Intranasal allergen challenge during corticosteroid treatment.

The effect of a corticosteroid drug on type I reactions in the nose was studied in eighteen hay fever patients. They were treated with a daily dosage of 800 microng beclomethasone dipropionate intranasally for I week, in a double-blind, placebo-controlled cross-over trial. Allergen challenge on the last day of each treatment period showed that steroid treatment diminished sneezing and rhinorrhoea, but the effect was not statistically significant (P = 0-10). Immediate nasal blockage was unaffected. There is a marked discrepancy between the minor protection offered by beclomethasone dipropionate in type I allergy, as suggested in this study and the marked efficacy of the drug in the treatment of hay fever. It is suggested that two factors may account for this discrepancy. First, the significance of "delayed" reactions for hay fever symptoms. Second, the difference between a single allergen provocation and continuous exposure. It is suggested that a slight steroid inhibition of immediate reactions following allergen provocation is potentiated during natural exposure due to inhibition of the priming of the end-organ.

Scanning electron microscopy of a bacterial infection of the human nasal mucosa.

Scanning electron micrographs of a bacterial infection of the human nasal mucosa indicate that the surface attachment of bacteria to epithelial cells is of significance for their pathogeneity in the airways. It is suggested that further scanning electron microscopic studies of natural and experimental infected human nasal mucosa can give valuable information about the initial interaction between micro-organisms and host cell and add to our understanding of airways infections.

Histology of nasal polyps of different etiology.

The aim of the present study is to elucidate the correlation between etiology and histology in nasal polyps with special regard to cystic fibrosis (CF). Nasal polyps from 15 children with CF and a control group of non-CF polyps from 15 adult patients were examined by light-microscopy. The histological evaluation was carried out on a blind basis in order to avoid bias. Among the parameters used, the tissue eosinophilia proved to be the most valuable factor in the differentiation between CF and non-CF polyps, as only few eosinophils were found in the CF-polyps. The polyp glands were few and generally pathological. Some characteristics abnormalities in the grandular morphology are apparently more common in CF polyps. It is concluded, that the histological examination of nasal polyps is of importance for the correct classification of the patient, but the diagnosis of CF cannot be made based on microscopy of polyps. Further studies including blinded histological examination of nasal polypous tissue might contribute to a more differentiated diagnose of nasal polyposis.

Serial serum enzyme studies in infectious mononucleosis.

Serial examinations of the white cells in the blood and serial determinations of serum glutamic oxaloacetic transaminase (GOT) and serum lactic dehydrogenase (LDH) isoenzymes have been performed in 36 patients with seropositive infectious mononucleosis. The first blood sample taken after the onset of the disease showed an increased number of mononuclear cells in the blood in 90% of the cases, elevated GOT level in 70% and elevated LDH level in 100%. Mainly isoenzymes I, II and III accounted for the elevated LDH values, while only 35% of the patients had increased LDH V (liver fraction). The number of mononuclear cells in the blood was normalized in all patients after 5 weeks and GOT normalized after 10 weeks. On the other hand, half of the patients had increased LDH, particularly isoenzymes I, II, and III, 4 months after the onset of the disease. The results indicate that the liver involvement probably is slight and of short duration, while the proliferation of lymphoid cells in the tissues, probably responsible for the main part of the LDH increase, continues for a considerably longer period.

[Allergic rhinitis in children (author's transl)].

Some of the problems within the research field of allergology are discussed. The main topic is the new treatment of allergic nasal diseases with a local glucocorticosteroid. Beclomethasone dipropionate (Beconase) can be given intranasally without any risk of systemic side-effects. The drug is found very effective in the treatment of hay fever, vasomotor rhinitis and nasal polypi in adults. In a double-blind crossover trial with 19 hay fever children, aged 8-16 years, 18 preferred the active drug, none preferred the placebo and one had no preference. It is concluded that 400 mug beclomethasone dipropionate a day is a safe and very effective treatment of nasal hay fever symptoms in children.

Treatment of nasal polyps with intranasal beclomethasone dipropionate aerosol.

In a double-blind trial thirty-five patients with moderately-severe nasal polyposis were treated with intranasal beclomethasone dipropionate aerosol for 3 weeks. The dose given (400 mug/day) had only local effect on the symptoms. Judged by diary card scores the nasal symptoms were reduced to 52% of the pre-trial level for the whole group. Corrected for the placebo effect the percentage was 68. The reduction of symptoms was equally apportioned to the three symptoms, sneezing, nasal secretion and blockage. The treatment was tolerated well, and it is concluded that intranasal treatment with beclomethasone dipropionate aerosol offers most patients with nasal polyps a good response without any risk of systemic steroid side-effects.

ICI 74,917: a new anti-allergic drug administered by pressurised aerosol.

Ten patients with allergic rhinitis were challenged intranasally with allergen before, and half an hour, 2 hours and 4 hours after intranasal administration of placebo, disodium cromoglycate powder and an aerosol containing ICI 74,917, a new compound with anti-allergic action. ICI 74,917 had a maximum protective effect after half an hour, but was significantly better than placebo 2 hours after nasal use. The effect was significantly better than that of disodium cromoglycate powder. The results will encourage studies of ICI 74,917 in the treatment of allergic rhinitis.

Beclomethasone dipropionate aerosol treatment of hay fever. A dose-response investigation.

In a controlled, double-blind study 20 children and adults, suffering from summer hay fever, were treated intranasally with a daily dose of 200 mug, 300 mug or 400 mug beclomethasone dipropionate (Beconase, Becotide Nasal) or with placebo for 2 weeks during the hay fever season. No beneficial effect of the placebo treatment was observed. In patients treated with 200 mug and 300 mug beclomethasone dipropionate a day there was a moderate decrease in nasal symptom scores and in use of antihistamine tablets. As the results indicated 400 mug a day to have the most pronounced effect on nasal symptoms, this dosage is recommended for children as well as adults suffering from summer hay fever.

Beclomethasone dipropionate aerosol in treatment of hay fever in children.

Eighteen children suffering from hay fever were treated with intra-nasal beclomethasone dipropionate (400 mug/day) and an identical placebo aerosol in a double-blind cross-over trial. 17 of the children preferred the intranasal beclomethasone dipropionate, one had no preference, none preferred the placebo. The effect on the nasal symptoms was impressive. Symptom scores decreased, on average, to 12% and the number of antihistamine tablets taken to 18% of the pretreatment amount. Some beneficial effect on eye symptoms was also discernible, possibly due to an indirect influence from the nasal mucosa via the nasolacrimal reflex. Adrenal function was not affected. It was concluded that 400 mug beclomethasone dipropionate given intranasally daily for some weeks is an effective and safe treatment for hay fever in children.
