[Therapeutic cranial irradiation in patients with metastatic brain tumors from pulmonary carcinoma].

Fifty-three patients with intracranial metastases from pulmonary carcinoma were treated with cranial irradiation between 1976 and 1983. There was improvement in neurologic symptoms in 80%, and the one and two-year survival rates for the entire group of patients were 21% and 4%. Those or patients without metastasis tumor in other organs were 38% and 9%, respectively. It was suggested that the total dose should be in excess of 40 Gy. Histologically, adenocarcinoma and epidermoid carcinoma showed better prognosis than other types of cancer.

[The effects of PS-K on long-term survival of uterine cervical cancer patients treated with radiation].

The effects of PS-K on long-term survival of uterine cervical cancer patients treated with radiation was studied. The patients receiving PS-K were anxious about curability more or less at the time of completion of radiation therapy. Between 1977 and 1983, out of 96 patients with stage III, IV uterine cervical cancer treated with radiation, 27 were given PS-K as adjuvant therapy. The number of cases with PS-K administration were 15 (23%) out of 64 in stage IIIb, seven (37%) out of 19 in stage IVa and five (38%) out of 13 in stage IVb. Out of 27 patients administered PS-K, 19 were given the agent within three months after irradiation and the remaining eight after recognition of recurrence or metastasis. As a rule, PS-K was given orally, 3.0 g per day, intermittently in the pattern of two weeks per month. Judgement of the effects of PS-K was made in comparison with the survival rate of the patients with and without PS-K, and then by observation of the changes of the PPD skin test, ESR, appetite and body weight as early effectiveness of PS-K. The results were as follows. Obvious prolongation of survival was recognized in the patients with PS-K after irradiation. Namely, the cumulative five-year survival rates of stage IIIb were 65% and 49% with and without PS-K, respectively. Improvement by PS-K as early effectiveness was obtained in seven cases (37%) out of 19. The conditions necessary for long-term survival with PS-K were thought to be as follows: that the tumor almost disappears as a result of irradiation and that the condition of the host is superior to that of the tumor in the tumor-host relationship. The effectiveness of intermittent administration of PS-K was suggested.

[Possibility of complete eradication of early esophageal cancer by radiotherapy alone].

Twenty-five patients with esophageal cancer less than 5 cm in length receiving radical radiotherapy alone were studied to consider the radicality of this therapy for early esophageal cancer. The relative five-year survival rate for the 25 patients was 41%, which was better than 23% for 60 patients receiving radical radiotherapy and many reported results. And for 11 stage I patients, a five-year survival rate of 79% could be obtained. Summing up Japanese results reporting survival rates by tumor length revealed that crude five-year survival rates were 21.5% for tumors less than 5 cm and 4% for tumors more than 5 cm long, respectively. Radiotherapy can be considered to be among the effective modalities for the treatment of early esophageal cancer.

[CT and pathologic studies on detecting hepatic involvement of malignant lymphoma].

In order to determine the value of CT in detecting lymphomatous involvement of the liver, 57 autopsied cases (48 cases of non-Hodgkin lymphoma and 9 of Hodgkin's disease) were analyzed in detail. Additionally, 90 recent cases of abdominal CT with liver study in the initial staging were reviewed and 6 cases in which the liver had been examined within 2 weeks before autopsy were compared for macroscopic and pathologic findings. The following results were obtained. Autopsy findings in 57 cases of malignant lymphoma. (1) Hepatic involvement was demonstrated in 56% of the patients with non-Hodgkin lymphoma and in 67% of those with Hodgkin's disease. Of these patients with hepatic involvement, intrahepatic nodular lesions over 1.0 cm in diameter were macroscopically identified in 22% of the cases of non-Hodgkin lymphoma and 33% of those with Hodgkin's disease. Lymphomatous involvements were seen in many organs in the cases of hepatic involvement. CT examinations of 90 cases for the initial staging showed no significant abnormality in the liver or spleen. Of the 6 cases in which the liver was examined by CT within about 2 weeks before autopsy, lymphomatous involvement was pathologically diagnosed in 5. Of these, only one having multiple nodules larger than the tip of a thumb was correctly diagnosed by CT. Hepatic involvement of lymphoma was thought to be the end stage, and minimal or early disease was detected in the region of Glisson's capsule. The above findings indicate that early hepatic involvement is not likely to be detected by CT at the present time.

