favoring the combined therapy (FT + PSK) over single administration (FT) to relatively early-stage patients (30%, difference), PSK, MMC and FT combination (10%), and PSK, OK, MMC and FT (22%), over single chemotherapy alone. Serum alpha-2 globulin levels, indicative of immunosuppression, were lower in PSK-administered cases. The overall findings indicated that PSK and/or OK with MMC and FT are effective in prolonging the lives of post-operative patients.

[Quantitative and qualitative analysis on the lymph node metastasis of carcinoma of the esophagus].

The results of quantitative and qualitative analysis of lymph nodes dissected during resection of the esophageal carcinoma are as follows. The rate of positive lymph nodes per number of cases was 51.3% (120/234 patients). The rate of positive lymph nodes per number of nodes examined was 15.9% (406/2548 nodes). The correlation between the n-number and the average number of positive lymph nodes for a patient is shown. In the cases in which tumors disappeared from the adventitia of the esophagus by pre-operative radiation, the rate of positive lymph nodes per number of cases and of nodes examined were less than other cases. The metastases in the distant lymph nodes were much less. In the cases in which tumors reached the surgical margin on the adventitia side of the esophagus, the rate of positive lymph nodes per number of cases and of nodes examined were more than others. The metastases in the distant lymph nodes were much more. The pre-operative radiation had no influence on the spreading of the lymph node metastasis. The n-number, the number of positive lymph nodes and of dissected nodes, and the rate of them for each patient had great influence on long-term results.

[Lymph node involvement in carcinoma of the esophagus--re-evaluation for the grouping of lymph nodes].

Based on the analysis of the lymph nodes dissected during resection of 234 esophageal carcinomas and the long-term results of the patient, lymph nodes of the esophagus were grouped into 3 categories, n1, n2 and n3. The lymph nodes of n1 group were frequently involved in metastases and the long-term results of the patients with these lymph nodes metastases were excellent. The n2 lymph nodes were frequently involved in metastases but the survival rates of the patients were low. The n3 lymph nodes were rarely involved and the prognoses of the patients were poor. In comparison with the classification of lymph nodes for surgical dissection as described by the Japanese Society for Esophageal Disease, some problems in this category are discussed. However, this category was revealed to have close relation to the long-term results of the patients with carcinoma of the esophagus and was considered to be of clinical use.

[Selective hepatic arterial infusion of liposomes containing antitumor agents].

To improve the targeting effect in cancer chemotherapy, liposomes containing Adriamycin (Lip-ADM) and monoclonal antibody-conjugated Lip-ADM (Lip-ADM = Ab) were prepared and examined experimentally and clinically. The liposomes used as biodegradable drug carriers, were prepared from a lipid mixture of egg yolk phosphatidylcholine, cholesterol and dipalmitoyl phosphatidic acid and Adriamycin (ADM) solution was added to prepare multilamellar vesicles (MLV). Small unilamellar vesicles (SUV) were prepared from MLV by sonication and monoclonal antibodies were conjugated to SUV by the SPDP method. Experimentally, antigen-specific acceleration of in vitro and in vivo anti-tumor effects was observed. As a high tissue distribution of Lip-ADM in the liver was observed and as sinusoidal capillaries, which are found in the liver, were expected to allow penetration of liposomes through the relatively large gaps in the endothelium into the underlying organ parenchymal cells, clinical application of Lip-ADM was started for patients with hepatic metastasis of gastrointestinal cancer by selective hepatic arterial infusion. Ten patients were treated by Lip-ADM which was administered at 8-30 mg/body cumulatively. No serious adverse reaction was observed except slight fever and leukocytopenia. The preliminary evaluation of anti-tumor effects on hepatic metastasis was LPR, 1MR, 6NC and 1PD. Repeat administrations of Lip-ADM or Lip-ADM = Ab using a retained infusion catheter and silicone reservoir were started in order to improve the more accelerated clinical effects.

[Development of 1,2-dimethylhydrazine-induced colonic neoplasia in rats and changes in cellular proteins of colonic mucosa].

Development of 1,2-dimethylhydrazine (DMH) induced colonic neoplasia were studied using male Wistar rats given 120 mg DMH per kg s.c. weekly for 5 weeks. During the course of colon carcinogenesis, changes in cellular proteins of colonic mucosa were analysed by two-dimensional gel electrophoresis. Rats were sacrificed just before and at 10, 15 and 20 weeks after the initial DMH treatment together with controls. Incidence and number of colorectal tumors gradually increased. At the 20th week, colon carcinoma was found in every rat. Most tumors (92%) were found in the major flexure and the distal colon and rectum, while only 1% and 7% were found in the cecum and proximal colon, respectively. Histologically, most (92%) were classified as well differentiated or moderately differentiated adenocarcinoma. Eighty-five percent of the tumors were semipedunculated or sessile without depression, and the remainder were sessile with depression. All of the latter were carcinomas with invasion to the submucosa or further. Two-dimensional gel electrophoresis revealed 180 spots in cellular proteins before and after the initial treatment. Three new spots appeared and four spots greatly increased during the course of carcinogenesis, while one spot disappeared. The above results suggest that the appearing and increasing spots may be associated with cancer and that the disappearing spot may be associated with the normal colon.

[Auchincloss's mode versus Patey's mode in early breast cancer].

On the assumption that an early breast cancer is tumor under 1.0 centimeter in diameter, sixty five cases of the early disease were picked out from 970 patients who had breast cancer during 20 years from 1961 to 1980. The former were 6.7 percentage among them. Only five cases (7.6%) among patients with such early breast cancer had metastasis in the axillary lymph nodes, except lymph nodes of the level III group. On the other hand, ten year survival after modified radical mastectomy for early breast cancer was 93 percentage. An inquiry of the atrophy of the major pectoral muscles after modified radical mastectomy revealed that it was rather fewer after Auchincloss's mode than after Patey's. In conclusion, the two facts suggested that surgery of Auchincloss's type was the most available for such early breast cancer.

Primary structure of Vicia angustifolia proteinase inhibitor.

The complete amino acid sequence (72 amino acid residues) of a double-headed proteinase inhibitor from seeds of Vicia angustifolia L. var. segetalis Koch has been determined and compared with those of other double-headed inhibitors of known structure. Sequencing was performed by conventional methods with the aid of the fragments produced by reduction and S-carboxymethylation of the enzymatically modified inhibitors, and also using tryptic and chymotryptic peptides. The positions of the 14 half-cystine residues agreed among all the reported primary structures of the legume double-headed inhibitors. However, V. angustifolia inhibitor possessed extensive amino acid differences compared to the others. The phylogenetic relationship among these inhibitors was established using the unweighted pair-group method and revealed that the V. angustifolia inhibitor and the peanut inhibitor B-III had diverged at a relatively earlier stage compared to the other inhibitors.
