Evaluation of echographic diagnosis of rectal cancer using intrarectal ultrasonic examination.

Ultrasonic examinations conducted in order to diagnose the depth of invasion and local lymph node metastases of rectal cancer. The intrarectal approach was performed preoperatively in 99 patients with rectal cancer, using either an Olympus-Aloka ultrasonic endoscopeTM (7.5 MHz) or other probes (Aloka, 7.5 MHz, 5 MHz). Through this method, intrapelvic organs were detected clearly, and hypoechoic findings due to tumors were detected in all patients. The normal rectal wall was echogenically divided into five layers, the third layer being the submucosal and the fourth layer being the proper muscle layer. In some cases, the proper muscle layer was divided into three layers in the echogram. In 79 of 88 patients, the diagnosis of depth of invasion, classified into three groups, was possible. Metastatic lymph nodes were shown as a hypoechoic round mass. In 52 of 71 patients proven to have local lymph node metastases in surgical specimens, lymph node metastases were diagnosed preoperatively. Thus, intrarectal ultrasonography provides valuable information concerning the choice of operating methods for rectal cancer.

[Preoperative clinical evaluations of rectal cancer using rectal echo, X-ray CT and MR-CT].

In order to evaluate the depth of invasion and to determine the stage of rectal cancer, we have recently performed intra-rectal echo, X-ray CT and MR-CT preoperatively. Conventional approaches such as barium enema X-ray and colonofiberscope are not satisfactory to determine the depth of invasion of tumor and to detect adjacent metastatic lymphnodes. Intra-rectal echo can reveal rectal wall layers fairly well, which can be divided into five layers. Using this technique, we are able to determine the depth of invasion of rectal cancer. X-ray CT can demonstrate all the intra-pelvic organs and metastatic lymphnodes surrounding rectal tumor. Invasion to the neighboring organs such as urinary bladder, prostate, seminal vesicle and uterus can also be evaluated preoperatively. MR-CT is also helpful to determine the stage of rectal cancer before operations. Though the images of MR-CT are not so sharp, multi-directional slices of X-ray photographs can be taken by MR-CT and so furthermore evaluations of rectal tumor invasion and adjacent metastatic lymphnodes are possible. In addition, IR-image of MR-CT demonstrates the tumor site dramatically. In conclusion, using these three techniques we can evaluate preoperative clinical stages of rectal cancer and the best operative method can be determined preoperatively.

Mesothelioma of the peritoneum during 1967-82 in England and Wales.

The time-trend and geographical distribution of mesothelioma of the peritoneum during the years 1967-82 in England and Wales have been studied from the Mesothelioma Register held by the Medical Division of the Health and Safety Executive. Over the 16-year period the annual number of cases registered rose from about 15-20 to about 30-50. Although the number occurring in men was double that in women, the trend was similar for both sexes. There is likely to be some further increase before any improvement due to the recent diminished usage of asbestos is seen. Local Authority areas with raised rates have been identified, and the geographical pattern is similar to that of the distribution of the asbestos-using industry in the past. In both sexes there are high registration rates on the east side of London but, in contrast to mesothelioma of the pleura, a concentration of cases among men in the major ports where shipbuilding and ship repairing were carried out is not so apparent.

Solubilization and characterization of rat brain alpha 2-adrenergic receptor.

alpha 2-Adrenergic receptors labelled by [3H]-clonidine (alpha 2-agonist) can be solubilized from the rat brain in a form sensitive to guanine nucleotides with a zwitterionic detergent, 3-[3-(cholamidopropyl)-dimethylammonio]-1-propane sulfonate (CHAPS). About 40% of the original [3H]CLO binding sites in the membranes were solubilized with 6 mM CHAPS. Separation of the soluble [3H]CLO-bound complex was performed by the vacuum filtration method using polyethylenimine-treated GF/B filters. Solubilized [3H]CLO binding sites retained the same pharmacological characteristics of membrane-bound alpha 2-adrenergic receptors. Scatchard plots of [3H]CLO binding to solubilized alpha 2-receptors were curvilinear, indicating the existence of the two distinct binding components. Solubilized receptors were eluted as a single peak from Bio-Gel A-1.5 m column with a Stokes radius of 6.6 nm. The isoelectric point was 5.6-5.8. Regulations of the receptor binding by guanine nucleotides, monovalent cations, and sulfhydryl-reactive agents were maintained intact in the soluble state, whereas those by divalent cations were lost. The apparent retention of receptors and guanine nucleotide binding regulatory component(s) in the soluble state may allow a investigation of the regulation mechanisms of the brain alpha 2-adrenergic receptor system at the molecular level.

[A case of triple neoplasm with acute myelogenous leukemia, squamous cell carcinoma of the lung and adenocarcinoma of the stomach].

A 70-year-old man had undergone middle and lower lobectomy of the right lung for pulmonary squamous cell carcinoma in January 1979. He was treated with anticancer drugs including alkylating agents. In February 1983, he was diagnosed as AML (FAB, M2). Although cytoreduction effect was observed, he died of prolonged pneumonia on the 30th admission day. Autopsy revealed a well-differentiated tubular adenocarcinoma of the stomach (Borrman II type) and a poorly differentiated epidermoid carcinoma of the right hilar lymph node.

