Trichophyton mentagrophytes granulomas. Unique systemic dissemination to lymph nodes, testes, vertebrae, and brain.

A man had Trichophyton mentagrophytes infection that ultimately involved the lymph nodes, testes, vertebrae, and CNS. It was associated with anergy and defective lymphocyte transformation to T mentagrophytes antigen. The patient was also anergic to repeated delayed skin testing with PPD and dinitrochlorobenzene. His lymphocyte responsiveness, as demonstrated by lymphocyte transformation to phytohemagglutinin and PPD, was substantially decreased. However, the results of an in vitro leukocyte migration inhibition assay showed that his lymphocytes were responsive to T mentagrophytes antigen as well as to PPD. The patient's serum IgE level was increased, but serum transferrin levels were persistently abnormal. Therapeutic attempts, including the systemic administration of griseofulvin, amphotericin B, clotrimazole, and transfer factor failed, and the patient died five years after systemic disease onset.

Cineroentgenographic and electromyographic studies of Eustachian tube function.

To determine the opening and closing process of the Eustachian tube during swallowing, the simultaneous recording of cineroentgenograms and electromyograms (EMG) was carried out in three monkeys. It was revealed that synchronous with the activity of the tensor veli palatini muscle, the cartilaginous tube opened for a very short time (ca. 250 ms) by an outward movement of its lateral wall. On the other hand, the pharyngeal orifice of the tube dilated for a relatively long time synchronous with the activity of the levator veli palatini muscle. These time lags between two parts of the tube seemed to help the pumping function of the tube.

[Antitumor activity of human interferons: their direct cytotoxic activity and clinical effects].

It was found that both interferon-beta and interferon-alpha had a direct cytotoxic action against a few sensitive cultured cell lines. The action was not concentration dependent, but time dependent. Interferons expressed their maximum effect when they were given daily for long time to nude mice bearing transplantable ascitic human cancer cell lines, indicating that interferons had schedule dependency. In preliminary phase II study of interferons (3-9 X 10(6) U daily im, iv or local use) to 40 patients with various malignancies, only one patient with B-cell lymphoma responded to systemic use of both interferons and two out of five patients with pleural malignant effusion of breast cancer also responded to intrapleural use of interferons. Side effects were usually mild, but included fever, weakness, gastrointestinal, hematopoietic, liver and kidney function disturbances.

Life of advanced cancer patients after knowing the nature of their own disease: a personal experience of seven cases.

To seven patients suffering from advanced cancer who wished to know what their disease was and whose families supported them in their wish, I informed them of the name of the disease and how advanced their case was. After being told they had cancer, not only did the patients become more cheerful to and more trustful of their families, but also they came to place greater confidence in us' doctors and nurses. Their wish to live was strong and they all wanted to have their anticancer therapy continued. At the terminal stage, four of them merely wanted to have their pain relieved, and two of them became depressive when the symptoms of approaching death became evident. But judging from the total clinical course of the seven carefully screened patients, I believe that informing them of the true nature of the disease brought about favorable results.

Minute hepatocellular carcinoma without appreciable change in size for seven years: a case report.

A small hepatocellular carcinoma found and followed in a Japanese man is described. He died of diabetic nephropathy and bleeding esophageal varices seven years after surgical biopsy of the liver that revealed hepatocellular carcinoma. Clinical and autopsy findings demonstrated no appreciable change in tumor size during the seven-year period during which no specific treatment was given for cancer.

[Two cases of carcinoid tumor of the stomach which responded to oral administration of UFT].

UFT was given to two patients with carcinoid tumor of the stomach and the effect of the drug was evaluated. The first patient was a 67-year-old female. She was admitted because of upper abdominal tumor. Exploratory laparotomy revealed gastric tumor and additional huge tumor with liver metastasis and peritoneal dissemination. Histology of biopsy specimens from gastric tumor and metastatic lesions was a composite type, of carcinoid tumor. Postoperatively UFT (600 mg/day) was given to the patient. During three months of the treatment the size of the large tumor reduced from 10 X 10 cm to 4 X 4cm. The second patient was a 55-year-old male. He was admitted because of severe diarrhea. Biopsy from gastric lesion and metastatic skin lesions revealed carcinoid tumor. After administration of UFT and Mitomycin C, metastatic skin lesions became smaller and some of the lesions disappeared. Two cases suggest a possibility that UFT may be effective for carcinoid tumor of the stomach.

Evaluation of Eustachian tube function by contrast roentgenography.

Contrast roentgenography and cineroentgenography were carried out on nine patients with perforated eardrums to reveal the opening process of the Eustachian tube and its pumping-like clearance function in human beings. The results were as follows: (1) the tubal lumen opens along the cartilaginous part of the tube owing to the outward displacement of its lateral wall; (2) the pumping function of the tube consists in two phases: accumulation of the fluid in the tube by tensor contraction and discharge of the fluid by tensor relaxation.
