[Psychosocial evaluation: can prematurity in a primipara be prevented?].

Consideration of psycho-social factors in monitoring pregnancy, especially in a gravida I patient, permits predicting an increased risk of unfavorable results. Research conducted in the Rhone-Alpes region concerns specifically the risk factors for prematurity related to the psycho-social environment of the woman, for the purpose of long-term prevention, i.e., from the beginning of the pregnancy. The prematurity rate remains much higher in women under 20, in foreign or French women living overseas, in unmarried women living with someone or alone, and in women of low socio-cultural level. A profession does not, per se, constitute an increased risk factor for prematurity, since women staying at home present a higher prematurity rate than that of working women. However, tiresome working conditions constitute an increased factor for prematurity. Several factors related to living habits are evaluated. Finally, the role of psychological modifications related to the pregnancy is also considered, and appears to be important, especially in women without any other known medical risk factor. In view of these results, the authors present a number of suggestions for a better prevention of prematurity.

[Construction of questionnaires for the study of psychological components in the etiology of somatic diseases. The example of prematurity].

A study whose aim was to define the role of psychological factors in prematurity etiology has led us to develop a methodology for evaluation of personality which could have a connection with the occurrence of a premature birth. The proposed method--interviews based on free association followed by the construction of questionnaires serving as a base for an epidemiological study--seem to us applicable in order to show the psychological components which intervene in the occurrence of a somatic illness.

[Maternal and perinatal antecedents of encephalopathies: their contribution to a trial classification of encephalopathies of unknown origin].

We had a sample of 135 mentally retarded children, whose case history (maternal, obstetrical and perinatal) was known before their mental retardation was suspected. This analysis made it possible to pinpoint characteristic anamnesic giving rise to mental retardation by comparison with a check population. It appears that these factors differ according to the supposed origin of mental retardation (genetic, embryonic, foetal, perinatal and postnatal periods) therefore, they can be considered "explicative" of these origins. A multivariate analysis of these factors indicates that the majority of cases of mental retardation with unknown origin are due to disorders during foetal life, which had not been detected.
