[Statistical observations on patients with herpes zoster and postherpetic neuralgia (author's transl)].

From 1969 to 1980, 83 patients with herpes zoster and postherpetic neuralgia were referred to our clinic. They consisted of 36 male and 47 female, whose ages ranged from 18 to 91 years. They were tested mainly with stellate ganglion block or epidural block. Patients with postherpetic neuralgia received imipramine 60 mg per day in addition. About half of the patients were relieved of pain or considerably improved after treatment. Other half of the patients somewhat improved excluding those patients whose therapeutic effect could not be assessed. More favourable results were obtained in patients whose treatment was instituted within 2 weeks from onset of herpes zoster than patients over 2 weeks from onset. Duration of treatment was shorter in younger patients (approximately 59 years) than older patients (60 years approximately), but results were same in both age groups. About 10% of the patients had concomitant malignant disease or autoimmune disease. No relationship between the localization of herpes zoster and the site of malignant disease was found.

Neutralizing activity against Clostridium difficile toxin in the supernatants of cultured colostral cells.

Human colostral specimens were obtained from 60 Japanese postpartum women within the first 3 days after delivery. Neutralizing activity against Clostridium difficile toxin was evaluated with Y1 adrenal cells in miniculture. When Y1 adrenal cells were exposed briefly to the toxin, they showed a rounding response in culture, resembling that effected by Escherichia coli enterotoxin; however, preincubation of the toxin with aqueous phase of colostrum significantly reduced its cytopathic effect on Y1 adrenal cells. Of 60 colostral specimens, 17 samples had neutralizing activity against the toxin. Cell-free supernatants of colostral cells cultured for 7 days without mitogens contained significant amounts of both immunoglobulin A (IgA) and IgM, but very small amounts of IgG. Neutralizing activity of cell-free supernatants of cultured colostral cells was evaluated as described above. Neutralizing activity against the toxin was identified in five samples of culture supernatants out of 60 colostral cell specimens. In all five cases, the aqueous phase of colostrum also had a neutralizing effect against C. difficile toxin. Neutralizing activity against the toxin found in five supernatants of cultured colostral cells was completely abolished only by anti-human IgA antibody as assessed by immune precipitation.

[Halothane-pentazocine interaction in dogs (author's transl)].

It is known that narcotics reduce the alveolar concentration of inhalation anesthetics in man and animals. However the magnitude and duration of narcotic effect on inhalation anesthesia is not known. Accordingly, I determined in dogs the temporal effect of various doses of pentazocine, a commonly used anesthetic adjuvant, on the minimum alveolar concentration (MAC) of halothane. In addition, I compared plasma pentazocine concentrations in dogs both awake and during halothane anesthesia using an identical intramuscular dose of pentazocine. Intramuscular injection of pentazocine, group II (2.5 mg/kg), group III (5 mg/kg), group IV (10 mg/kg) reduced MAC of halothane required for anesthesia. The magnitude of MAC depression were 19.3% of control halothane MAC in group II, 36.4% of control in group III and 41.7% of control in group IV. Postinjection plasma concentration was fitted by computer with a 2-compartment open pharmacokinetic model. Plasma pentazocine concentration for awake (group I) and anesthetized (group II) dogs given the same dose (2.5 mg/kg) did not differ significantly on biological half-life, total apparent volume of distribution and body clearance. Halothane minimum alveolar concentration (MAC) was correlated to plasma pentazocine concentration (r= -0.60) and cerebrospinal fluid pentazocine concentration (r= -0.74).

Treatment of malignant tracheobronchial stenosis by Dacron mesh-covered Z-stents.

Z-stents partially covered with an expandable Dacron mesh into which epinephrine and thrombin were infiltrated, were used to reopen both mainstem bronchi, stenosed by bronchogenic carcinoma and to deliver hemostatic agents to the lesion. On follow-up 4 and 6 weeks later, airway patency was retained, the tumor surface was compressed, and the glossy surface of the stent suggested that it was covered with mucosa. For treatment of airway stenosis due to cancer invasion, a stent covered with a Dacron mesh sheet is thought to be effective in compressing the tumor and preventing tumor ingrowth into the stent.