Bioconversion of arachidonic acid by human uterine cervical tissue and endocervix in late pregnancy.

Prostaglandins (PGs) may play an important role on cervical ripening in late pregnancy, namely cervical dilatation and softening. To investigate this, arachidonic acid metabolites of cervical tissue and endocervix were studied. To separate and identify the metabolites, silicic acid chromatography, thin layer chromatography, reversed phase chromatography, gas-liquid chromatography and GC-MS were used. In cervical tissue, arachidonic acid was converted to 6-ketoPGF1 alpha, PGF2 alpha, PGE2, thromboxane B2, and 12-HETE. In endocervix, arachidonic acid was converted to PGF2 alpha, PGE2, thromboxane B2, 12-hydroxy-5, 8, 10-heptadecatrienoic acid, and 12-HETE. There was no relation between the arachidonic acid conversion rate and the Bishop score (points of cervical ripening).

Bioconversion of arachidonic acid to prostaglandins and related compounds in human myometrium and uterine cervix.

Arachidonic Acid metabolites in human myometrium and uterine cervix were studied using silicic acid column chromatography, thin layer chromatography, reversed phase partition chromatography and gas-liquid chromatography. Myometrium produced 6-ketoPGF1 alpha, PGF2 alpha, PGE2, thromboxane B2. Uterine cervix produced 6-ketoPGF1 alpha, PGF2 alpha, PGE2, thromboxane B2, and one hydroxyacid. There was no difference between the rate of conversion of prostaglandins in myometrium and cervix. But only cervix could convert arachidonic acid to hydroxyacid.

[Three cases of primary lung and mediastinal malignant germ cell tumors --clinical and pathological studies].

This is a clinical and pathological study of malignant germ tumors in the lung or mediastinum. Germ cell tumors may be considered to originate from primitive germ cells with totipotency. One of 3 cases showed clinical and pathological findings suggesting that a part of the tumor differentiated into many kinds of tissue, such as growing and metastasizing. Initial diagnosis was performed radiographically and pathologically with difficulty. But finally, the first patient was diagnosed as malignant teratoma, the second as yolk sac tumor and the third as choriocarcinoma, in consideration of the tumor markers and clinical process.

[Effects of PS-K as adjuvant therapy for primary lung cancer--an autopsy case].

A 57-year-old woman underwent left lower lobectomy for primary lung cancer (papillary adenocarcinoma, T2NOMO). Despite postoperative adjuvant immunochemotherapy with 5-Fu, Carboquone and PS-K, bone metastases were recognized at 9 months after the surgery. She received radiation therapy and was administered PS-K alone, 3.0 g daily. As a result, long-term good condition was maintained with a positive PPD skin test. The bone metastases developed slowly, and the patient survived 5 years and 8 months after the operation. At autopsy, no obvious metastatic lesions were identified except for the bone metastases. The cause of her death was thought to be acute renal failure due to severe hemorrhage from an esophageal ulcer. Moreover, she was doomed because of advanced polycystic disease.

[A method for determination of prostaglandins in human seminal fluid].

Human seminal fluid has been shown to contain mainly eleven different prostaglandins. Methods for quantitative determination of prostaglandin E and 19-OH prostaglandin E are described. The methods include extraction with acetone and alkali treatment. After alkali treatment prostaglandin E was transformed to prostaglandin B, and 19-OH prostaglandin E to 19-OH prostaglandin B. Prostaglandins were separated by using Lipidex column chromatography and silicic acid column chromatography. Prostaglandin B was quantitated by gas chromatography using C24 fatty acid methyl ester as internal standard. 19-OH prostaglandin B was quantitated by measuring ultraviolet photoabsorbance at 278 nm.