Initial detection of [3H]clonidine binding to solubilized rat brain alpha 2-adrenergic receptors: regulation by guanine nucleotides.

Alpha 2-adrenergic receptors labeled by [3H]clonidine (alpha 2-agonist) can be solubilized from the rat brain with a zwitterionic detergent, 3-[(3-cholamidopropyl)-dimethylammonio]-1-propane sulfonate (CHAPS). CHAPS-solubilized receptors have the same characteristics of membrane-bound alpha 2-receptors in the brain, and the regulation of receptor binding by guanine nucleotides is retained in the soluble state.

Studies on experimental immune complex nephritis (3). Therapeutic effect of prostaglandin E1 alpha-cyclodextrin host molecule (PGE1 . CD) on serum sickness nephritis in rats.

The effect of prostaglandin E1 alpha-cyclodextrin host molecule (PGE1 . CD) was estimated by using a model of immune complex glomerulonephritis induced in rats by i.v. injection of 1 mg of rabbit serum albumin every other day. PGE1 . CD was given subcutaneously for 10 days after nephritis was definitely induced and resulted in a rapid restoration of various biochemical parameters, especially in plasma. The continuous s.c. administration of PGE1 . CD (300 micrograms/rat/day) with a mini osmotic pump provoked a therapeutic effect similar to that obtained with twice daily s.c. administration of PGE1 . CD (300 micrograms/rat X 2/day). Both PGE1 . CD groups revealed less glomerular damage and fewer locations of immune complexes in glomeruli as demonstrated by light and immunofluorescence microscopy. The beneficial effect of PGE1 . CD may be associated with reduced immune complex deposits in glomeruli. The present studies suggest that PGE1 . CD may enhance the clearance of immune complex deposits from the glomeruli rather than inhibiting the formation of immune complex in the circulation.

A prospective cohort study of hepatitis B surface antigen carriers in a working population.

A 10-year prospective cohort study was made of 495 male hepatitis B surface antigen (HBsAg) carriers aged over 40 at entry. The observed/expected mortality rate ratio of primary liver cancer (PLC) was 10.40, based on the general population in Japan. The annual incidence rate of PLC (including 2 survivals) was 365/100,000 person-years. All histologically confirmed cases of PLC were hepatocellular carcinoma (HCC). The chronic HBsAg carrier state is suggested to be a risk factor of HCC.

[Fundamental and clinical evaluation of ceftriaxone in the pediatric field].

Fundamental and clinical evaluation of ceftriaxone (CTRX) was performed in the pediatric field and the following results were obtained. The MIC of CTRX against E. coli isolated from urinary tract infections in children ranged from less than or equal to 0.024 to 0.39 mcg/ml except for 1 strain. CTRX was superior to other 3rd generation cephalosporins such as CPZ and LMOX, showing effectiveness also against ABPC-resistant bacteria. The clinical efficacy and bacteriological efficacy in 6 children consisting of 5 with respiratory tract infections and 1 with urinary tract infection were 83% and 100%, respectively. As to the adverse reaction, diarrhea was observed in 2 cases. The determination of PIVKA-II performed during the therapy with CTRX, which is observed when vitamin K is deficient, showed positiveness in 2 cases out of 6 cases including 1 which the clinical efficacy could not be evaluated. The test of platelet function in 3 cases found no inhibition of agglutination. Twice-daily administration with 20 mg/kg CTRX was considered to be a useful and safe method for treatment of bacterial infections in children, although attention should be taken not to cause vitamin K deficiency as in other 2nd and 3rd generation cephalosporins.

Estradiol suppresses electrically-induced arterial thrombosis by increasing prostaglandin biosynthesizing activity of the aorta in rats.

The anti-thrombotic effect of estradiol was studied using an electrically induced thrombus model in rats. Estradiol treatment significantly inhibited the thrombus formation induced by electric shock. In the meantime, a stimulation of prostacyclin biosynthesis by estradiol in aortas was found. This is the first report showing the correlation between the effects of the estradiol on the thrombus formation in an experimental model and on the prostacyclin production by aortas in the same animal system.

[Biological activity of 1alpha-hydroxycholecalciferol (1). Effect on intestine and bone in rats (author's transl)].

Biological activity of 1alpha-hydroxycholecalciferol was studied in rats. 1alpha-Hydroxycholecalciferol was found to be more potent and rapidly active than vitamin D in stimulating intestinal calcium transport and calcium mobilization from bone both in normal and vitamin D deficient rats. 1alpha-Hydroxycholecalciferol was also active in nephrectomized and/or thyroparathyroidectomized rats both in intestine and bone. Although it is well known that 1alpha-hydroxycholecalciferol is metabolized to 1alpha, 25-dihydroxycholecalciferol in the liver, there is the possibility that the former is active without further metabolism. In rats in which hepatitis was induced by CCl4, 1alpha-hydroxycholecalciferol was active both in the intestine and in the bone, while it was inactive in hepatectomized rats. These data clearly demonstrate that 1alpha-hydroxycholecalciferol is not active by itself and must be metabolized in the liver. This idea also shows the lag time in response of rats to 1alpha-hydroxycholecalciferol has more potent antirachitic activity than vitamin D and does not lose its activity with chronic oral administration. In view of these findings, 1alpha-hydroxycholecalciferol appears to have a good potential for clinical application in cases of renal failure and metabolic bone diseases.
