[The clinical importance of atrial fibrillation-flutter induced by electric transesophageal stimulation of the heart].

A study was made of the clinical importance of atrial fibrillation-flutter (AFF) induced by using different modes of left atrium stimulation via the esophagus. Ninety-eight patients were entered into the study including 40 patients with a history of AFF paroxysms, 24 with risk factors of AFF development, and 27 practically normal persons. The stimulation modes applied permitted reproducing stable paroxysms of AFF in 85 percent of the patients with a history of arrhythmias and in none of the normal persons. As for the patients with risk factors, stable paroxysms of AFF could be induced in 33 percent of the cases. The specificity and sensitivity of transesophageal electrophysiologic stimulation (TEES) with the modes applied were 100 and 82 percent, respectively. The investigations have demonstrated that TEES appeared most effective when applied in the mode of an even increase of the frequency of the set pace up to 300 imp/min.

[Sinoatrial reciprocal paroxysmal tachycardia].

Detection rates and features of the clinical course of paroxysmal reciprocal sinoatrial tachycardia were examined in 34 patients. The features of the tachycardia should include its short-term and less marked clinical manifestations. In tachycardia, mean heart rate was 118 +/- 13 per minute. Long-term ECG monitoring was found to be the most adequate tool for the diagnosis of paroxysmal sinoatrial tachycardia. The potentials of transesophageal electrostimulation of the heart in the diagnosis and assessment of the clinical significance of that tachycardia were demonstrated to be limited due to instability of induced paroxysms.

[Status of the pituitary-adrenal and cholinergic system of patients with a pain syndrome in spinal osteochondrosis before and after a course of reflexotherapy].

The pituitary-adrenal and cholinergic systems were studied in 57 patients with chronic pain related to vertebral osteochondrosis before and after an acupuncture course. The degree of the therapeutic effect of electroacupuncture (EAP) was found to correlate with the nature of changes in the level of the spontaneous secretion of aldosterone and intensity of the process of acetylcholine (AC) release. Most patients in whom the treatment was followed by the recovery of the physiologic level of the initially depressed secretion of aldosterone and a clear-cut decrease in the initially elevated AC concentrations showed good clinical response. In patients in whom the initially elevated levels of aldosterone and AC failed to decline or in whom there was an even greater increase in AC concentrations, the therapeutic effect of EAP procedures was insignificant and short-lived.

[Arrhythmogenic effect of anti-arrhythmia agents in patients with paroxysmal supraventricular reciprocal tachycardia].

A retrospective study of the results of consecutive testing of antiarrhythmic agents was carried out in 82 patients with paroxysmal reciprocal supraventricular tachycardia. Arrhythmogenic effects of anti-arrhythmic drugs were detected in 22 (6.1%) of 359 pharmacologic tests. The electrophysiologic basis of arrhythmogenic effect is delayed impulse conduction at some point of the blood flow pathway, conducive to the emergence or maintenance of the re-entry phenomenon.
