Repair of ptosis possibly attributable to the long-term wearing of a contact lens.

In recent years contact lenses have become more popular from the ophthalmological viewpoint and because they are easy to wear and aesthetically more acceptable than eyeglasses. One of the complications produced by wearing contact lenses, however, is ptosis. We have had a patient with ptosis possibly due to the long-term wearing of a contact lens in one eye. We performed a skin resection together with repair of the aponeurosis of the levator muscle of the upper eyelid. According to our investigations, there has been only one report on the repair of aponeurosis of the levator muscle of the upper eyelid for this type of problem, and the approach and findings were similar to ours.

Clinical experience with nasolabial flaps.

The nasolabial flap was used for reconstruction of the nose, lip, and cheek in 23 patients with satisfactory results. From the standpoint of color and texture match, this flap was of considerable value for skin repair of the nose. As for the donor site, the limitation of the nasolabial flap is considered to be about 3 x 10 cm. The nasolabial flap is also discussed from the point of view of recent considerations on its special characteristics and applications.

Stimulation of glucose utilization and lactate production in cultured human fibroblasts by thyroid hormone.

Human dermal fibroblasts were obtained by harvesting outgrowing cells from the dermal tissue explants and cultured in Dulbecco's modified Eagle medium containing 10% fetal calf serum. After the cells reached confluency, culture was continued in the medium containing calf serum which was deprived of thyroid hormone by the treatment with activated charcoal. These fibroblasts were responsive to exogeneously added thyroid hormone (triiodothyronine) at physiological concentrations, resulting in enhanced utilization of glucose and production of lactate. This timulation by thyroid hormone was dependent upon the length of exposure to the hormone and its concentration. The hormone did not show any effect on cellular DNA and protein content. The experimental system described above seems to be easy to reconstitute and should be useful for the elucidation of the mechanism of thyroid hormone action.

Cryosurgery for hemangiomas of the body surface and oral cavity.

The indications for and limitations of cryosurgery for various types of hemangiomas on the body surface or in the intraoral region are reported based on our clinical experience. In our opinion, cryosurgery is not the method of choice in the management of hemangiomas but is useful under certain conditions, as with a localized cavernous under certain conditions, as with a localized cavernous hemangioma in the facial or intraoral region. It is also valuable for strawberry marks with bleeding or infection and may be acceptable for small port-wine stains; in the treatment of extensive port-wine stains, a resulting hypertrophic scar or depigmentation may present difficulties.

Aesthetic surgery of the trunk and extremities in the Japanese.

The number of patients seeking dermolipectomy of the trunk and thighs is increasing among the Japanese. The bodily proportions of the Japanese are different from those of Caucasians. Furthermore, the Japanese tend to hypertrophic scarring. We report our thigh lift technique, including preoperative and postoperative management.

Modified Limberg flap for lumbosacral meningomyelocele defects.

We have recently developed a technique for closing round or wide oval lumbosacral meningomyelocele defects by modifying the Limberg flap. Four cases of repair of lumbosacral meningomyelocele were treated, with good skin closure in 3. The indications for and advantages of our method are discussed.

Methods of vascular pedicle coverage.

Based on our clinical experience, various methods of donor or recipient vascular pedicle coverage for an arterialized or free flap are reported. The vascular pedicle may be placed in the subcutaneous space either by incising the skin along the full length of the vascular route or by passing it through a subcutaneous tunnel. It has been our experience that the vascular pedicle can also be covered by either a local flap or a free skin graft. On rare occasions, a narrow skin flap strip including the vascular pedicle can be lifted en bloc and rotated toward the base of the area transferred.

Unusual facial hemangioma.

An unusual hemangioma of the face, found in a 7-year-old girl in the masticator space and consisting of multiple lobes, is reported. Direct angiography was effective in clarifying the space occupied by the tumor. It was removed through a combined approach, using both temporal and oral incisions, with almost no bleeding.

Penile disorders in two paraplegics.

We present two rare cases of paraplegics with remarkable sequelae of the penis due to long-term usage of a condom-catheter. One involved hypertrophy of the glans prepuce, and the other had severe constriction of the distal portion of the corpus penis with two fistulae.
