Comparison of the lipid composition of breast milk from mothers of term and preterm infants.

Milk was collected from mothers of 18 very premature (26 to 30 wk gestation age), 28 premature (31 to 36 wk), and six term (37 + wk) infants on day 2 to 3 (colostrum), and at 1, 3, 6, and 12 wk postpartum. Fat content for 154 milk samples was 2.80 g/dl gravimetrically and 2.66 g/dl by quantitative thin-layer chromatography. Fat content increased during lactation, whereas phospholipids and cholesterol declined. Concentrations of medium-chain fatty acids increased from colostrum to mature milk and were highest in preterm milk. Compensatory decreases were observed in very premature and premature oleic acid. Long-chain polyunsaturated fatty acids were highest in colostrum and reduced in mature milk. Long-chain polyunsaturated fatty acids were also higher in very premature and premature milk than in term milk. These elevated levels of readily absorbed medium-chain fatty acids and long-chain polyunsaturated fatty acids in preterm milk may be of special benefit for the needs of premature infants.

The lipolytic triad: human lingual, breast milk, and pancreatic lipases: physiological implications of their characteristics in digestion of dietary fats.

The characteristics of human lingual, breast milk, and pancreatic lipases as related to the digestion of dietary fats in infants are discussed. The activity and specificity of these enzymes and structure of the dietary fats largely determine the rates of lipolysis, the types of digestion products formed, and the rates of absorption. Also possibly influenced are micelle formation, intestinal health, breast milk jaundice, and the absorption of other nutrients. In premature infants, the action of lingual and breast milk lipase are particularly important in the absorption of dietary fatty acids.

Subjective and objective evaluation of bruxing patients undergoing short-term splint therapy.

Subjective and objective criteria were utilized to evaluate the effectiveness of short-term bite splint therapy on bruxistic behaviour. A simple bruxism monitoring device was used to check pre-treatment and post-treatment nocturnal bruxism. This preliminary study suggests that short-term splint therapy is effective in eliminating mild to moderate facial pain and dysfunction while reducing bruxistic behaviour. The bruxism monitoring device is a simple and effective tool for diagnosing bruxism quantitatively.

An assessment of school survey as a method of detection of leprosy cases.

The present study includes the analysis of case-control study and hospital records of leprosy patients. 26 cases of leprosy were detected through examination of 21,412 school children of 5-19 years age, attending 30 and 25 schools in urban and rural area respectively of Surat District. Matched controls of these leprosy cases were selected from their healthy class-mates by employing certain criteria. Home visits paid to these 24 cases and their controls revealed a positive family history in 50% (12/24) of affected students. Case detection rates were 17.9% and 0.0% among the contracts of affected students and their controls respectively. Out of 26 cases, 16 (61.5%) were detected for the first time (undetected cases). However, record analysis revealed that only 32.6% and 39.5% of the detected cases of school going age, were attending schools in rural and urban area respectively. School survey and contact survey of child cases, in spite of certain limitations, can be considered as a useful, cheap and rapid method for detection of leprosy cases in the community.

An epidemiological study of leprosy through school survey in Surat District (South Gujarat).

A school survey covering 21,412 students attending 30 primary and secondary schools in Surat city and 25 primary and secondary schools in surrounding villages was carried out. The overall prevalence rate of leprosy was 0.12% and it was significantly higher in rural area. Prevalence of leprosy did not show any association with the age of students but male preponderance was observed. Prevalence rate increased significantly with deterioration of socio-economical condition and home sanitary condition. B.C.G. vaccination seemed to protect against leprosy in rural area. Majority of the cases (92.3%) were non-lepromatous type. Single skin lesion was seen in 15.4% cases only. Nearly 1/3 of the total skin lesions were observed on the area of skin covered by clothes.

Stresses created by occlusal prematurities in a new photoelastic model system.

A new photoelastic model system incorporating a simulated periodontal ligament sleeve around an experimental first molar has been developed. The system was used to visualize internal compressive stresses created by various premature occlusal incline relationships in both buccolingual and mesiodistal dimensions. When prematurities were applied singly and observed buccolingually, both class 1 and and class 2 prematurities produced stresses along the lingual aspect of the root, whereas the class 3 prematurity produced stresses along the buccal aspect of the root. When viewed buccolingually, combinations of class 1 and class 2 prematurities resulted in stresses along the lingual aspect of the root. The stress pattern was similar to those created by the single class 1 or class 2 prematurities. A combination of class 2 and class 3 prematurities did not produce a well-defined indication of stress distribution. There was slightly more stress along the buccal aspect of the root. Comparison of this combination with individual class 2 and class 3 prematurities indicates that such a combination tended to diminish tipping of the tooth in any one direction. A combination of class 1 and class 3 prematurities showed slightly greater stress along the buccal aspect of the root surface. Again, the combination produced less tipping of the tooth in any one direction when compared with tipping caused by the individual prematurities. When all three prematurities were tested at the same time, the stress pattern was almost coincident with that achieved in the absence of prematurities. When prematurities were observed mesiodistally, the location of stresses was dependent on the mesial or distal placement of the prematurity, irrespective of its position buccolingually.
