[Immunohistochemical study of the lymph-follicles and lymphocytes in the gastric mucosa].

Immunohistochemical stainings according to ABC method (UCHL-1,L-26,MT-1,MB-1,IgG,IgA,IgD,IgM, kappa, lambda, LN-1 and LN-2) for the lymphocytes in the germinal center, mantle zone and infiltrative lymphocytes in the gastric mucosa of 30 cases of chronic gastritis and 10 cases of reactive lymphoreticular hyperplasia (RLH) were performed. Lymphocytes in the germinal center and mantle zone consisted usually of B-cells positively stained by L-26 and MB-1. However, in the interstitially infiltrative cells,T-cells positively stained by UCHL-1 and MT-1 were not infrequently contained. Immunoglobulin stainings revealed marked positivity for IgG,IgA,IgD,IgM, kappa and lambda in the interstitial lymphocytes and plasma cells. As to the RLH, small number of T-cells scattered in the germinal center and surrounding area of lymph follicles, and large number of T-cells were found among the follicles, where B-cells were more infrequently found than in the interstitial area of propria mucosae. Confusion of enlarged germinal centers and monotonous proliferation of lymphocytes among the lymph follicles showing monotonous positivity for the stains of heavy and light chains in the cases of RLH were suggestive of malignant change.

[Vasitis nodosa suspected of the spermatic cord tumor: a case report].

We report a rare case of vasitis nodosa. A 54-year-old man was found to have a right spermatic cord mass during the herniorrhaphy operation. There was no history of surgical or traumatic causes. An indurated mass in the right spermatic cord was suspected to be spermatic cord tumor, and a right high orchiectomy was performed. Histological examination revealed disruption of the ductal lumen, granulomas with the sperm-like necrotic tissue and proliferation of the lymphocytes and plasma cells in the smooth muscle bundles. Pathological diagnosis was vasitis nodosa.

[Management of vesicoureteral reflux in patients with neurogenic bladder dysfunction].

We studied 20 cases of neurogenic bladder with vesico-ureteral reflux retrospectively. Seven patients voided with Valsalva's maneuver and 12 patients were managed with clean intermittent catheterization, but one patient required permanent urinary diversion because of uncontrollable urinary tract infection. The patients were followed by intravenous pyelography, radioisotope renogram, voiding cystourethrography, determination of serum creatinine level and urodynamic study. Anti-reflux surgery was performed in 22 ureters and reflux resolved in 19 ureters. Among 12 ureters not treated by anti-reflux surgery, reflux disappeared in 5 ureters and improved in 2 ureters. Of 5 ureters, reflux resolved after clean intermittent catheterization in 2 ureters, and remained stable without recurrent infection or renal deterioration in the remaining 5 ureters. Regardless whether antireflux surgery was done or not, most of the patients who had high grade reflux and obstructive renal damage on radioisotope examinations had marked low compliance bladders. Our experience suggested the necessity of a suitable treatment to improve bladder compliance before considering anti-reflux surgery.

[The relation of stone and carcinoma of the gallbladder based on the mucosal findings].

Based on the mucosal findings in gallbladder patients, intestinal metaplasia was found in 18.4% of such cases without a stone or a carcinoma, in 30% of the with a bilirubin stone, in 37.7% of those with a cholesterin stone, in 69.1% of those with a carcinoma but without a stone and in 78.9% of those with both a carcinoma and a stone. In contrast the incidence of a well differentiated adenocarcinoma was more frequent in the cases with a stone (more often a cholesterin stone than a bilirubin stone) than in cases without a stone. Similarly, the incidence of intestinal metaplasia was more frequent in those with a cholesterin stone than in those with a bilirubin stone. Further, small size lesions were found to be more frequent in the cases of a cancer with a stone. Granular early carcinomas within the superficial muscle layer are usually resected under the diagnosis of gallstone, whereas early carcinoma that protrude are resected as a carcinoma.

[Neurogenic bladder dysfunction due to tethered spinal cord syndrome in adults: report of two cases].

Two cases of neurogenic bladder dysfunction due to tethered spinal cord syndrome in adults were experienced. They visited our hospital with the complaint of dysuria. The excretory urogram were almost normal. Urodynamic examinations revealed normoactive detrusor-overactive sphincter in case 1, and overactive detrusor-overactive sphincter in case 2. Cystometry-electromyogram demonstrated detrusor-external urethral sphincter dyssynergia in either cases. They were diagnosed as neurogenic bladder due to intraspinal tumor with spina bifida by myelography, computed tomography, and urodynamic examination. Laminectomy, subtotal extirpation of tumor and neurectomy of filum terminale were performed by orthopedists in both cases. The tumor was pathologically diagnosed as lipomas. Postoperatively they showed satisfactory recovery in micturition.

