[Quantitative measurement and clinical analysis of serum levels of immunosuppressive acidic protein (IAP) in hematopoietic malignancies].

Serum levels of immunosuppressive acidic protein (IAP) in 105 patients with hematopoietic malignancies, there were 12 cases of acute myeloblastic leukemia, 1 acute monocytic leukemia, 13 myelomonocytic leukemia, 4 acute promyelocytic leukemia, 26 chronic myelogenous leukemia, 22 non-Hodgkin's lymphoma, 5 Hodgkin's disease, 6 adult T-cell leukemia, 5 acute lymphoblastic leukemia, 3 chronic lymphocytic leukemia, and 8 multiple myeloma. High levels of serum IAP were detected in all of the patients except chronic phase of CML, malignant lymphoma in stage I and II, and multiple myeloma. In the cases of malignant lymphoma, serum IAP levels in stage III and IV were higher with statistical significance (p less than 0.01) than those in stage I and II. Serum IAP levels in the patients with CML in blastic crisis were higher than in the chronic phase, so serum IAP levels are useful as one diagnostic parameters in blastic crisis. However, in patients with ANLL in relapse, serum IAP levels showed normal values. Serum IAP levels paralleled those of acute phase reactants such as alpha 1-acid glycoprotein , C-reactive protein, alpha 2-globulin, and alpha 1-antitrypsin, and had inverse correlations with PPD and PHA skin test.

The effect of streptozotocin-induced diabetes on prolyl hydroxylase activity in rat gingiva.

The diabetes resulted in marked decrease in prolyl hydroxylase (EC 1.14.11.2) activity in rat gingiva. Insulin treatment of diabetic rats returned the decreased enzyme activity to the level of normal controls. The results suggest that diabetes decreases the rate of collagen biosynthesis in gingival tissue by inhibiting the hydroxylation of peptidyl proline in collagen precursors.

Effect of dibutyryl cyclic AMP on collagen synthesis in a clonal osteoblast-like cell line derived from newborn mouse calvaria.

The effects of dibutyryl cyclic AMP (DBcAMP) and related compounds on collagen synthesis in a clonal osteoblast-like cell line, MC3T3-E1, were investigated. The addition of DBcAMP to cultures increased the hydroxyproline content of the cells. It also enhanced the incorporation of labeled proline into collagen and elevated the activity of prolyl hydroxylase, an enzyme involved in collagen synthesis. These effects were observed at concentrations of 0.1 to 2 mM DBcAMP. 8-Bromo cyclic AMP also increased the hydroxyproline content of the cells, while sodium butyrate and dibutyryl cyclic GMP had no such effect. These results suggest that the intracellular accumulation of cyclic AMP in osteoblasts leads to their active production of collagen, a major component of the organic matrix of bone.

Preventive and therapeutic effects of gliclazide on diabetic retinopathy: comparison with glibenclamide treatment.

The effects of long-term treatment with gliclazide on diabetic retinopathy and platelet adhesion were investigated. The subjects comprised 25 patients with NIDDM, of which 12 cases and 13 cases were treated with gliclazide and glibenclamide respectively. An improvement in retinopathy was observed in 7 out of 12 cases in the gliclazide-treated group. Of the remaining 5 cases, four cases remained unchanged and one deteriorated. Of the 13 cases treated with glibenclamide, on the other hand, only two cases showed an improvement in retinopathy after treatment, 7 cases remained unchanged and 4 cases deteriorated. The values for the platelet adhesion test following 3 years treatment were 52 +/- 14% and 42 +/- 17% in the gliclazide and the glibenclamide-treated groups respectively. These results were not statistically significant. These data indicate that gliclazide might be more effective than glibenclamide with respect to either improving diabetic retinopathy or preventing its progression.

Adult T-cell leukemia on the east coast of Kii Peninsula--presentation of an anti-ATLA-negative case.

Nineteen patients with adult T-cell leukemia (ATL) have been found in the last seven years along the east coast of Kii Peninsula in Japan. The leukemic cells were of the immunologically inducer/helper T-cell phenotype. The prognosis was very poor (median survival time, 85 days), and most of the patients had fatal complications of pulmonary infections. Antibody against ATL-associated antigen (anti-ATLA) was detected in sera from 9 of 10 patients who were born along the coast. However, it was not detected in one patient who was born in a district surrounded by mountains. Although he had neither superficial lymphadenopathy nor skin lesions, he showed rapid clinical deterioration. His leukemic cells appeared to be extremely bizarre with marked nuclear deformation compared with those of the other patients. In surface marker studies the leukemic cells reacted positively with OKT3, OKT4 and OKIa-1 monoclonal antibodies. The characteristics of the anti-ATLA-negative case are discussed in comparison with the other ATL cases.

[Clinical experience with cefoxitin in infections associated with hematopoietic disorders].

