Prematurity and occupational activity during pregnancy.

The aim of this study was to determine the elements of fatigue in occupations which constitute possible risk factors for the course of a pregnancy, and, in particular, that could cause premature birth. In 1977-1978, a total of 3437 women in France were surveyed after giving birth in two maternity hospitals. One hospital was located in Lyon (a large city) and the other in Haguenau (a small town). Among 1928 working women, it was found that certain occupational categories are more prone to risk of prematurity than others. The authors carried out an analytical breakdown of the job into its diverse components which led them to define five sources of fatigue and to construct an index capable of detecting the strenuous working conditions. There is a significant relationship between the prematurity rate and the number of high fatigue scores observed in the job. When the number of scores varies from 0 to 5 the rate of premature births increases from 2.3% to 11.1%. This relationship remains significant after controlling for confounding factors.

[A prospective study of malignancy criteria in 407 surgical patients (author's transl)].

A prospective study of 407 patients with "cold" solitary thyroid nodule seen over 4 1/2 years and systematically operated upon was undertaken with the view of determining the pre-operative criteria of malignancy. Thirty-nine nodules were malignant. Since various methods of multidimensional statistical analysis failed to give satisfactory results, an attempt was made to base rational surgical indications on purely descriptive criteria. Neither the patient's age, morphology, personal or family history of thyroid disorders, nor the size of the nodule, the circumstances in which it was discovered and the presence of calcification made it possible to distinguish between malignant and non-malignant nodules. Cervical lymph node involvement, paralysis of the recurrent nerve, previous irradiation of the neck and site of the nodule equally had no discriminative value. Only two clinical findings, softness of the nodule and liquid content, were clearly associated with the absence of malignancy.

Enzyme-linked immunosorbent assay for anti-histone antibodies and their presence in systemic lupus erythematosus sera.

We describe an enzyme-linked immunosorbent assay (ELISA) with adsorption of histones (total and fractions) on glass beads and saturation of excess sites with sheep serum. The anti-histone antibodies are detected with peroxidase conjugate and developed with Trinder's reagent which has great stability. This very sensitive method detects anti-histone antibodies in 53% of SLE patients and in virtually no other diseases. Positive reactions are observed only with total histones and fractions H1 and H2b.

[Viral hepatitis: incidence and epidemiologic pattern in an urban area (author's transl)].

In order to describe the incidence and the epidemiologic pattern of viral hepatitis (V.H.) in a French urban area, with a special attention to ambulatory cases, an epidemiological information system has been developed during one year. The physicians (sample), the medical laboratories and, at a lesser degree, the hospital care units, have taken part in this work. The incidence rate of ambulatory cares has been estimated 85 cases/10(5) persons (69/110), less than previously assumed. Non-B V.H. remains more frequent (52 cases/10(5) persons), but B.V.H. are rather close to them (33 cases/10(5) persons). Large differences exist between geographic areas, without evident rational. Higher incidence rates characterize children (especially 5-9 years) and migrant people (especially from North Africa): those facts are completely account by very high rates within the subgroup of migrant children (473 and 260 cases/10(5) persons). The reported patterns allows us to point on this high risk characteristic of migrant's children and to assume an important modification in the pattern of local inhabitants V.H. in the region. These trends have to be followed by further studies.

[An allometric study of the relative growth of the head and the body of the fetus and of the new born (author's transl)].

An allometric study of the relative growth of the head and the body of the fetus and of the newborn shows that there is a more rapid growth of the head in relationship to the length of the fetus before the 30th week of pregnancy and then there is a slowing down between the 30th and the 40th weeks, followed by a re-acceleration after birth for full term infants. This phenomenon is not shown in premature babies born before the 30th week of pregnancy who continue their head growth with the same speed after birth and slow down only for a short time near their theoretical term. These results led to a hypothesis being formed that cephalic growth is slowed down in the uterus because of steric (molecular) congestion, a slow-down that is slightly demonstrated in breech presentations and much later in very premature babies. This opens the way to a discussion on the final results of this phenomenon on the brain.

[Weight, height, skull and chest circumferences, biparietal diameter in the newborn infant as a function of gestational age, weight, height and weight gain of the mother].

The weights, heights, skull and chest circumferences and biparietal diameters were measured in 7,905 newborns in 1972, in the 5 maternity hospitals of Lyon. These data were studied with respect to the gestational ages of the children and according to the usual weights, heights and weight gains of the mothers. The evolution of these parameters was compared to each other; tentative explanations are given.

[Systematic registration of malformations].

This study concerns 727 cases of deformed children born in the Rhône-Alpes region in 1973. 61 maternity hospitals, out of 181 in the region, took part in the study. Data concerning sex, mortality, mother's age, age of gestation, gemellity, body weight and length, as well as frequency of genetic findings in the families are published. A classification of the types of malformation in relation to their frequencies is reported.

[Diagram for the prognosis of cephalo-pelvic disproportions. Application in 300 cases of pelvic contraction].

The authors using the information obtained by X-ray pelvimetry and ultrasound cephalometry in 300 cases of contracted pelves, suggest a diagram of cephalo-pelvic disproportion that can be used in the great majority of cases of contracted pelves. It is not applicable to those where there is significant deformation of the middle strait or an abnormality of presentation. It is divided into three zones: 1. A superior zone, favourable, into which the majority (60%) of cases of contracted pelvis fit. The risk of a serious dystocia during delivery is so small (less than 2%) that it can be considered scarcely different from those found in women with a normal pelvis. 2. An intermediate zone into which 24% of cases of contracted pelvis fit where the prognosis is uncertain. The labour should be considered as an "at risk" labour and a trial of labour conducted. 3. A lower zone into which 16% of the cases fit and which consists of two categories: --those in which the biparietal diameter is equal to or less than 90 mm. Prophylactic caesarean section should be avoided in these cases where disproportion is rare and where there is a risk of delivering a premature infant: and --those more numerous (9 out of 10) where the biparietal diameter is more than 90 mm and where, unless there is a clinically definitely favourable prognosis, it is justifiable to carry out prophylactic caesarean section. If this line is followed: one can avoid a large number of trials of labour, all of which are inconvenient and some of which are dangerous for the child; few unnecessary caesarean sections are carried out.