[Renal cell carcinoma with a long-term hemodialysis patient: a case report].

A 59-year-old man was found to have a renal cell carcinoma during about one hundred months of hemodialysis by means of an ultrasound tomography. Nephrectomy was done and renal cell carcinoma, clear cell type, with acquired renal cysts were observed, histologically. The usefulness and routine application of ultrasound tomogram are stressed for examination of the renal appearance during the management of hemodialysis.

[Percutaneous nephro-ureterolithotripsy in patients after urinary diversion or reconstruction using the intestine: report of 5 cases].

Five patients who developed urolithiasis after urinary diversion or reconstruction using the intestine were treated with percutaneous nephro-uretero lithotripsy (PNL). These patients were successfully treated, and we did not experience any severe complications. For the stone removal in this case, PNL seems to be superior to other open surgery methods.

[Clinical study of extracorporeal shock wave lithotripsy].

Extracorporeal shock wave lithotripsy was performed on 81 patients with urolithiasis (35 patients with ureteral stones, 25 patients with renal stones less than 2 cm in diameter, and 21 patients with renal stones more than 2 cm in diameter) at Kanbara Hospital from August to October, 1986. A 4 Fr catheter was placed transurethrally in the ureter up to the stone to identify the stone position and the degree of fragmented stones. In four patients with staghorn calculi, a double-J catheter was placed in the ureter to prevent stone street formation. More than 50% of the patients with renal stones less than 2 cm in maximum diameter and ureteral stones had satisfactorily excreted fragments or sand of crushed stones not later than 2 weeks after the operation. However, in patients with renal stones more than 2 cm in maximum diameter, it took much more time to discharge the crushed stones compared with the foregoing two groups and some patients needed further management to remove the remnants. Combined treatment, ureteral catheterization or endoscopic operation with ESWL is recommended for treatment of renal stones larger than 2 cm in diameter.

[Measurement of nerve conduction velocity of the dorsal nerve of the penis].

Tests for evaluating the function of the nervous system in patients with erectile impotence have been performed indirectly with the measurement of nerve conduction velocity in the extremities and other neurological procedures, since it has been technically difficult to measure the nerve conduction velocity of the dorsal nerve of the penis. Calculation of the nerve conduction velocity of the dorsal nerve of penis from values of the bulbocavernosus reflex latency was not reliable and the development of a technique to measure accurately the nerve conduction velocity of the dorsal nerve of penis has been awaited. Ten impotent men were studied, and a precise description was made about the procedure of measuring the orthodromic nerve conduction velocity of the dorsal nerve of the penis. The dorsal nerve of the penis was stretched optimally with a weight of 300 g, and its conduction velocity reached the maximal value. This new methodology is of use in the evaluation of erectile impotence.

[Extracorporeal shock wave lithotripsy of urolithiasis].

A total of 159 patients underwent 194 treatments with extracorporeal shock-wave lithotripsy for upper urinary tract calculi between August 4 and November 30, 1986. A single treatment was performed in 76% of the patients with renal stones and 90% of the patients with ureteral stones. Treatment was successful in 153 patients (93%). Seventy percent of the patients with renal stones and 95% of those with ureteral stones were stone-free over the one-month follow-up, while about 15% had small and asymptomatic fragments believed to be passable spontaneously. Only one patient required surgical removal of the calculi due to severe ureteral edema. Adjunctive urological management is required in about 9% of the patients preoperatively or postoperatively. The period of hospitalization averaged about 11 days after treatment and patients usually returned to work within a few days after their discharge. Extracorporeal shock-wave lithotripsy is the preferred form of management for symptomatic ureteral and renal calculi.

A macrocyclic antibiotic M-230B produced by Myxococcus xanthus. Isolation and characterization.

Myxococcus xanthus strain M516E produced at least three related antibiotics against Gram-positive and Gram-negative bacteria. From physico-chemical properties, a main component was identical to myxovirescin A and a second component, designated M-230B was found to be an antibiotic which is closely related to myxovirescin A. The structure of M-230B was determined from its physico-chemical properties, especially from 13C NMR spectrum as compared with that of myxovirescin A. The addition of alcohol, such as isobutyl alcohol, to the culture medium markedly stimulated production of the antibiotics.