Ten inpatients at the Second Department of Internal Medicine, Mie University Hospital, developed infections in the course of treatment for hematopoietic disorders and were administered cefoxitin (CFX). Patients suffered from the following infections: pharyngitis, 2; bronchitis, 2; pneumonia, 2; sepsis, 2; bacteremia, 1; suspected cases of bacteremia, 2; and fever of unknown origin, 1. The number of infections totaled 12 as 1 patient with pharyngitis also developed sepsis and 1 patient with pneumonia developed bacteremia. Duration for the administration of CFX ranged between 5 and 18 days with a total dosage of between 30 and 108 g. Of the 10 patients treated with CFX, 9 were treated concomitantly with micronomicin (MCR), doxycycline (DOXY), or sulbenicillin (SBPC), some were treated concomitantly with only 1 of the drugs and some were treated concomitantly with 2 of the drugs. The following clinical results were obtained: Following treatment, 4 patients were considered "excellent", 5, "good", and 3, "poor". Clinical efficacy rate was 75%. Four strains of Gram-positive cocci (1 strain of S. aureus, 2 strains of S. epidermidis and 1 strain of Streptococcus sp.) and 3 strains of Gram-negative rods (2 strains of P. aeruginosa and 1 strain of E. cloacae) were found in the clinical specimens of the 10 patients. These results differed somewhat from reported data that Gram-negative rods such as E. coli, Klebsiella sp., Pseudomonas sp., Serratia sp., are dominant. No serious side effects requiring cessation of treatment were observed. Elevations in the levels of S-GOT, S-GPT, serum alkaline phosphatase, blood urea nitrogen, etc. were observed.(ABSTRACT TRUNCATED AT 250 WORDS)

Plasminogen activator activity in the subunits of mouse submandibular gland nerve-growth factor and epidermal growth factor.

The 7S nerve-growth factor (7S NGF), high molecular weight epidermal growth factor (HMW-EGF) and their subunits were isolated from mouse submandibular gland. Plasminogen activator activity was found in the 7S NGF and HMW-EGF, as well as in the gamma subunit and EGF-binding protein. Later activities were two- or three-fold higher than those of original growth factors. The alpha subunit, beta subunit and low molecular weight EGF showed no activity. The gamma 1, gamma 2 and gamma 3 subunits of 7S NGF showed plasminogen activator activity almost equal to that of the original gamma subunit.

Effect of epidermal growth factor on collagen synthesis in osteoblastic cells derived from newborn mouse calvaria.

We investigated the effect of epidermal growth factor (EGF) on collagen and protein synthesis in clone MC3T30-E1, a cell line which retains osteoblast-like characteristics. EGF at concentrations of 2-50 ng/ml significantly the hydroxyproline content of the cell layer. These effects were completely abolished by the addition of anti-EGF rabbit serum. The addition of indomethacin did not affect these EGF-induced effects. Collagen fiber formation was also reduced by EGF; a fine and unstriated type of fibril was detected compared to the typical cross-striated fibrils seen in control cultures. EGF at concentrations of 2-50 ng/ml significantly decreased collagen synthesis in the cells, whereas protein synthesis was rather stimulated. Thus, the proportion of collagen to protein synthesized decreased markedly with increasing concentrations of EGF. Unrelated to its effect on collagen synthesis, EGF at concentrations of 0.4-50 ng/ml significantly increased the activity of prolyl hydroxylase, an enzyme involved in the biosynthesis of collagen. Since the plasma concentration of EGF in humans is sufficiently high to cause the observed effect, osteoblasts in vivo may be responsive to this peptide in the same manner as those observed in vitro.

Elevation of glucosamine 6-phosphate synthetase activity in bleomycin-induced pulmonary fibrosis in hamsters.

The single intratracheal instillation of bleomycin sulfate (0.5 mg (potency) per 100 g body weight) in hamsters rapidly increased the activity of glucosamine 6-phosphate synthetase (EC 2.6.1.16) of the lung, a major regulatory enzyme for the synthesis of acidic glycosaminoglycan (AGAG). The activity increased as early as day 2, reached maximum level at day 10, then decreased and returned to the control level at day 45. The content of AGAG was also increased by bleomycin treatment, but the increase of AGAG followed the elevation of the enzyme activity. These results suggest that the early elevation of glucosamine 6-phosphate synthetase activity is closely related to the accumulation of AGAG in the fibrosing lung caused by bleomycin.

Lethal factor in the male mouse submandibular gland.

The presence of a toxic component (lethal factor) has been reported in the male mouse submandibular gland (SMG). This study was designed to clarify whether the toxic component was secreted into saliva or not and to investigate some characteristics of the component. Not only SMG extract but also phenylephrine-induced saliva from male mice was highly toxic to guinea pigs, rats, hamsters, and mice. The toxic activity of the saliva was found in all strains of male mice examined. In female mice, the toxic activity was extremely low. One of the toxic components in the saliva was purified by isoelectric focusing and DEAE Sephadex A-50 chromatography and identified with a kallikrein-like enzyme. The activity of the enzyme markedly increased in saliva secreted to alpha-adrenergic agents but not to beta-adrenergic or cholinergic agents. These results indicate that the lethal factor in the male mouse SMG is an exocrine protein with Kallikrein activity and its secretion into saliva is controlled through alpha-adrenergic receptors.
