[Immunological prognosis of the effectiveness of soybean diet].

During 50 days a group of 23 volunteers received lunches prepared on the basis of textured proteins of soy beans. Each lunch included 50 g of proteins. Laboratory investigation was carried on dynamic values of lipid metabolism, cell-mediated, humoral and local immunity. A total of more than 30 tests were made. The analysis of immunological reactivity in volunteers has revealed the immunomodulatory activity of soy bean protein similar to the reference modulator with known quantitative estimation of effects on cell mediated, humoral and local immunity. Statistic analysis of preliminary individual laboratory results and plotting of prognostic curves allowed to develop an original method of volunteer selection for whom soy bean diet may bring positive or negative results.

[Association of the metabolic syndrome components in patients with arterial hypertension and their relationship with dyslipidemia].

AIM: To detect the correlation between complete metabolic syndrome (MS) and combinations of arterial hypertension (AP) with abdominal obesity (AO), hyperlipidemia (HLP), and both without disorders in glucose tolerance. MATERIALS AND METHODS: 122 men aged 35-66 years were examined, in whom AP was diagnosed 4-6 years before. The following criteria were used for detecting MS components: AP was diagnosed at diastolic AP of at least 90 mm Hg or systolic AP of 140 mm Hg; HLP was confirmed at total cholesterol (CS) level of at least 5.2 mmol/liter and/or triglyceride (TG) level of at least 2.3 mmol/liter; AO was diagnosed at body weight index of at least 26 kg/m2 and the ratio of waist/hip circumferences more than 0.90. Poor glucose tolerance was diagnosed at glucose level in capillary blood 120 mg/dl and higher, but no more than 180 mg/dl 2 h after 75 g glucose loading, if glucose level after an overnight fasting was no higher than 120 mg/dl. RESULTS: The majority of patients (39.3%) had combinations of AP with AO and HLP; complete MS was diagnosed in 21%. Isolated AP was found in only 8.2% of examinees. Dyslipidemia in the presence of MS and in combinations of AP with AO and HLP was characterized by increased levels of TG and total CS and decreased content of CS and high-density lipoproteins, which was associated with basal hyperinsulinemia. CONCLUSION: MS alone and clusters of AP, AO, and HLP characterized by insulin resistance, accelerate the development of cardiovascular diseases associated with atherosclerosis.

[Analysis of polymorphism of the angiotensin gene in patients with ischemic heart disease and in a random sample from the Moscow population].

Polymorphism of the (CA)n repetitive sequence in the angiotensinogen gene was studied in a random sample of Russian males from the Moscow population and in a sample of patients with coronary heart disease. Thirteen allelic variants of this microsatellite with 10 to 22 (CA) dinucleotides were revealed. In the patients, the spectrum of the allelic variants of the angiotensinogen gene tended to shift toward smaller numbers of repeats (n < or = 16). This shift was not associated with changes in the lipid composition of blood.

[The characteristics of the blood lipid transport system in the disabled with lost lower extremities].

A comparative analysis of major blood lipoprotein values in 108 males aged 16-65 years with lower limbs amputations has shown a higher level of triglycerides (TG) and a reduced level of high density lipoprotein cholesterol (HDL C) in them than in control males with normal limbs. In the subgroup of patients who had undergone the amputation as a result of obliterative arterial disease, the TG level was the highest while HDL C the lowest; moreover, these values appeared to be the same as in the age-matched subgroup of patients with manifestations of atherosclerosis. In the subgroup of older men with posttraumatic lower limb amputations (aged 40-59 years) the TG and HDL C levels did not differ from the corresponding parameters in age-matched subgroup of healthy subjects, whereas young amputees (17-39 years) had the increased blood TG concentration and reduced level of HDL C and apolipoprotein AI. The conclusion is made: disability following limb amputation accompanied by restricted mobility, inadequate physical static efforts as well as by chronic psychological stress seem especially dangerous for young invalids because of associated lipoprotein profile changes which can be regarded as highly atherogenic.

[The apolipoprotein AII gene is not a risk factor for development ischemic heart disease in the Moscow population].

Polymorphism of microsatellite (CA)n repeat in the second intron of the apolipoprotein AH gene--one of the major structural proteins of high-density lipoproteins--was performed in male coronary heart disease (CHD) patients and in a random sample of men from the Moscow population. No association between the polymorphism and the development of CHD was revealed. Therefore, apolipoprotein AII gene is not a factor of genetic risk of the development of atherosclerosis in the population studied.

