OBJECTIVE: To assess the clinical effectiveness and tolerability of wax-matrix, controlled-release nicotinic acid (CNA) in persons with hypercholesterolemia. DESIGN: Randomized, double-blind, placebo controlled, crossover trial. SETTING: Ambulatory clinic at an academic cardiology center in Moscow, Russia. PATIENTS: A volunteer sample of 135 men and women, aged 20 to 70 years, with hypercholesterolemia greater than 5.82 mmol/L (225 mg/dL) (70th-95th percentile for age and sex) who otherwise met study inclusion and exclusion criteria, were initially recruited into the study. Cholesterol levels were reduced to less than 5.82 mmol/L (225 mg/dL) in 46 subjects who participated in the initial diet intervention and were excluded from the drug intervention. Eighty-nine subjects were randomized into the clinical trial; 4 subjects (4.5%) dropped out of the study because of intolerance of CNA. INTERVENTION: Eight weeks of diet alone (American Heart Association Step I Diet) was followed by randomization to 2 treatment groups (1500 mg/d CNA [ENDURACIN] or placebo) for 2 months followed by a crossover of treatments for 2 months, followed by all subjects taking 2000 mg/d of CNA for 2 months. MAIN OUTCOME MEASURES: Significant improvements in baseline measures for total serum cholesterol (TC) and low-density lipoprotein cholesterol (LDL-C) were observed after initial diet (TC, 6%; LDL-C, 6%; P < .001, t test), after 1500 mg/d CNA (TC, 14%; LDL-C, 18%; P < .001, t test), and after 2000 mg/d CNA (TC, 16%; LDL-C, 21%; P < .001, t test). Triglyceride, high-density lipoprotein cholesterol, and lipoprotein(a) levels also improved. No serious toxic reactions were encountered, and 4 subjects withdrew from the study because of intolerance of cutaneous and gastrointestinal adverse effects. CONCLUSION: Wax-matrix CNA is an effective and well-tolerated pharmacological treatment for hypercholesterolemia.
The effect of micronized fenofibrate (Lipanthyl 200 M, Laboratoire Fournier, France, in dose 200 mg per day) on the serum level of 7 alpha-hydroxycholesterol (7 alpha-(OH)C) was studied in 10 men (aged 31-60 years) with hyperlipidemia (total C > 6.5 mmol/l). The levels of 7 alpha-(OH)C as well as that of total C, LDL C, VLDL C and HDL C and triglycerides (Tg) were measured before Lipanthyl 200 M treatment (point 0) and after 1, 2 and 3 months of the drug administration. The content of 7 alpha-(OH)C was determined by the reversed phase HPLC after the enzymatic conversion of serum 7 alpha-(OH)C to 7 alpha-(OH)-4-cholesten-3-one in cholesterol oxidase reaction. 7 beta-(OH)C was used as the internal recovery standard. Lipanthyl treatment resulted in considerable reduction of total C, VLDL C and LDL C levels and 7 alpha-(OH)C content. After the second and third months of therapy serum levels of 7 alpha-(OH)C were significantly reduced from 1.3 +/- 0.1 to 0.8 +/- 0.2 and 0.7 +/- 0.1 mumol/l; (P < 0.02). The decrease of 7 alpha-(OH)C content was associated with the decrease in Tg and VLDL C levels. Thus, our data suggest that the level of 7 alpha-(OH)C of in human serum may be used as an indicator of intensity of cholesterol oxidation into bile acids.

Single administration of hydrocortisone into rabbits with experimental atherosclerosis increased the content of atherogenous classes of lipoproteins (Low Density Lipoproteins (LDL) and Very Low Density Lipoproteins (VLDL) in the animals with distinct atheromatosis. Content of the lipoproteins was decreased in animals, which had no atheromatous alterations in aorta under the effect of atherogenous diet. Within 5 hrs after the single administration of hydrocortisone in rabbits with pronounced atheromatosis of aorta relative content of cholesterol was increased in the VLDL fraction. In rabbits, resistant to development of atheromatosis, composition of VLDL was not altered. Under the effect of hydrocortisone the LDL fraction fo lipid composition was altered in the direction of relative increase in triglycerides content. In the LDL fractions subfractions were observed, which were similar to the VLDL in their cholesterol/triglycerides ratios. The data obtained suggest that in blood plasma transformation of the lipoproteins was impaired under the effect of hydrocortisone. Hydrocortisone influenced the spectrum and lipid composition of lipoproteins in blood serum independently of the degree of hypercholesterolaemia.
Mathematical model was developed for transport of apo B in blood plasma, which allowed to estimate constants of metabolism of lipoproteins containing apo B, considering the alterations of apo B concentration in circulation followed by immuno- or hemosorption. Some cases were detected where stimulating effect of these procedures on the rate of apo B entering into circulation was found as well as definite ratios were shown between the constants using the parameters of alterations in apo B concentration after elimination of lipoproteins containing apo B from blood plasma. Existence of cases was predicted where fluctuations in apo B concentration attenuated after immuno- and hemosorption which were difficult to find in experiments.