[The effect of micronized phenofibrate on the lipoproteins of the blood plasma at different initial lipid levels].

Novel antihyperlipidemic micronized phenofibrate lipantil 200 M was tested for safety and efficacy (one 200 mg capsule a day for 3 months) in 27 patients having cholesterol level above 6.5 mmol/l. The effect emerged upon 1 month of administration and persisted till the end of the treatment. In combined hyperlipidemia triglycerides decreased by 45-60%, total cholesterol and LDL cholesterol by 10-12%. In isolated hyperlipidemia the latter two values appeared lower by 20-23 and 22-27%, respectively. After lipantil 200 M treatment HDL cholesterol went up by 9% in low baseline level (< 1 mmol/l) by 27%. No negative clinico-biochemical shifts were seen, while fibrinogen and uric acid reduced by 16%, apoB by 23-30%. Lipantil 200 M proved active against hyperlipidemia and is recommended for clinical practice.

[Analysis of polymorphism of the 5'-terminal region of apolipoprotein B gene in patients with ischemic heard disease].

Comparative analysis of polymorphism of two polymorphic regions in the 5'-end of the apolipoprotein B gene-insertion/ deletion polymorphism of a signal peptide of apo B and microsatellite (CA)n repeat-was performed in a random sample of men from the Moscow population aged 49-51 and in heart patients. No associations among the gene polymorphic regions, development of coronary heart disease, and certain changes in the lipid spectrum of plasma are revealed. This indicates that the structural organization of the 5'-region of apolipoprotein gene B (Moscow population) has no substantial role in determining the risk of coronary heart disease.

Association of high-density-lipoprotein cholesterol with mortality and other risk factors for major chronic noncommunicable diseases in samples of US and Russian men.

Previous reports from the Russian Lipid Research Clinics (LRC) study showed no association between the level of high-density-lipoprotein (HDL) cholesterol and mortality from coronary heart disease (CHD), while US LRC data indicated a strong negative association between HDL cholesterol and CHD mortality. This report investigated the association of HDL cholesterol and mortality in these same population samples with follow-up extended to 12 years. The association between HDL cholesterol and mortality remained inverse and significant in the US sample. In the Russian sample, high levels of HDL cholesterol were associated with higher risk of all-cause and cancer mortality, although adjustment for known risk factors reduced the strength of the association. The association between HDL cholesterol and CHD mortality was negative in the Russian sample, although the strength of the association was less than that for the US sample. Extended follow-up reduced the difference in the association between HDL cholesterol and mortality between the two countries; however, important differences remained. Further research will be required to clearly determine the cause for their differences.

Diagnostic value of immune cholesterol as a marker for atherosclerosis.

BACKGROUND: The serum of patients with coronary atherosclerosis contains circulating immune complexes including low-density lipoproteins (LDLs). We have developed a technique for the evaluation of LDL content in circulating immune complexes by measuring total cholesterol levels in polyethylene glycol precipitates (immune cholesterol). In the present study, the value of immune cholesterol in the diagnosis of atherosclerosis was compared with that of other laboratory parameters, such as total cholesterol levels, triglycerides, LDL cholesterol, high-density lipoprotein cholesterol, lipoprotein(a), and apolipoproteins B and A-1. METHODS: Immune cholesterol and the other parameters were determined in blood samples from 200 patients with documented coronary and extracoronary atherosclerosis. Coronary atherosclerosis was assessed by coronary angiography; stenoses in the aortic arch and branches and in lower-limb arteries were evaluated by angiography and ultrasonography. RESULTS: Only immune cholesterol and the ratio of apolipoprotein B to apolipoprotein A-1 correlated significantly with the severity of coronary atherosclerosis. The accuracy of the diagnosis of coronary atherosclerosis by immune cholesterol was 78%, considerably higher than that of other laboratory parameters. Use of a combined parameter consisting of immune cholesterol, LDL cholesterol, and the patient's age increased the diagnostic accuracy to 81%. A high level of immune cholesterol is characteristic not only of coronary atherosclerosis but also of extracoronary atherosclerosis. The sensitivity, specificity and accuracy of the diagnosis of extracoronary atherosclerosis were even higher than those for coronary atherosclerosis. CONCLUSION: Immune cholesterol may be employed as a novel marker in the diagnosis of advanced atherosclerosis.

[Allelic variants of apolipoproteins B and CII genes in patients with ischemic heart disease and in healthy persons from the Moscow population].

Allelic frequencies of a microsatellite of the apolipoprotein CII gene (APOCII) and a minisatellite of the apolipoprotein B gene (APOB) were studied using polymerase chain reaction (PCR). The study was conducted on a random sample of male Moscow inhabitants and a sample of patients with coronary heart disease (CHD) from the same population. Fourteen variants of the APOB minisatellite (the 82% heterozygosity level) and 13 alleles of the APOCII microsatellite (the 85% heterozygosity level) were found. CHD patients significantly differed from the control group in the distributions of alleles in these loci: APOB 32, APOB 46, APOB 48, and APOB 50 as well as APOCII 17 and APOCII 29 were found more frequently. A relationship was found between the distributions of APOB and APOCII in the CHD patients. The CHD patients with alleles APOCII 21 and APOCII 30 very often had the allele APOB 32; and patients with the genotype APOB 34, 36 had the allele APOCII 29 even more often than affected individuals in general. Individuals of the control group with the allele APOCII 30 exhibited hypertriglyceridemia without increased levels of total cholesterol and apolipoprotein B in plasma.

Correlates of mortality in Russian and US women. The Lipid Research Clinics Program.

Associations between selected risk factors and 7-year all-cause mortality were studied in 2,187 Russian women and 2,146 US women who were screened as part of a US-Russian collaborative program. The US women were screened during the period 1972-1976, while the Russian women were screened from 1978 to 1982. Cigarette smoking and elevated systolic blood pressure were associated with increased mortality in both samples. High density lipoprotein (HDL) cholesterol was inversely related to mortality in US women, but there was no association of HDL cholesterol with mortality in Russian women. Prevalent angina and electrocardiographic abnormalities were associated with mortality in both samples, but the relations achieved statistical significance only in the Russian sample. The problems of cigarette smoking and elevated blood pressure should be addressed with public health measures in both countries. The absence of an association between HDL cholesterol and mortality in the Russian sample should be investigated further.

Comparative study of the activity and composition of HDL3 in Russian and American men.

Previous studies conducted within the framework of the Lipid Research Clinics Program showed a strong inverse correlation between high-density lipoprotein cholesterol (HDL-C) level and coronary heart disease (CHD) risk in American male populations, whereas in Russian populations such a correlation was less pronounced. It was assumed that HDL was less protective of CHD in Russian than in American males. This study compared the functional activity and lipid composition of HDL3 isolated from the blood plasma of men with low, normal, and high HDL-C levels from Moscow (Russia) and Seattle (United States) populations. Results obtained showed that American HDL3, irrespective of the plasma HDL-C level, had higher activity in stimulating both [3H]cholesterol and cholesterol mass efflux from cholesterol-loaded fibroblasts and in suppressing cellular cholesterol esterification when compared with Russian HDL3. American HDL3 remained more active than Russian HDL3, even when apolipoprotein E-containing particles were removed from HDL3 by heparin-Sepharose affinity chromatography. Russian and American 125I-HDL3 had similar binding to high-affinity cell-surface sites, but Russian HDL3 had a higher nonspecific binding component compared with American HDL3. This study demonstrates for the first time potential functional differences between HDL particles isolated from Russian and American populations. The lower activity of Russian HDL3 in promoting cellular cholesterol efflux may partly explain the higher CHD risk in the Russian population compared with the American one.

[Long-term results of the program for preventing ischemic heart disease in Moscow (10-year follow-up study)].

The paper provides the results obtained from implementation of the programme on multifactorial prevention of coronary heart disease (CHD) in Moscow, which provided strong evidence for a reduction in the levels of major risk factors and mortality from cardiovascular diseases, including coronary heart disease. The efficiency of comprehensive prophylactic measures in persons with clinical signs of CHD showed up at year 3 of their implementation and retains in subsequent years. At the same time in persons without CHD signs a mortality reduction starting at years 4-5 of the follow-up virtually came to an end when a further active intervention ceased. The impact of aftereffect of active prophylactic measures after intervention cessation turned out to be different in the examinees at various baseline risk factor levels, which suggests that continuous prophylaxis should be done, on the one hand, and it requires a differential approach, on the other. The reduction in the extent and mean levels of major risk factors, which had been achieved during a 5-year prophylactic intervention, retained, though it was less pronounced and 10 years later. However, this was followed by an increase in the mortality from myocardial infarction and stroke and their incidence in the population that had undergone active prophylactic measures, though the value of these parameters was lower at the end of the tenth year than in the controls.
